Employee was hanging 50lb empty bags on loading spout and felt a pop/pull in EE's right forearm. Massaged it for a while and continued working thru EE's shift. Came back the next day worked half a shift doing same job then changed jobs for 2nd half. Not sure of damage, was told to follow up with an orthopedic doctor.

Driller and drill helper were attempting to take off drill bit from pipe when drill pipe unscrewed from top head and fell down on helper

EE was caring a bag of sand on his shoulder. A wind gust blew a sand particle into his eye. The employee was wearing safety glasses as required. The employee did not report the incident until he returned to work his next shift (7/24/15). His eye was irritated. The doctor removed the foreign body and gave him a prescription for an antibiotic cream and he returned to work.

Employee was helping the lift truck operator lift a 6" x 25' rubber hose out of a storage rack when his right index finger got caught between the fork of the lift truck and a steel support on the rack. This caused a laceration requiring 23 stitches to close.

An employee was installing a hydraulic motor on a portable screening machine. Due to oily conditions, the motor slipped from the employees hands and when he attempted to catch the motor, his finger was pinched between the motor and the equipment frame.

EE was in the cab of stock truck, when he noticed the passenger side door was open. EE not thinking placed his left hand on the door post of the hinged side and reached with his right hand slamming the door closed on his left thumb. He received an open fracture of the tip of his left thumb.

victim was stacking 50lb. bags of sand on a pallet. victim reached out for a bag that was falling on to stacking table and the bag hit the tip of his ring finger on his left hand and bent it backwards. victim was taken to emergency room where it was x-rayed and it was determined that he had a closed fracture of the finger.

Landon was picking up an empty pallet and turned to place it on the bag table when he felt a sharp pain in his lower right abdomen area. Landon went to an emergency room where he was diagnosed with a right groin strain.

EE WAS SHOVELING AROUND THE CRUSHER WHEN HE HIT HIS HEAD ON AN ANGLE IRON LADDER SUPPORT. HE RECEIVED THREE STITCHES TO CLOSE THE LACERATION. HE WAS NOT WEARING A HARD HAT AT THE TIME. WHEN AS KED WHY HE STATED HE HAD JUST GOTTEN OUT OF THE CLEAN-UP LOADER FOR A MOMENT.

EE WAS REPLACING THE DRIVE BELTS ON THE PRIMARY SCREEN AT THE WET MILL AND PINCHED HIS RIGHT THUMB BETWEEN THE BELTS AND THE SHEAVE. EE RECEIVED SEVEN STITVCHES AND A SPLINT ON HIS THUMB TO RE PAIR THE WOUND.

EE WAS REPLACING DRIVE BELTS ON PUMP AFTER DOING SOME REPAIRS TO THE PUMP. HE PLACED HIS FINGER BETWEEN THE BELT AND THE PUMP SHEAVE WHILE PUTTING THE BELTS ON AND LACERATED THE END OF HIS RIN G FINGER ON HIS RIGHT HAND. EE RECEIVED 5 SUTURES TO CLOSE THE WOUND.

EE WAS MOVING A SCAFFOLD PLANK. THE WT. OF THE PLANK CAUSED EE'S LEFT FOREARM TO STRIKE THE TOEBOARD (KICKPLATE) OF HANDRAIL WHICH LACERATED EE'S LEFT FOREARM WHICH REQUIRED FIVE STITCHES TO C LOSE THE WOUND.

INJURED WAS STACKING 50# PAPER BAGS ONTO A WOODEN PALLET 36X44" OVER STRECHED THE MUSCLES IN HIS LOWER BACK CAUSING PAIN.
