Employee was changing v-belts on hummer screen when the counterweight moved. The employee's glove was caught by the v-belt causing a finger to be pinched between the belt and the belt pulley.

Employee was getting off the vertical drill and slipped off the last rung of the entry ladder. Employee fell on employee's right side and arm. Employee was taken to the ER as a precautionary method and x-rays were negative for rib injury.

The employee was walking near the maintenance shop when EE tripped over a flume cover and fell to the ground. EE put hand down to catch self and felt pain in the left wrist almost immediately. EE had a wrist x-ray showing a strain.

Employee was standing on a ladder replacing a rubber gasket on a condensate line when employee lost balance and fell to the ground. The ladder was being tended by another employee due to no tie-off anchors in the immediate area, per our ladder policy.

Employee was pulling on the v-belts to spin the dust collector fan. Once EE stopped pulling on the belts, the fan rolled backwards pinching EE's fingers between the belts and pulley.

As employee was helping to lift a Rotex screen, EE felt a "pop" in the right shoulder

Employee had bent over to seal a tanker truck lid and felt a twinge in their back when they raised up.

Breaking salt lumps out of a dryer with a sledge hammer (20#). Hit the dryer chute at a awkward angle and felt a vibration in EE's right shoulder. Was unable to use the hammer without pain.

During a belt extension, employee was hit by a cable used to pull the old conveyor off. An employee started moving the jeep unaware that the cable was still attached. Once the cable starting moving it tripped the employee causing EE to fall on EE's right side.

Employee was descending down a stairwell when EE felt a "pop" in EE's left knee along with a dull pain on the side of EE's knee.

While tightening a conveyor belt using a adjusting wheel, the employee felt a pull in lower back. Employee stopped using the wheel until another maintenance person arrived to help.

After loading a tanker truck the employee strained EE's back as EE pulled self up onto the loading platform. The trailer EE had completed loading was lower than other trailers causing a larger area between the trailer and the loading platform

Employee slipped on ice while walking in parking lot injuring left ankle.

Mechanics were in process of changing dryer trunion roller, the floor grating shifted and fell through building structure, employee lost balance and fell to floor below. About 19 feet.

Employee was walking when they felt a twinge in right knee. The next day employee was walking into a break room when they felt a "pop" in knee. Employee went to the clinic and found to have arthritis under knee cap.

Employee was operating a bag machine when EE pulled a bag back toward self to realign the bag with the sealer. EE states EE felt a "twinge" in EE's left shoulder when EE pulled the bag backwards.

Inspection door blew open due to air leak striking employee on the forearm. Air had leaked from a partially opened inlet valve causing the inspection door to be pressured up to 30 psi.

Employee was walking up stairs and felt a "pop" in left hip. The incident occurred on Friday, September 27 but was not reported to a supervisor until Monday, September 30. The employee was taken to the clinic on Tuesday, October 1 and diagnosed with a lumbar strain. EE is currently on light duty and receiving physical therapy.

Employee was lifting a 12" vinyl hose about 1' to clear the hose of a salt lump. When the employee pushed up, employee felt a pull sensation in shoulder.

Pulling on conveyor belt and strained lower back during belt replacement. Employee noticed stiffness in lower back next morning. and reported to supervisor Monday morning when they returned to work.

The employee tripped on a piece of loose flooring. When they put left foot down to brace themselves, they felt a pain in the calf muscle. EE said they felt ok and continued to work. After five days of work they said it was still sore so I sent them to the doctor to be examined. Examination found a pulled left calf muscle.

Employee was helping a fellow employee pull a wheeled hopper out of a trash chute when EE felt EE's back tightened up. EE reported the incident the next day, January, 16.

The employee was starting to ascend a vertical ladder when EE heard a "pop" in the right shoulder.

Stepped on pallet top sheet which was slick and caused employee to fall. When ee fell ee tried to catch self, ee bent thumb backwards as ee fell.

Employee tripped on a piece of angle iron, causing a sprain of the right ankle.

Hoist #1 would not reset. MSHA was notified at 7:07 am, Evacuation started at 7:05 am and completed at 7:18 am. The hoist was repaired at 7:30 am and the miners went back underground at 7:39 am. The hoist was tested before being put back into service. To prevent recurrence the pinion brake switches was adjusted.

Hoist #1 would not reset. MSHA was notified at 3:06 pm, evacuation started at 2:55 pm and was completed at 3:05 pm. The hoist was repaired at 3:25 pm and the miners went back underground at 3:33 pm. To prevent a recurrence the pinion break switch was adjusted.

#2 guide pump coupler and line. EE notified MSHA at 9:40 am, evacuation started at 9:40 am and was complete at 10:05 am. The hoist was repaired at 2:00 pm and the miners went underground at 2:05 pm. The joist was tested before being put back into service. To prevent a reoccurrence the coupler was replaced, replaced fitting and electrical motor.

Hoist #2 switch broke by loose scale. MSHA notified at 2:05 pm by EE. Evacuation started at 2:05 pm and was completed at 2:12 pm. The hoist was repaired at 2:20 am. The miners went back underground at 8:00 am. From now on will check loose scale in the #2 shaft.

Hoist #2 would not reset. Evacuation started at 7:20 pm. Evacuation was completed at 7:22 pm, the hoist was repaired at 7:50 pm. The miners went back underground at 8:00 pm. No injuries occurred. To prevent this from happening again the operator was informed to only stop at designated landing.

Outage on Hoist #2 due to cold weather. MSHA was notified at 3:55 am by Mike Law. Evacuation started at 3:53 am and was complete at 4:05 am. The hoist was repaired at 10:25 am and the miners went back underground at 10:45 am. Heat was added to prevent the outage from happening again. No injuries occurred.

#2 Hoist would not reset.

Due to cold weather the hydraulics would not work properly.

Electrical Issue with motor. MSHA notified at 7:00 am., evacuation started at 6:55 am., evacuation complete at 7:10 am., Hoist motor was replaced and tested at 8:35 pm., miners returned underground at 8:35 pm.

A tube winder operator was poking some stuck cans near a roller when the roller pulled the poke rod under the roller. The operator instinctively tried to grab the rod but missed. EE's hand was pulled into the roller and the machine was stopped and EE got EE's hand out. The hand was x-rayed and no fracture was found. No stitches were indicated.

Hoist contact, Mine evacuated at 7:30 am, MSHA notified was notified at 7:30 am, evacuation complete at 7:46 am. Hoist was repaired at 9:15 am. Tested and miners returned at 9:20 am.

Hydraulic pump leak. Mine evacuated at 5:35 pm, MSHA was notified at 5:40 pm, evacuation complete at 5:55 pm, hoist was repaired at 9:00 pm, tested and miners returned at 9:05 pm.

Employee was moving a bag of ingredient off of a pallet and hit a fire extinguisher, knocking it off the wall onto the top of foot. Employee was examined at the ER, no broken bones just deep contusion. I overlooked employee's restrictions, therefore did not report this properly.

Employee was operating a portable band saw to cut a seized bolt. The employee put too much force on the saw and pushed the blade of the saw into finger. EE cut finger.

Number 2 hoist had a hydraulic break. MSHA was notified at 10:00 am, miners evacuated at 10:00 am, evacuation was complete at 10:22 am. Hydraulic was repaired, tested, and miners returned underground at 10:40 am.

Number 2 hoist had a hydraulic break. MSHA was notified at 9:42 pm., miners evacuated 9:30 am, and evacuation was complete at 9:40 am. Hydraulic was repaired. tested, and miners returned underground at 1:00 am.

#2 gear had a loose electrical wire. MSHA was notified at 2:25 a.m., miners evacuated at 2:20 a.m., and evacuation was completed at 2:45 a.m. Hoist wire was repaired, tested, and miners returned underground at 3:45 a.m.

Electrical issue with switch. MSHA notified at 8:51 a.m., evacuation started at 8:50 a.m., switch was repaired and tested at 11:05 a.m. miners returned underground at 11:25 a.m.

Electrical outage. Miners were evacuated at 7:30 a.m., evacuation completed at 7:40 a.m., MSHA notified at 7:43 a.m. Replaced magnet switch at 9:15 a.m., hoist was tested, and miners returned underground at 9:20 a.m.

#2 gear wire became loose, mine was evacuated at 4:05 a.m., MSHA was notified at 4:07 a.m., evacuation was completed at 4:20 a.m., hoist was repaired at 4:55 a.m., tested and the miners returned to work at 5:05 a.m.

Outage caused by back out switch, MSHA was notified at 11:00 p.m., evacuation began at 10:55 p.m., and completed at 11:15 p.m., hoist was repaired at 2:50 a.m., was tested and miners returned underground at 3:00 a.m.

Electrical, loose Wire #1 contact. MSHA notified 8:25 p.m., evacuation of miners started at 8:14 p.m., repairs were made and tested at 8:40 p.m., miners returned underground at 8:48 p.m.

Air compressor went out. MSHA was notified at 12:16 a.m., evacuation started at 12:15 a.m., hoist was repaired at 12:28 a.m. hoist was tested and men returned underground.

Electrical problems, evacuation started at 4:55 p.m., completed at 5:05 p.m., MSHA was notified at 5:06 p.m. Repairs were made and completed, at 6:45 p.m., the hoist was tested, and miners returned underground at 6:50 p.m.

Cage would not go to mine level. Evacuation started at 5:20 p.m., completed at 5:25 p.m., MSHA was notified at 5:27 p.m. Timbers were greased and completed at 6:20 p.m., and miners returned underground at 6:30 p.m.

Electrical problem, MSHA was notified at 5:13 p.m., evacuation started at 5:00 p.m., completed at 5:13 p.m. Wire was repaired at 8:15 p.m., miners returned underground at 8:35 p.m. The hoist was tested before put back in service.

Plant Wide power outage because transformers caught on fire. Miners were evacuated at 11:55 a.m., MSHA was notified, transformers were repaired then tested and the miners returned underground at 11:00 p.m.

Gear stuck on Hoist #1. Evacuation started at 9:10 a.m., MSHA notified at 9:32 a.m., contactor was replaced by 11:15 a.m., men returned underground at 11:30 a.m.

Bushing in guide rope sticking. Evacuation started at 10:25am, and completed at 10:36 am. Hoist repaired at 11:50am. Hoist was tested at 12:00pm and miners returned underground at 12:10pm.

Encoder went out. Miners were evacuated at 12:35 a.m. and encoder replaced at 2:40 a.m., miners returned to work at 2:50 a.m.

Shaft switch at level #1 failed on #2 Hoist. Started evacuation at 2:05 pm. Hoist switch was replaced before evacuation complete. Miners returned to their jobs.

Hoist contactor bumper pad came loose. Miners were evacuated at 10:45 pm. Evacuation completed at 11:00 pm. The hoist contact pad was repaired and miners returned underground 12:55 am.

Experienced computer controller problem earlier in the morning. Repaired and normal operation until 12:20 p.m. when computer went down again. Evacuated miners. Computer was replaced with new unit. Hoist was tested and miners returned underground at 3:00 p.m.

Computer that controls hoist mal-functioned due to software problem. Miners were evacuated. The computer was repaired, the hoist was tested and miners returned underground at 10:15 am.

EE was operating fork truck in an easterly direction when he slowed down at an intersection in warehouse. Another fork truck operator was proceeding in a southerly direction and became distracted while looking for her scan gun. EE looked up and saw fork truck and attempted to stop to no avail and a collision resulted.

The frequency drive for the hoist motor failed. Miners were evacuated at 6:53 pm. The frequency drive was replaced, the hoist tested, and miners returned underground at 2:05 a.m.

An electrical contactor wire broke and the starter stuck. Mine evacuation started at 5:15 a.m. and was completed at 5:40 a.m. The contactor was repaired and the hoist tested. Miners returned underground at 7:45 a.m.

Electrical contactors stuck rendering hoist inoperable. Mine evacuation started at 10:15 a.m. and was completed at 10:30 a.m. The contacts were replaced and the hoist tested. Miners returned underground at 12:20 p.m.

Drive motor coupling bearings on #1 Hoist failed. Miners were evacuated at 10:55 p.m. and was completed at 11:04 p.m. The bearings were replaced & the hoist was restored to service on 6/19/14 at 3:00 p.m. after being tested.

The proximity switch for the #2 hoist failed at 9:45 a.m. Miners were evacuated at 9:50 a.m. The switch was replaced and the hoist was tested. Miners returned underground at 11:20 a.m.

A push button control relay to the hoist was showing a fault. The mine was evacuated until repairs were made. The hoist was restored to service at 7:25 a.m. The hoist was tested and miners returned underground at 7:40 a.m.

Blown fuse caused by east compressor. The mine was evacuated at 5:17 pm and completed at 5:46 pm. The hoist was repaired and tested. Miners returned underground at 9:50 pm.

Electrical breaker in control panel tripped on #2 hoist resulting in a short outage. The mine was evacuated and the breaker was repaired. Hoist restored to service and miners returned underground at 12:25 p.m.

An electrical contactor located in the hoist house motor control panel became stuck causing the hoist to not function properly. The hoist was restored to service and tested. Miners returned underground at 10:12 p.m.

Employee states he was manually stacking 50 lb. salt bags onto a wooden pallet when he felt discomfort in lower middle back.

Employee states he stepped down from tractor cab and his foot contacted uneven surface where asphalt transitioned into concrete causing his right foot to roll resulting in ankle sprain.

A transformer fuse blew on a 3 phase conductor disabling the #2 hoist. The mine was evacuated using the #1 production hoist. The evacuation started at 12:15 a.m. and was completed at 12:25 a.m. The #2 hoist was restored to service at 1:35 a.m. The hoist was tested and miners returned UG at 1:55 a.m.

Employee states he was using a scaling bar to clean out the #5 Hummer when a salt lump separated, and fell striking his right leg and pushing it into the Hummer wall.

Employee states he was performing a flow test on a fire hydrant and the valve was stuck. As he applied more force on the spanner wrench, the valve broke loose and the wrench spun in an uncontrolled manner allowing his hand to contact the metal ID tag affixed to the hydrant resulting in a laceration that required sutures.

Lost power to underground mine due to transformer failure related to weather. The mine was evacuated at 1:08pm. The transformer was replaced and the hoist was tested. Miners returned underground at 6:55pm

EE states he was in the parking lot and placed left foot on steps to ascend staircase. Left foot was halfway on stair when his weight shifted causing his foot to bend backward resulting in plantar muscle tear.

Employee states he was inspecting the centrifuge chute after cleaning with the inspection door closed. Employee removed the inspection door pin and opened the door. While holding the inspection pin in his right hand, the pin contacted an appendage on the rotating drum which slammed the metal door pin into the thumb of his right hand resulting in an injury.

The hoist brake engaged due to losing air pressure through the air valve weep hole. Miners started evacuating the mine at 8:30 a.m. and were on the surface at 8:45 a.m. The hoist brake valve was repaired. The hoist was tested and miners returned underground at 11:00 a.m.

The #1 hoist pinion brake sensors became out of alignment. The mine was evacuated at 11:00 pm and completed at 11:10 pm. The pinion brakes sensor mounting brackets were realigned, tightened and hoist was tested. Miners returned underground at 1:30 am.

The encoder that controls hoist position in the shaft malfunctioned. The Mine was evacuated at 9:10p.m. The hoist was repaired and tested at 9:30 p.m. Miners returned underground at 9:30 p.m.

The hoist controls would not respond and were not functional. The mine was evacuated at 7:00 am and completed at 7:15 am. An electrician was summoned and the cause was determined to be a stuck contactor. The hoist was repaired and tested. Miners returned underground at 8:50 am.

EE was using air wand to clear a can on conveyor line. While blowing out can, his right index finger contacted the raised part of a belt splice that resulted in a laceration to his finger. EE was seen and treated at the E.R. in Grand Saline. TX and released to return to work without restrictions.

A hoisting control breaker tripped resulting in an hoist outage. Cause of trip believed to have been unusually wet conditions and humidity. Miners were evacuated at 10:26am. and out of the mine at 10:42 am. The control breaker was tested and reset. The hoist was tested and miners returned U.G. at 10:45a.m.

Hoist motor failed due to loose conductor and motor stator nut. Mine was evacuated at 7:57a.m. and completed at 8:21 a.m. Repairs were made, hoist tested, and miners returned underground at 9:15a.m.

The bolt on the #2 auxiliary contactor which controls the hoist speed came loose. Miners were evacuated. The contactor bolt was re-secured. The hoist was tested and miners returned underground.

The Dissolver Circulation pump at Power & Production was locking up intermittently. (2) employees were sent to trouble shoot the pump. It was determined the pump would need to be opened. The pump was locked/tagged, isolated & bled. During the removal process when the pump was pried loose from the housing, hot brine was released under pressure that resulted in burns to both EEs.

A wire termination for the #2 gear was damaged. Miners were evacuated at 3:14 a.m. and on the surface at 3:24 a.m. An electrician was called out to repair the damaged wire. The hoist was tested and miner returned underground at 4:25 a.m.

EE states he was wiping sweat from eyes using his shirt. While doing so he thought he scratched his eye. During weekend his eye became irritated and he went to the E.R. A small piece of rust was removed from his eye.

While removing salt growth from level one entrance to the #2 shaft, an electrical control wire was damaged. The mine was evacuated at 9:41 pm. The control wire was repaired and the hoist was tested. Miners returned underground at 11:00 pm.

EE is employed by J Rayl Transport. On the day of January 26th 2011 he was running a normal Morton Salt Shuttle. EE was getting out of his tractor to check the trailer and slipped on wet steps. When EE fell, he fell backward and hit his head. This was around 6:15 am. The wet condition of the steps contributed to his fall.

A loose wire on the #3 gear contractor prevented the hoist from operating. An electrician was called and the wire was tightened. Miners remained underground during the repair due to surface weather conditions. The hoist was restored to service at 4:55 am.

After performing maintenance work on the bucket in the HP building top floor, ee was descending a 12'fixed vertical ladder. During the descent ee's hand slipped resulting in his falling from one level to another to the grated fiberglass floor below (approximately 11').

EE was operating the bag machine, producing 25lb sacks of table salt in paper bags. A bag had cleared the machine without the bag tape being cut. EE attempted to cut the tape with a retractable safety knife. While cutting toward his right hand, the knife blade did not retract when it left the tape surface and lacerated the top of his right hand.

The MVR process startup resulted in an undervoltage condition that resulted in power being lost to the #2 hoist and reset would not function. The mine was evacuated at 12:22PM and completed at 12:28PM. Power was restored to the hoist at 1:20PM. The hoist was tested and miners returned underground at 1:20PM.

Lost SWEPCO power to the #2 hoist due to an electrical storm. Miners were evacuated at 6:05PM and completed at 6:14PM. Power was restored and the hoist was tested. Miners returned underground at 6:35PM.

While lifting and lowering an outrigger extention block and pad on the condor 100 lift truck, employee states he mashed his right hand between the extension block and water cooler mounting bracket as he was lowering the extension block to ground level.

EE states he was tilting vacuum cleaner back to lie on its side to move under a conveyor belt. While doing so, his finger became caught between the vacuum cleaner frame and a handrail leading to an upstairs area.

The main upper hoist contactor coil failed causing the hoist to malfunction. The mine was evacuated at 12:20 a.m. and completed at 12:30 a.m. The faulty coil was replaced and the hoist was tested. The miners returned underground at 1:45 a.m.

The #1 hoist speed control contactor stuck on control #3. An electrician was called to replace the entire switch with a new one. The miners were evacuated at 9:38am the evacuation was completer at 9:58am. The contactor relay switch was replaced and the hoist was tested. Miners returned u/g at 10:40am.

The #1 hoist speed control contractor stuck on control #3. An electrician was summoned and freed the switch. Miners were evacuated at 8:22am. Evacuation completed at 8:39am. The hoist was repaired and tested. Miners returned u/g at 9:05am.

A wire supplying power to the upper limit switch was compromised by a short circuit. Miners were evacuated at 7:50 p.m. Evacuation completed at 8:04 p.m. The encoder and track limit switch was replaced. The hoist was tested and the morning shift miners entered the mine at 2:10 a.m.

There was a malfunction in the main contactor. The mine was evacuated at 4:33 a.m. and completed at 4:55 a.m. The hoist was repaired at 6:55 a.m. and the miners returned underground at 7:00 a.m.

Hoist operator noticed arc flash on brush end of electric hoist motor. Motor was shut down and an electrician was called and the mine evacuated. The lug connections to the brush rings were tightened. The hoist was tested and placed back into service. Miners returned underground at 7:15 am.

Vibration cause a wire to break on second stage contactor resulting in loss of second gear. the mine was evacuated at 3:52 am and complete at 4:05 a.m. The hoist was repaired at 4:15 a.m. and the hoist was tested. Miners returned u/g at 4:20 a.m.

EE states he was washing the KCl crude filter when he noticed the tank was filling too fast. He left the filter to shut off the water when the tank ruptured and sprayed hot brine on employee's ankle.

The #2 Hoist was shut down to replace a broken hydraulic gage. Miners were evacuated at 11:00 a.m. and evacuation was completed at 11:11 a.m. The hoist was tested and miners returned underground at 1:30 p.m.

Power outage occurred in the power and production department due to a tripped breaker. Evacuation started at 10:31 a.m. was completed at 10:41 a.m. The breaker was reset, the hoist was tested and the miners returned underground at 10:57 a.m. and mining operations resumed.

Employee was saw cutting asphalt when he started to feel weak. Employee was transported to local hospital and given IV fluids and released for work.

Lost SWEPCO power on both hoists due to component failure on SWEPCO side. Miners were not evacuated due to the unavailability of an electrician to operate the emergency generator. SWEPCO power was restored at 6:08 AM and the hoist was tested and returned to service. Miners returned to the surface at the 7:00 AM shift change.

Termination on second set of hoist contacts broke loose from vibration causing hoist to malfunction. The mine was evacuated at 11:33 pm and completed at 11:47 pm. The hoist was repaired and tested and miners returned underground at 12:06 am.

While clearing a jammed can in the north shaker line accumulator, employee's hand became caught between discharge wheel and accumulator frame resulting in lacerations to top of right hand, middle and index fingers.

The pinion brake on the # 2 hoist drive would not release. The mine evacuation was complete at 1:50pm. A new pinion brake releif valve was installed, the hoist was tested and restored to service. Miners returned underground at 3:00 pm.

Employee was assigned to clean up oil spillage near the block press. While applying oil to the dry floor, he slipped and grabbed a stack of pallets with his left hand to avoid falling to the ground level. Later in the shift he observed a knot that formed on his left wrist as a result of grabbing the pallets.

Swepco power outage affected city of Grand Saline and multiple counties. Power was restored at 8:31 a.m., the hoist was tested and mining operations resumed.

Experienced intermittent loss of control power due to faulty contactor relay that controls hoist speed. Miners were evacuated and the control contactor relay was replaced. The hoist was tested and miners returned underground at 4:55 pm.

The employee was demoing some (RMC) ridgid metal conduit during a weekend shutdown. The employee used Tru-trape to support one end of the conduit while he cut a strap loose with a reciprocating saw. The tru tape broke, causing the RMC to fall on one end and the other end to raise up catching his arm between the RMC and a angle iron brace cutting left arm.

Employee states he was walking around the back of a fork truck at the maintenance shop to access the area he was working in. While walking behind the fork truck and through the bay door, his right foot turned on an elevated part of the asphalt walking surface causing him to fall to ground level.

Employee states that while stacking 50 lb bags of USP product onto a pallet for loading, he felt a burning sensation in his left wrist. A short time later, he noticed some swelling in his left wrist. He was removed from the job and was sent to see a physician.

EE states he was bagging 40lb film sacks of pellets, when he stepped off the wooden platform to retrieve more bags. While stepping down from the platform, his foot contacted the edge of the wooden platform, causing him to lose his balance and fall forward. He raised his right arm and grabbed the ink jet coder box to keep from falling. Strained right ankle and arm below shoulder.

EE states he was manually palletizing 50lb paper stacks of USP product. While removing bag from scale to stack on pallet, he felt a spasm and discomfort in the middle of his back. EE was seen by a physician and assigned to restricted duty for a period of two weeks.

EE states he was preparing to move a load of round cans using the Nissan electric fork truck. When he applied pressure on the operating handle without activating the speed control, the machine abruptly moved forward pulling the employee with his left arm resulting in a shoulder strain.

The electrical windings in the rotor shorted in the #1 hoist motor. The mine was evacuated at 5:45 a.m. The hoist motor was replaced with a new rebuilt motor and the hoist was tested. Miners returned underground at 1:45 p.m.

During the shaft inspection, a defective slow down switch was found. The mine was evacuated and the slow down switch was replaced with a new one. The hoist was tested and miners returned underground at 11:00 a.m.

The incremental encoder failed. The mine was evacuated at 1:20am, and completed at 1:30am. The encoder was replaced and tested at 3:35am, and the miners returned underground at 3:45am.

Bad fuses (2400v at sub station) Evacuation of the mine started at 10:27 am, and was completed at 10:37 am. The fuse were replaced around 11:15 am, and the miners returned underground at 11:20 am.

Relay on the main Lower contacts had a broken finger that activates the steps (speeds). The mine was evacuated at 8:10 p.m. the relay was replaced around 1:40 am, & the miners returned underground at 1:55 am.

A sensor showed the pinion brake on the #1 hoist to have a wear switch and brake release fault. The mine evacuation was complete at 7:30 a.m. The hoist was repaired and tested. Miners returned underground at 8:05 a.m.

EE received an electrical shock while hanging a super sack bag onto filler spout. Physician instructed to remain off work one day. When he returned to work he felt ill. Dr diagnosed with coronary artery blockage, did stress test and CT scan, tests negative. Instructed to stay off rest of week until he sees Dr again to determine when able to return to work.

The #2 hoist showed a computer fault code of "drive stall". The mine was evacuated & an electrican was notified. The hoist computer system was reset & the code cleared. The hoist was tested at 10:50 pm. Miners returned underground at 10:50 pm.

Excessive moisture entered the #1 hoist electric motor causing it to fail. Miners were evacuated at 4:46 a.m. and out of the mine at 6:59 a.m. The hoist motor was replaced and aligned at 8:05 p.m. The hoist was tested and miners returned underground at 8:20 p.m.

An interruption in power supplied by the public utility company interfered with use of #1 & #2 hoists. Miners remained underground for 50 minutes until an electrician arrived to restore electrical service or activate the emergency generator. Electrical service was restored to the hoists by resetting a breaker in the substation.

Vibration caused a wire on the speed controller to become disconnected from the hoist speed controller. The mine was evacuated at 6:42 am and was completed at 6:55 am. A new termination was installed on the speed controller, was tested and restored to service. Miners returned underground at 7:30 am.

A loose connection caused a wire termination to burn off a speed control contactor. The mine evacuation started at 3:28 am and was completed at 3:42 am. The hoist was repaired at 4:07 am and tested prior to use. Miners returned underground at 4:15 am.

Vibration caused wire to loosen on mechanicl contactor that controls speed steps on the production hoist. The mine was evacuated at 4:10pm and completed at 4:23am. The hoist contactor wire was repaired, the hoist tested, and miners returned underground at 5:30am.

While in process of transferring diesel fuel into mine via the #2 intake shaft, the fuel pipe leaked allowing approx. 400 gallons of diesel fuel to accumlate in #2 sump pump containment area. The mine was evacuated at 9:55am as precaution. Evacuation complete 10:05am, ret'd 7:45pm. The sump was pumped out. There was no damage to the hoist.

Employee states he was operating a fork truck unloading a pallet in the warehouse. While backing out of the pallet rows, he was struck by another fork truck that was traveling from the dock area. He was seen by a physician and released for full duty. On April 30, 2007 he went back to the doctor and was not released to return to work.

The slow down switch for the #2 hoist failed to function properly. The mine was evacualted and the slow down switch was replaced with a new one. The hoist was tested and miners returned underground at 3:57 p.m.

Employee states he was moving a 55 gallon waste salt drum using a 3 wheeled barrel dolly. While moving the salt container, the car dolly encountered a damaged piece of flooring which caused the dolly to turn sharply in a different direction. When the barrel turned, his right foot contacted the barrel resulting in a twisting motion to his knee.

Lost power to #2 Hoist. When power to the hoist was restored, the level 1 sensor stop switch failed to function. The mine evacuation was complete at 9:18pm. The sensor switch was replace, the hoist was tested and miners ret'd underground at 12:55am

The #2 hoist motor relay contactor malfunctioned causing the hoist to not function properly. The mine was evacuated at 3:30am and complete at 3:42am. The defective contact relay was replaced, the hoist tested and miners returned underground at 6:10am.

The balance chain that attaches chain to cage and counterweight failed (broken chain link). The mine evacuation was completed at 2:45 p.m. A new balance chain was installed and the hoist was tested. Miners returned underground at 6:35 pm.

October 27, 2006, at 6:15 pm, an electical conductor to the #2 hoist slow down switch became disconnected. The mine evacuation started at 6:15 and was completed at 6:23 p.m. The hoist slow down switch wiring was repaired, the hoist tested and miners returned underground at 9:15 pm.

The #2 Hoist pinion set during hoist operation. The miners were evacuated at 9:10 a.m. and the evacuation was complete at 9:21 am. Time was added to pinion brake system to allow the brake to hold open and function properly. The hoist was tested and miners returned undergorund at 11:10 a.m.

Computer went down, evacuation started at 8:25 a.m. and completed at 8:37 a.m. the computer was rebooted at 8:45 a.m. and miners returned underground at 8:45 a.m.

Gate swithch came open as a result of salt acc- umulation loosened on platform because of platform cleaning. Miners were evacuated at 8:00 a.m. evacuation was completed at 8:25 a.m. the platform was cleaned, the hoist was tested and the miners returned underground at 9:00 a.m.

The #2 hoist slowed down & stuck at the bottom landing due to salt build up on timber lubricant. Miners were evacuated at 3:37pm. Evacuation completed at 3:51 pm. The hoist Timbers were lubricated, the hoist tested & miners returned underground at 4:15pm.

While scaling the #2 shaft, a lump fell and damaged internal conductors in the trail cable. The mine was evacuated at 1:55pm. Evacuation complete at 2:11pm. The three miners on the #2 cage reached the surface at 2:34pm. The cable was repaired, the hoist tested and miners returned underground on the evening shift at 5:30pm.

The hoist encoders failed on #1 hoist drive and interfered with hoist operation. The mine was evacuated at 8:40pm and completed at 9:05pm. The hoist encoders were replaced/repaired, the hoist tested and miners returned underground on the morning shift at 1:15am.

Lost power to #2 hoist while performing maintenance work. Evacuated mine at 1:35 p.m., evacuation complete at 1:50 p.m. Cause of outage was a tripped circuit breaker. Encountered difficulty opening breaker box for reset so mine was evacuated. Power was restored, the hoist tested and miners returned u/g at 2:00 p.m.

EMPLOYEE STATES HE WAS SLIDING AN 80 LB. BAG OF SALT ONTO A PALLET WHEN HE TWISTED & FELT DISCOMFORT IN HIS MIDDLE BACK. HE WAS DIAGNOSED WITH A THORACIC/LUMBAR MUSCLE STRAIN. RESTRICTED DUTY FOR 5 DAYS & MUSCLE RELAXANTS.

EE states he injured his back while operating a fork truck or picking up a 50 lb bag of salt. EE was seen by a physician on 3/27/06 & had X-rays. EE was instructed to remain off work until X-rays read. Physician was away from the office through 3/30/06. Diagnosed with siatica & EE instructed to remain off work and follow up visit on 4/10/06.

A city wide interruption by public utility company SWEPCO resulted in the mine being evacuated using the mine's secondary power source (diesel). The evacuation began at 11:25am & was completed at 12:25pm. The outage was projected to last for several hours so miners were sent home. SWEPCO restored power at 12:48 pm and mining operations were resumed.

THE NUMBER 1 HOIST FAILED AS THE RESULT OF A BROKEN COIL WIRE. MINERS WERE EVACUATED AT 6:27 AM AND THE EVACUATION WAS COMPLETED AT 6:38 AM. THE BROKEN WIRE WAS REPAIRED AT 7:15 AM, THE HOIST WAS TESTED AND THE MINERS RETURNED UNDERGROUND AT 7:20 AM.

The number 1 hoist failed as the result of a broken wire. Miners were evacuated at 1:20 p.m. and the evacuation was completed at 1:38 p.m. The broken wire was replaced at 1:45 p.m. The hoist was tested and the miners returned underground at 1:55 p.m.

The #2 hoist failed as the result of a faulty pneumatic timer & proximity switch. Miners evacuated at 8:45 am. The switch & timer were replaced. The hoist was tested & miners returned underground at 10:50 am.

EMPLOYEE STATES HE REACHED OVER THE STARWHEEL PLATE TO REMOVE A PEPPER CONTAINER FROM THE PEPPER SEAMER. WHILE DOING SO HE BECAME DISTRACTED, TURNED AWAY AND CAUGHT THUMB BETWEEN STARWHEEL FINGER AND CIRCULAR OPENING IN THE STARWHEEL BASE PLATE.

Generated power interrupted due to low water level in boiler. The mine was evacuated at 11:55 p.m., and completed at 12:04 a.m. The boiler level was restored, the hoist tested and miners returned underground at 12:22 a.m.

When mill started up, power was lost. Power was restored at 7:15 a.m., hoist was tested and miners returned underground at 7:25 a.m.

EE was attempting to remove a defective product (can of salt - approx. 1 lb.) from a conveyor belt when she lost her footing, slipped and fell to the floor at ground level.

EE STATES HE WAS OPENING THE SPRING-LOADED HATCH DOOR ON TOP OF A HOPPER RAILCAR IN PREPARATION FOR CLEANING. WHILE LIFTING ON THE HATCH TO OPEN, THE EE EXPERIENCED DISCOMFORT IN HIS LOWER BACK. INCIDENT REPORTED ON 8/5/05.

WHILE OPERATING THE CARTON MACHINE ON THE SECOND FLOOR, EMPLOYEE REACHED INTO THE MACHINE TO CLEAR A JAM. DURING THE PROCESS, HE PULLED HIS HAND AWAY FROM THE MACHINE AND CONTACTED THE BOX GUIDE RESULTING IN A LACERATION BEHIND HIS LEFT INDEX FINGER KNUCKLE.

LOSS OF SWEPCO ELECTRICAL POWER CAUSED BY ELECTRICAL STORM. MINERS WERE EVACUATED AT 4:16PM. ELECTR4ICAL POWER WAS RESTORED AT 4:45PM, THE HOIST WAS TESTED AND MINERS RETURNED UNDERGROUND AT 4:50PM

THE SECONDARY POWER SOURCE FOR THE #2 HOIST WAS BRIEFLY INTERRUPTED DUE TO A LOSS OF STEAM PRESSURE IN THE BOILER. THE EVACUATION OF MINERS WAS COMPLETE AT 5:10 AM THE STEAM PRESSURE AND POWER WAS RESTORED AT 5:25 AM AND MINERS RETURNED UNDERGROUND.

An electrical storm interupted power by tripping a breaker. The miners were evacuated at 1:30 pm and on the surface at 1:43 pm. The electrical issue was resolved, the hoist was tested and miners returned underground at 1:50 pm.

a seal in the main brake valve went out. The mine evacuation started at 8:55 am and was complete at 9:03 am. The seal in the main brake was replaced, the hoist tested and miners returned u/g 9:35 am.

CITYWIDE SWEPCO (UTILITY COMPANY) POWER INTERRUPTION OCCURRED AT 8:45 AM ON 5/21/05. THE MINE EVACUATION WAS COMPLETED AT 9:00 AM. POWER WAS RESTORED BY SWEPCO AT 10:20 AM. THE HOIST WAS TESTED AND MINERS RETURNED UNDERGROUND AT 10:35 AM.

BOILER THAT POWERS ELECTRICAL GENERATOR WENT DOWN, SWITCHED TO SWEPCO POWER AND EVACUATED THE MINE. BOILER AND ELECTRICAL POWER WERE RESTORED AND MINERS RETURNED UG AT 11:55 PM

Air flow safety device detected improper air/gas mixture and automatically shut down boiler which powers generator for power generation. Secondary power to the hoist was lost and evacuation commenced at 3:25 am and was complete at 3:39 am. Power was restored at 3:50 am, the hoist was tested and miners returned underground 4:00 am.

While assisting another employee in making an air flow adjustment to the Pellet Bagging machine, employee reports he lacerated his left index finger when it contacted the drive belt chain and sprocket at the top of the machine.

Employee was in office checking his mail box when he experienced chest pains. Employee stabilized and transported to the Hospital Emergency Room. Diagnosed as a mild heart attack by enzyme levels in blood.

Ee was assisting another ee in resecuring a vibrator screen. when tension was applied to the tension adjustment screws, the screen moved forward and struck ee s right thumb resulting in a linear fracture.

A FAILED FUSE AT THE MAIN SWEPCO SUBSTATION RESULTED IN A POWER INTERRUPTION THAT AFFECTED BOTH HOISTS. AUXILIARY POWER WAS DOWN AT THE TIME WHICH REQUIRED MINERS TO REMAIN UNDERGROUND UNTIL SWEPCO POWER WAS RESTORED AT 2:03PM. THE HOISTS WERE TESTED & MINERS RETURNED TO WORK.

LOSS OF SWEPCO ELECTRICAL POWER CAUSED BY LIGHTENING/THUNDERSTORMS. MINERS WERE EVACUATED AT 9:50 PM ELECTRICAL POWER WAS RESTORED AT 12:25 AM (6/2/04), THE HOIST WAS TESTED & MINERS RETURNED UG AT 12:55 AM.

A SHORT CIRCUIT OCCURRED IN THE HEADFRAME WIRING TO LIMIT SWITCHES. THE MINE WAS EVACUATED AT 1:40 PM & COMPLETED AT 1:55 PM. THE WIRING WAS REPAIRED & THE HOIST WAS TESTED. MINERS RETURNED UG AT 2:20 PM.

DUE TO INTERRUPTION OF POWER FROM SWEPCO, THE MINERS WERE EVACUATED AT 7:30PM. POWER WAS RESTORED AT 10:25 PM THE HOIST WAS TESTED & MINERS RET'D UNDERGROUND AT 10:30PM

Loss of electrical power caused by high winds/ storms. Miners were evacuated at 4:55 p.m. Electrical power was restored at 7:00 p.m., the hoist was tested and miners returned underground at 7:10 p.m.

INTERRUPTION OF POWER FROM SWEPCO ELECTRIC. MINERS WERE EVACUATED AT 9:48 A.M., AND COMPLETED AT 10:07 A.M. ELECTRICAL POWER WAS RESTORED AT 10:40 A.M., THE HOIST WAS TESTED AND MINERS RETURNED UNDERGROUND AT 10:45 A.M.

The encoders were giving false readings int he computer. the miners were evacuated at 5:40 p.m. and was completed at 5:53 pm. the switches were reset and zeroed encoders. the hoist was tested at 9 pm and the miners returned underground at 9:15 pm.

A contractor removing siding from the tipple headframe structure inadvertantly pulled a wire out of the upper track limit switch. The mine was evacuated at 8:50 a.m. and completed at 9:08 a.m. The limit switch was repaired and tested at 9:35 a.m. Miners returned underground at 9:40 a.m.

BREAKER OUT OF ADJUSTMENT AND HOIST WOULD NOT RESET. MINERS WERE EVACUATED AT 6:35 PM. HOIST WAS REPAIRED AND TESTED AND MIVERS WERE RETURNED TO UNDERGROUND AT 7:45 PM.

LOSS OF SWEPCO ELECTRICAL POWER CAUSED BY LIGHTING/THUNDERSTORMS. MINERS WERE EVACUATED AT 4:35 P.M. ELECTRICAL POWER WAS RESTORED AT 6:10 P.M. THE HOIST WAS TESTED AND MINERS RETURNED UNDERGR OUND AT 6:25 P.M.

EMPLOYEE STATES HE WAS TRANSPORTING PELLET PALLETS TO SOUTH DOCK STORAGE AREA WHEN THE FORK TRUCK HE WAS OPERATING CONTACTED A DEPRESSION IN THE ASPHALT PAVEMENT. THE CONTACT WITH THE DEPRESSI ON RESULTED IN A SUDDEN TWISTING MOVEMENT OF HIS UPPER BODY THAT RESULTED IN LOWER BACK PAIN AND DISCOMFORT.

THE MAIN #1 HOIST BRAKE FAILED ITS BRAKE CHECK. THE MINE EVACUATION STARTED AT 6:48 AM AND WAS COMPLETED AT 6:59 A.M. THE BRAKE WAS ADJUSTED AND THE HOIST TESTED. MINERS RETURNED U/G AT 8:00 A .M.

EMPLOYEE WAS ATTEMPTING TO REMOVE A DUST COLLECTOR BAG THAT HAD FALLEN TO THE BOTTOM OF THE COLLECTOR IN THE VACUUM SCREEN ROOM. WHILE PULLING ON THE STRAPPING, EMPLOYEE STATES HE EXPERIENCED A PAIN IN HIS RIGHT SHOULDER.

DURING THE #1 HOIST PRE-SHIFT INSPECTION, THE MAIN HOIST BRAKE FAILED TO HOLD. THE MINE EVACUATION STARTED AT 10:35 P.M. AND WAS COMPLETE AT 10:46 P.M. THE HOIST BRAKE WAS ADJUSTED, THE HOIST TESTED AND MINERS RETURNED UNDERGROUND AT 11:05 P.M.

ELECTRICAL POWER FROM THE INCOMING MAIN SWEPCO LINE WAS INTERRUPTED DUE TO MELTED CONDUCTOR. MINE EVACUATION STARTED AT 12:45 P.M. AND WAS COMPLETED AT 1:02 P.M. ELECTRICAL POWER WAS RESTORE D, THE HOIST TESTED AND MINERS RETURNED UNDERGROUND AT 6:05 P.M.

THE HOIST WAS SHUT DOWN TO REPLACE A DEFECTIVE HOIST BRAKE VALVE. THE MINE EVACUATION STARTED AT 10:30 AM & WAS COMPLETE AT 10:40 PM. THE VALVE WAS REPLACED, THE HOIST TESTED & MINERS RETURN ED UNDERGROUND AT 11:05 PM.
