EE was moving a roll of flashing material & dropped the roll on a pallet of screen rails, causing a screen rail to whip up & hit EE in the left side of face.
Poor work practice, ee removing a maintenance platform by himself, stated he should have gotten a helper. Was unhooking a pull-behind compressor, one of the wheels was on a rock, when the compressor was unhooked, it spun around & smashed left index finger against the frame.
SECTION OF METAL SCALE SURFACE WAS LIFTED TO POWER WASH BASE OF OFFICE SCALES. AFTER CLEANING WORKER WAS WAITING FOR OTHER HELP TO LOWER SCALE TOP BACK TO NORMAL HE REMOVED SAFETY CHAIN AND LI FTED IT TO AVERTICAL POSITION THE METAL SECTION BROKE OVER CENTER AND CAUGHT THIS EE BETWEEN SURFACE AND BASE.
PUSHING ON PICK UP TRUCK AND STRAINED RIGHT ARM.
DRILLER WAS CHANGING STEEL WHEN TOP SECTION SLIPPED OUT OF STEEL RACK AND FELL. DRILLER ATTEMPTED TO CATCH IT AND CAUGHT IT WITH HIS HAND BETWEEN STEEL AND CENTRALIZER BROKE BONE IN LEFT HAND IN THE THIRD FINGER BETWEEN SECOND AND THIRD DIGIT.
LOWR BACK STRAIN CAUSED BY LIFTING A BOX OF HYDOMITE BLASTING MATERIAL.
