Employee was installing lock ring on a 29.5 x 29 water filled tire. As he was lifting the ring onto the wheel, the ring slipped from his gloves, slid down his leg and landed on the side of foot. Fracturing his baby toe.

Employee bent down to pick up a piece of metal when he stood up, made note of slight pain. He continued working making no other complaints and finished his work day. REPETITION OF GETTING UP AND DOWN OUT OF HER CHAIR IN THE SCALES OFFICE. LOWER BACK WAS HURTING SO WENT TO SEE DOCTOR. DOCTOR FOUND A HERNIATED DISK AND PRESCRIBED TWO WEEKS OFF WITH FURTHER OBSERVATION.
EE SAID HE HURT HIS BACK LIFTING A CUTTING EDGE ON MONDAY, ON THURSDAY HE SAID HE HURT HIS BACK ON WEDNESDAY PRYING ON A ROCK FROM THE JAW OF THE CRUSHER.
