Employee was injured while removing metal screen from salt processing plant. The screen was stuck in position and he jerked on it and felt pain in his right wrist. He continued working his regular shift. It was finally determined on 7/27/10 that his wrist had a minor fracture and a cast was applied, forcing restricted duty.

Mostrando los 44Employee was hooking up a welding trailer to a mine tractor. The jack was lowered to its lowest point and the pin was pulled on the jack when the trailer went forward the hitch missed the ball and the trailer went to the ground on top of the employee's right foot.
Employee felt pain in right side when assisting removal of poly bag roll from machine. While lifting, he twisted and stepped down from approximately 10-12" platform.
EE was pulling his cooler out of the bed of the gator and hit the fire extinguisher. The fire extinguisher discharged, shooting chemicals into the bed of the gator and then into his eyes.
Employee helped lift 80# bag from palletizer. Later, when exiting fork, he said he had pain in low back. Incident not reported earlier due to mis-information from employee and doctor whether he could perform regular job or not.
While clearing a jammed can in the north shaker line accumulator, employee's hand became caught between discharge wheel and accumulator frame resulting in lacerations to top of right hand, middle and index fingers.
The employee was demoing some (RMC) ridgid metal conduit during a weekend shutdown. The employee used Tru-trape to support one end of the conduit while he cut a strap loose with a reciprocating saw. The tru tape broke, causing the RMC to fall on one end and the other end to raise up catching his arm between the RMC and a angle iron brace cutting left arm.
Employee waws replacing a .25 inch fuel line on the powder rig engine and did not realize the cover was not secured. When he attempted to remove the fuel line from the engine, the cover on the driver side of the powder rig fell and caught the tip of his finger.
Employee felt alleged pain as he bent over to shut off a water valve. He felt additional alleged pain as he slipped on a wet step after shutting off the valve. He also alleged pain from helping lift an explosives box the day prior.
Employee states that while stacking 50 lb bags of USP product onto a pallet for loading, he felt a burning sensation in his left wrist. A short time later, he noticed some swelling in his left wrist. He was removed from the job and was sent to see a physician.
EE states he was manually palletizing 50lb paper stacks of USP product. While removing bag from scale to stack on pallet, he felt a spasm and discomfort in the middle of his back. EE was seen by a physician and assigned to restricted duty for a period of two weeks.
Employee was standing on the southwest side of the chlorinator station inside the strip curtain observing another employee attempting to remove the chlorinator. Upon breaking the seal with the bottle by loosening the yoke of the chlorinator, chorine gase began discharging from the bottle.
Employee was standing on hitch receiver to put coolant in the Grove. Employee grabbed coolant and hitch rotated, causing employee to slip off and fall to the ground.
Employee began missing work on 10/11/07 due to injury received when helping another employee lower a 90# fire suppression cannister into place at a conveyor drive unit. Both employees were holding the cannister when lowering it.
Employee was assisting picking up, a conveyor trougher set of rollers that was from an un-banded stack. As they picked the set up, they bumped the stack and it began to fall over. The one employee got out of the way, but the other one was partially struck by the falling roller sets as he could not get completely out of the way. MEDICAL TREATMENT - Stitches
EE was inspecting the oil cooler for leaks on the scaler. The hinged cover was secured in an upright position with a rope attached to the scaler's boom. The rope that was securing the oil cooler in an upright position was removed. EE had repositioned himself beneath the oil cooler and continued checking for leaks. The cover dropped pinching his hand between cooler & frame.
Employee suffered an injury while helping move conveyor belt structure. Employee was turning to side, helping pick up structure and turning to place it in bucket of FEL. REPORT FILED AS RESTRICTED DUTY
EMPLOYEE STATES HE WAS SLIDING AN 80 LB. BAG OF SALT ONTO A PALLET WHEN HE TWISTED & FELT DISCOMFORT IN HIS MIDDLE BACK. HE WAS DIAGNOSED WITH A THORACIC/LUMBAR MUSCLE STRAIN. RESTRICTED DUTY FOR 5 DAYS & MUSCLE RELAXANTS.
Employee injured while closing freight elevator doors. He pulled on the strap on the outside of the door, instead of pulling on the inside strap. His hand was caught between the two doors as they came together.
EE STATES HE WAS OPENING THE SPRING-LOADED HATCH DOOR ON TOP OF A HOPPER RAILCAR IN PREPARATION FOR CLEANING. WHILE LIFTING ON THE HATCH TO OPEN, THE EE EXPERIENCED DISCOMFORT IN HIS LOWER BACK. INCIDENT REPORTED ON 8/5/05.
While assisting another employee in making an air flow adjustment to the Pellet Bagging machine, employee reports he lacerated his left index finger when it contacted the drive belt chain and sprocket at the top of the machine.
AN 8' X 8' X 1/2" STEEL PLATE WAS BEING USED AS FORWORK FOR A 20' X 8' TALL WALL. DURING THE CONCRETE POUR, THE WALL TIES FAILED AND THE EMPLOYEE'S LOWER RIGHT LEG WAS STRUCK BY THE STEEL PLATE AND PINNED BETWEEN THE PLATE AND SOME NEARBY CRIBBING.
EE ALLEGED THAT WHILE USING THE VACUUM ARM TO PALLETIZE 10# BAGS, THAT HE FELT A PAIN IN HIS LEFT SHOULDER.
EMPLOYEE WAS ATTEMPTING TO REMOVE A DUST COLLECTOR BAG THAT HAD FALLEN TO THE BOTTOM OF THE COLLECTOR IN THE VACUUM SCREEN ROOM. WHILE PULLING ON THE STRAPPING, EMPLOYEE STATES HE EXPERIENCED A PAIN IN HIS RIGHT SHOULDER.
EMPLOYEE WAS HELPING TO INSTALL A HEADER PIPE ON THE BLOCK PRESS WHILE PIPE WAS BEING ALIGNED. ANOTHER EE MOVED THE PIPE WITH A PRY BAR CAUSING THE PIPE TO FALL AND PINCH THE MIDDLE FINGER ON HIS RIGHT HAND BETWEEN PIPE AND PRESS FRAME.
EE WAS CHIPPING ON A POST-INDICATING VALVE FOR THE PLANT FIRE SYSTEM AND WAS WEARING SAFETY GLASSES WITH SIDE SHIELDS ALONG WITH FACE SHIELD. EE BEGAN TO EXPERIENCE IRRITATION IN HIS RIGHT EY E AFTER GOING ON BREAK. HE NEVER FELT ANYTHING STRIKE HIS EYE WHILE CHIPPING, NOR DOES HE REMEMBER RUBBING HIS FACE OR EYES. AN EXPLANATION AS HOW THE OBJECT ENTERED HIS EYE WOULD BE AFTER R
EE WAS REPAIRING PUMP. PIPE HANGERS IN CEILING PULLED OUT WHEN TRYING TO DISASSEMBLE THE PUMP CAUSING LACERATION AND FRACTURE TO EE'S THUMB.
WHILE PULLING THE DRIVE BELT BY HAND TO CHECK ALIGNMENT EMPLOYEE'S LEFT THUMB WAS CAUGHT BETWEEN THE BELT AND SHEAVE. HE WAS NOT WEARING GLOVES.
EMPLOYEE REPORTS HE WAS STACKING 50 LB SALT BLOCK ON PALLETS WHEN HE EXPERIENCED A PAINFUL SENSATION IN HIS LOW AND MIDDLE BACK. EMPLOYEE SOUGHT MEDICAL TREATMENT ON 9/10/02 AND WAS DIAGNOSED WITH A BACK STRAIN.
EE REPORTS LOW BACK PAIN WHILE ASSISTING ANOTHER EE IN MOVING A DRYER TRUNION IN THE WESTERN WAREHOUSE IN PREPARATION FOR LOADING A TRUCK FOR TRANSPORT. EE SOUGHT MEDICAL ATTENTION ON 8/19/02 & WAS DIAGNOSED WITH A LOW BACK STRAIN.
EE WAS ATTEMPTING TO PLACE A 12-VOLT BATTERY INTO THE BATTERY COMPARTMENT, WHILE AT THE SAME TIME TRYING TO HOLD HINGED COVER ON THE BATTERY COMPARTMENT OPEN. WHILE LOWERING THE BATTERY INTO THE COMPARTMENT, THE EE INADVERTANTLY PLACED HISTHUMB BETWEEN THE FRONT PLATE OF THE BATTERY COMPARTMENT AND THE BATTERY, SMASHING HIS THUMB.
EMPLOYEE WAS WORKING ON ROCK BAGGING LINE PALLETIZING 80# X-COARSE FILM BAGS WHEN HE NOTICED PAIN IN THE LEFT SHOULDER BLADE AREA. NO SHARP PAIN, JUST GRADUALLY GETTING WORSE. EMPLOYEE ALSO PA LLETIZED 50# BAGS AND CONTINUED TO FEEL THE PAIN IN THE SHOULDER AREA AFTER WHICH HE REPORTED IT TO THE FOREMAN.
EE WAS ATTEMPTING TO CLOSE A SLIDING GLASS WINDOW. WHILE PULLING ON THE ALUMINUM SASH ATTACHED TO THE WINDOW FRAME, EE'S HAND SLIPPED AND BROKE GLASS, RESULTING IN A LACERATION.
EE STATES HE WAS ASSISTING TWO OTHER EES REL0CATE THE SUPER SACK MACHINE. THE MACHINE IS ROLLED ON CASTERS WHICH ROLL THE MACHINE TO DIFFERENT LOCATIONS. WHILE PUSHING ON THE MACHINE DURING TH E RELOCATION PROCESS, EE S6ATES HE EXPERIENCED DISCOMFORT AND PAIN IN LOWER BACK.
EE STATES HE WAS REMOVING STEEL CABLE FROM SCAFFOLD POLES IN ORDER TO RETURN CABLE LINE INTO RETRACTABLE FALL PROTECTION DEVICE. WHILE DOING SO, THE LINE SLIPPED FROM HIS HANDS AND STRUCK A SC AFFOLD BOARD(ALUMINUM) CAUSING IT TO FALL AND STRIKE HIS LEFT ARM.
EMPLOYEE WAS MAKING MINOR ADJUSTMENT TO A PUMP IN A SQUATTING POSITION, AFTER MAKING THE ADJUSTMENT, EMPLOYEE FELT PAIN IN HIS LEFT GROIN AREA WHILE STANDING UP FROM THE SQUATTING POSITION.
EE WAS INJURED FELT A PAIN WHEN HE PULLED A SECTION 3-4' OF 440V TRAILING CABLE FROM REEL ON A JUMBO DRILL WHILE PREPARING TO ANCHOR THE CABLE. EE BEGAN MISSING WORK ON 1-27-01.\
EMPLOYEE STATES HE WAS POURING BAGS OF POPCORN SALT INTO THE SALTER TANK WHEN THE LID ABOVE THE SILO VIBRATED LOOSE AND FELL STRIKING HIS HARD HAT AND LEFT HAND. EMPLOYEE LATER COMPLAINED OF P AIN TO RIGHT SHOULDER.
EMPLOYEE STATES HE EXPERIENCED LOWER BACK AND HIP DISCOMFORT WHILE ENGAGED IN CLEANING THE #1 COARSE SALT BIN.
EE WAS INJURED WHILE TAKING NEW HOIST ROPE FROM SPOOL. THE END OF THE CABLE WAS UNDER TENSION AND NOT SECURELY CLAMPED. AS THE CABLE CAME OFF THE REEL, THE LOOSE END FLIPPED, HIT THE GROUND, T HEN STRUCK THE EMPLOYEE.
A LOCK RING BECAME DISENGEGED FROM THE WHEEL. THE TECHNICIAN DID NOT FOLLOW THE BEST PRACTICES AND PROCEDURES OF COMMERCIAL TIRE. AN ATTEMPT WAS MADE TO RECREATE THE ACCIDENT; HOWEVER THE TECH NICIAN IS UNABLE TO RECALL ALL EVENTS.
EMPLOYEE WAS INJURED WHILE ASSISTING REPLACEMENTOF WINDOW AIR CONDITIONER IN UNDERGROUND SCREEN PLANT OFFICE. UNIT WAS NOT RAISED HIGH ENOUGH AND ASUNIT WAS PUSHED TOWARD ITS POSITION, SHARP M ETAL SNAGGED EMPLOYEE'S FINGER.
EE WAS WAITING TO PUNCH OUT AT THE END OF HIS SHIFT WHEN HE REACHED UP TO SCRATCH HIS FACE AND THE EDGE OF HIS TIMECARD APPARENTLY SCRATCHED HIS LEFT EYE. HE WAS WEARING HIS STREET GLASSES.
ON THIS DATE INJURED EE AND ANOTHER EE WERE CHANGING A 25HP MOTOR ON THE CLEAR BRINE PUMP LOCATED IN THE EVAP PLANT PUMP PIT. THE MOTOR WAS REMOVED FROM THE PIT USING A CHAIN BLOCK. AFTER OPER ATING THE CHAIN BLOCK THE INJURE EE FELT A SORENESS IN HSI LEFT SHOULDER. HE DID NOT EXPERIENCE ANY SORENESS AGAIN UNTIL AFTER HE WENT HOME AFTER SHICH THE SORENESS PERSISTED.
