EE reached with left hand to stop a bag coming down conveyor chute and felt something pop in his left elbow. A visit to doctor on 09/25/2009 revealed a strained tendon and nerve as it was pushed across a bone spur already present at this site. EE was put on light duty until October 1, 2009. No days of lost work are expected.

Edoc 154045 corrects the date of injury to 01/04/2005 from 01/01/2005. Surgery was eventually performed on 02/11/2005: 5 days lost time and on light duty until spring - update will follow.
THE EMPLOYEE WAS STACKING 50 POUND BAGS ON A PALLET WHEN HE FELT A SUDDEN PAIN IN HIS UPPER BACK BETWEEN THE SHOULDER BLADES. HE VISITED THE DOCTOR, MISSED ONE DAY OF WORK AND IS ON LIGHT DUTY FOR 5 DAYS.
EE WAS WRAPPING A CHAIN AROUND ONE END OF A SCREW WHEN A FELLOW EE LIFTED THE OTHER END, CAUSING THE SCREW TO ROLL TO THE SIDE AND ONTO EE'S RIGHT HAND. X-RAYS AT THE DR'S OFFICE REVEALED NO B ROKEN BONES OR ABNORMALITIES. EE MISSED 3 DAYS OF WORK.
