Miner was shoveling rock into buckets at the tail end of the limestone tunnel and hauling the buckets outside the tunnel to dump. Miner hurt their lower back in the process.

Employee was pushing forks together on a telehandler when EE felt a pain in EE's left bicep.

Employee was unfamiliar with this type of Freight Elevator and while closing the elevator door, smashed EE's ungloved hand between the doors.

Employee did the line change on the 6" line to set up in cement Silo, then came down from Silo to inspect Kiln grease system and proceeded to clean additive system shoveling material and pain got worse. EE attempted to sit in the 320 loader, but the pain intensified. EE immediately reported it to the control room to report it to the supervisor.

Pulled plate from hopper to dump, slipped from hand and fell on foot.

Contractor was rolling a conveyor chute out of the way and hand was pinched between the chute and the wall cutting EE's left pinky finger.

While installing a coal mill top, the lid got stuck and the employee put fingers between the top and the mill and EE shook it. When the lid became unstuck, it came down and smashed EE's fingers.

While pulling blades from a separator, employee felt sharp pain in knee.

While cutting an angle in a piece metal, the machine cut the metal and causing it to pop up and cut my thumb. Resulting in a laceration needing stitches to close.

Employee had a hole in their pants. Finished products of GCC (cement)got into employee's pants and burned their leg.

While pulling a probe, felt pain in upper back, shoulder area.

When EE pulled #2 Kiln Analyzer Probe, EE pulled something in upper back shoulder area.

The bowls used for making pellets for the XRF/XRD are very heavy and require repetitive motion with hands and wrists. Over a 12 hour shift 40+ pellets are required and pain has increased over several months.

While adjusting a rotation switch sensor, the employee dropped the screwdriver. When EE reached to grab it, EE's finger was caught by the rotating target.

Employee was working on silo load spout. He climbed on top of case used to position spout and was trying to align spout from bottom. He caught his fingers in between spout and plunger smashing both sets of fingers.

Employee pulled a retractable line on a fall prevention yoyo and injured his left middle finger. He reported it at about 11:00 am. He was wearing all PPE including gloves. Employee declined medical treatment. Then at about 3:25 pm he said he wanted to go to the doctor as a precaution.

Employee attempted to lift a 3'X3"it up over his head and felt a pop in his right bicep. He lowered the sheet to the floor and felt a sharp pain in his arm and it cramped. He was taken to emergency room with no diagnosis and told to make an appointment with another doctor. He was told on the 20th he would need surgery.

Contracted employee was cleaning the tower and was looking up into the tower for buildup when the tower puffed out. Dust was blown underneath his face mask. Employee entered the control room and stated he got burned. He had already flushed the area with water and applied wet rags. Employee was transported to the emergency room.

A piece of material got into employee's eye. The material got past his PPE that he was wearing (safety glasses with side shields and a face mask that is connected to the hard hat he was wearing).

Employee was cleaning the tower. The dust on his hard had fell inside his shield. When he looked up it fell inside his safety glasses. Eyes became irritated.

Cutting holes to clear material from baghouse screws. Dust on cutting shield got into left eye when shield was lifted up.

EE was helping to stand a mobile loader spout up and it came together and smashed his finger

Moving crane pad felt pain in his bicep and forearm.

Using a shovel to move Gypsum and reaggravated same finger that had been previously fractured in the same spot

Employees were investigating a possible material plug. One employee opened a port when material shot out and hit him. The other employee was standing a couple feet behind and was hit with dusty material.

Moving a piece of 1/4 inch plate to access with toolcar and while trying to slide plate, braced arm, elbow against knee and while pushing, heard a pop in elbow.

EMPLOYEE STRAINED BACK.

Employee was disassembling a motor coupling when the coupling ring fell smashing fingers between ring and shaft. Employee had contusion to finger.

EE was clearing material on a belt. He called Control Room (C.R.) to restart the belt. Belt started, but soon shut down. C.R. called the employee but got no response. A 2nd employee found the 1st employee semi-conscious. Employee was airlifted to a local hospital. Employee suffered a right arm amputation at the elbow and numerous fractures and broken bones in his torso.

Employee was stacking cement sacks when he felt slight pressure on groin area.

EE was replacing a bearing, when it fell out of a face and struck him in the nose.

Employee was pulling up on the rooster tail of the jib on the crane, when his elbow popped.

Employee was climbing down a ladder when he felt a pain in his back. At the time, he did not feel he required medical attention and continued to work. On 10/24/2008 he was lifting 18lb blocks, when he felt the original injury became aggravated and sought medical attention on 10/27/2008. His first lost time day began 10/27/2008 and his return to duty information is unknown.

Two employees were moving an airslide when one of the employee's hands slipped, causing his finger to be caught between the lower section of the airslide and the grating. Employee was wearing gloves when the injury occurred.

Ee was holding heavy equipment when it shifted, catching his left hand against an electrical box.

The employee was checking the directional rotation of a motor by pulling on the belt. When the employee grabbed the belt again, it was still spinning. The pinky and ring fingers got caught between the belt and the pulley.

Employee picked up a metal portable panalytical table top x-ray machine to move it, when a piece of metal on the underside of the machine sliced his finger, resulting in 4 sutures.

The employee was installing terminations on motors and instruments on the sixth floor of the preheater. At the end of the shift, the employee's right eye was irritated and reddish color. The employee stated that he did not feel anything enter the eye.

Employee was opening pin gates in sequence and caught finger between handle of gates.

Employee was restacking auxiliary separator fan blades. The blades shifted and pinched his middle finger on his right hand, cutting through his glove. He received stitches to this digitus medius at the local Occupational Medicine Clinic and was at work the next day.

Employee was guiding a cable and his hand was pulled into the pulley by the cable.

Employee was assisting with the removal of a spring loaded support arm for a vent system. When the support arm was lifted out of its base, the spring released its stored energy and collapsed, pinching the employee's ring and little finger. Employee was taken to an occupational health center and received stitches for cuts on the 4th and 5th digits of his right hand.
