unloading plate steel from loader bucket. plate got stuck. hooked chain on it. pulled on chain, plate slid out of the bucket. struck EE on top of right knee with front/outside corner.

EE WAS PROPING UP AN IRON PLATE TO MEASURE WHEN THE PLATE FELL ON HIS LEFT FOREARM CAUSING A HAIRLINE FRACTURE. NOTE: THE MACHINARY WAS COMPLETELY LOCKED OUT AND SHUTDOWN WHEN INJURY OCCURRED.
EE WAS ASSISTING OTHER EE WHEN HE WAS HIT ON FOREHEAD WITH A METAL PIPE WHILE TRYING TO TURN PULLEY ON CRUSHER EQUIPMENT.
