Employee was changing screens in cyclone drain, when they rinsed the screen, magnatite and fine coal splashed in face getting into left eye. Went to Doctor, said they had scratched left eye.

Employee has strained back from continuously exiting and entering service truck.

Employee was helping work on 785C Truck when a metal plate fell off the fender striking the employee in back of head. Went to ER and got (2) staples in back of head.

Employee was at raw coal feeder when employee bend over to a rock out of the coal on the feeder, when employee raised up employee felt a pain in lower back.

Employee was taking cylinder off a D9T when EE was taking bolt out of light bracket a pin flew out of wobble socket, struck left hand cutting it. EE went to ER in Pineville, Ky. received (5) stitches and back to work 03-18-16.

Employee was changing blade on D-9 dozer when he caught his left index finger between blade and frame smashing index finger. He was taken to Pineville Comm. ER room, finger is broken and he received stitches.

Employee was working on a 800 Service Truck, when he picked up a clutch & pressure plate he felt a pain in his left groin area.
