(See Attached) EE loading powder truck with 50 lb. bags of ANFO. EE stated he had aggravated his lower back and needed to go home and lay down. EE RTW 7/3 and worked full shift. We did not work on 7/4. EE came in 7/5 prepared his truck and started work. At 7:30 a.m. EE came to my office and stated he would make a doctor's appt. because his back was still painful.

EE was using a shovel to remove partially frozen Ag lime that was stuck in the bucket of the loader. He believes he strained his back at some time in this process. He did not notice any pain at this time. He had pain when he got up in the morning of 2/8/07. He came to work and advised that he couldn't climb in out of the loader and was going to see his personal physician.

Inj EE & another EE were changing positioin of flop gate at feed end of 6x16 AC screen. Gate consists of a plate attached to a shaft. Gate weighs 100lbs. They tried to reposition gate, shaft broke. EE tried to hold weight of gate. Caused back pain EE went to chiropractor on 1/23/06 has possible compression frac. Excused fro work tl 1/30/06, follow up w/Dr 1/27/206

INJURED WAS ADMITTED TO CARTHAGE MEMORIAL HOSPITAL 12-27-93 WITH PNEUMONIA AND AN INFECTED ELBOW. I FIRST SPOKE DIRECTLY WITH HIM ON TEH EVENING OF 12-5-93. AT THAT TIME HE SHOWED ME HIS ELB OW AND TOLD ME HE HAD TO HAVE BUMPED IT AT WORK A FEW WEEKS AGO. HE DID NOT KNOW HWEN WHERE OR HOW THIS INJURY OCCURRED THIS WAS THE FIRST KNOWLEDGE I HAD THAT THE INJURY OCCURED AT WORK. TH

95149ED WAS PLACING A DRILL STEEL THAT HAD JUST BEEN REMOVED FROM BORE HOLE INTO THE STEEL RACK ON THE DRILL.HE CAUGHT HIS MIDDLE FINGER OF HIS RIGHT HAND BETWEEN THE STEEL HE WAS PLACING IN T 85091CK AND ANOTHER ALREADY IN THE RACK.

CLIMBED IN BIN TO RETRIEVE A TAPE. CLIMBING OUT OF BIN EXPER LITTLE PAIN IN L WRIST . WRIST SWOLLEN PAINFUL DR EXAM AND X RAY DEGENERATIVE ARTHRITIS IN JOINT.

I. NAME WAS LOADING DRILL STEELS IN THE BACK OF PICKUP TRUCK HE DROPPED ONE STEEL AND IT CAUGHT HIS THUMB BETWEEN ANOTHER STEEL ALREADY LOADED IN THE PICKUP TRUCK
