On 6/22/21 at 10:00am EE was restacking pallets alone on the east wall of packaging of MD-09 due to poor stitching resulting in leaking bags. EE made a bending twisting motion while moving a bag from one pallet to another. EE felt a sharp pain in middle right section of back.

Mostrando los 17Was opening lid on pneumatic tanker when ee pulled up on lid hand slipped. When ee was climbing back up the ladder, ee noticed finger was bent a little and was not completely straight. The doctor is stating that ee has acute mallet- finger defect (ruptured extensor tendon) to distal interphalangeal joint of RT 4th (ring) finger.
While chocking a dust truck; employee bent over to put chock in place and dropped chock into pile of dust. There was a hole in the chock; and the dust blew through this hole and went into teammates eyes. Teammate did have on safety glasses. Teammate went to clinic and had eyes washed out and checked for scratches. Prescription given for eyes.
Employee was lifting wheel chock to put on front end loader and strained his left shoulder/neck.
Operating a line small pain in hand after lunch went to B line about 1 hour stopped to dump bags and saw hand was swollen.
Employee was placing bags into bag rack when bags fell through and the employee reached to retrieve the bags. The guide rod to the upper separator caught the employees middle finger on his right hand and smashed it.
While gluing on B-line (25lb line) employee complained of pains going in and out of her back and left leg.
Employee was climbing down ladder on Griffin Baghouse. While climbing down he felt his left shoulder pop. Employee had discomfort in his shoulder during the night and went to the doctor the next day.
Employee was carrying a filter housing up a ladder. He propped the unit on the frame of the blower. Filter house slipped off the blower and caught his left middle finger between the blower and the filter housing. Employee tore his left middle fingernail and crushed the tip of his left middle finger.
EE CUT RIGHT SHIN ON A FAN BLADE WHILE ASSISTING IN REMOVAL OF FAN. EE SAID HE DID NOT NEED MEDICAL ATTENTION. ON 02/02/05 AND 02/11/05, EE WENT TO CLINIC FOR AN INFECTION TO THE WOUND. ON 2/14/05 THE DOCTOR LANCED THE WOUND. EE RETURNED TO WORK ON 2/16/05
Employee was tugging on a rope, trying to get it unstuck. A part of the rope was braided & stiff. The braided, stiff part of the rope went across employees face knocking off his safety glasses. employee has an l shaped abrasion on his left corneal. Oil-dri was notified on 1/4/05 that the ee was given prescription medication, which would qualify this incident as an msha reportable.
EE WAS FUELING TANK ON SERVICE TRUCK. HE WAS RUSHING BECAUSED HE NEEDED TO USE THE BATHROOM. EE WAS DRAGGING THE FUEL HOSE AROUND THE TRUCK AND RAN HIS HAND INTO THE MIRROR ON DRIVERS' SIDE. M IRROR HAS SHARP EDGES. EE WAS TAKEN TO THE HOSPITAL AND RECEIVED STITCHES IN HIS RIGHT INDEX FINGER.
EE WAS WASHING TAIL LIGHTS ON TRAILER, THE LIGHT WAS DAMAGED & EE CUT HIS LEFT PINKY FINGER. FIRST AID WAS PERFORMED & THE EE WENT TO THE HOSPITAL & RECEIVED SUTURES IN THE FINGER.
EE SUFFERED SMASHED LEFT HAND & FINGER CUT (MIDDLE/LEFT) WHILE SERVICING SHAKER VIBRATOR EQUIPMENT.
2 EMPLOYEES WERE LIFTING AN AIR-CONDITIONER TO INSTALL IN THE PACKAGING OFFICE. THE AIR-CONDITIONER SLIPPED OUT OF EE'S HAND AND FELL ON INJURED'S HAND. THIS RESULTED IN A CUT ON INJURED'S 3RD FINGER ON HIS LEFT HAND. THE CUT REQUIRED 10 STITCHES.
EE WAS LIFTING A CHEP PALLET OVER HIS HEAD TO LOAD IN THE STACKING MACHINE EE FELT A SHARP PAIN IN HIS LOWER BACK. EE WAS TAKEN TO COMMUNITY HEALTH IN CAIRO ILL, AND WAS GIVEN MEDICATION FOR BACK STRAIN. EE WAS ALSO PUTON 3-2 DAYS LIGHT DUTY
EMPLOYEE WAS INSTALLING A HYDRAULIC HOSE ON THE SCRAPER WHEN IT GOT CAUGHT. HE PULLED HARD ON THE HOSE WITHOUT FIRST RELEASING THE CAUGHT END. THE HOSE CAME LOOSE HITTING HIM IN THE MOUTH. HE RECEIVED TWO STITCHES IN HIS UPPER LIP.
