On 11/1/07 employee was aligning a crawler assembly when he felt pain in his left shoulder. He continued working until 11/21/07 when he sought medical attention and was prescribed restricted duty.

Employee was pulling wire out from under rocks when he felt pull in his left shoulder. He continued working until 12/18/07 when he sought medical attention and started missing work.

Employee was unloading I-beam from scoop bucket when he pinched his left little finger between beam and bucket, causing laceration.

Employee felt pain in his right bicep and shoulder when he was pulling cable.

Employee was pulling and hanging cable, when he felt pain in his lower back. He sought medical assessment and started missing work on 6/13/07.

Employee was picking up 2x4 when he felt pain in his lower back.

Employee was getting into battery fork lift when he felt pain in his lower back. He continued working until 2/13/07 when he sought medical assessment and started missing work due to lower back strain.

Employee was pulling on cable, when the cable caught on a chunk and employee felt pain in his lower abdomen. Employee had hernia surgery on 2/15/07 and began missing work.

Employee was filling cap lamp battery when a piece of corrosion flew off, striking him in his left eye. Employee sought medical treatment from eye doctor and was given prescription medication.

Employee picked up a crib and twisted to throw the crib when he felt pain in his lower back.

After roof bolting, employee was assigned to remove and cover shield electrical connection. At some point, employee felt a foreign object in his right eye.

On 7/25/06 employee was installing longwall jumbo track and on 7/28/06 employee was hanging 13.8 cable. On 7/31/06 employee reported pain in back, shoulders and neck as a result of these two work tasks and began missing work. Verification was received on 8/10/06 relating her injury to the 7/25/06 incident date, with further aggravation on 7/28/06 task.

Employee was pulling down a piece of line curtain and felt strain to his left rib cage and back. He continued working until 7/10/06 when he sought medical assessment and began missing work.

Employee's left arm came in contact with guarding causing a laceration while attempting to install drive belt on plant blower fan motor. Employee received fourteen stitches to close laceration.

Dust particle fell into employee's left eye while he was plastering a ventilation wall, causing a corneal abrasion. Employee was given prescription medication for infection.

Employee felt a sudden pain in his right arm as he attempted to a lift a 38-lb. plastic tote box onto the back of a supply vehicle. Employee ruptured the distal biceps tendon on his right arm.

Employee was descending steps when he felt pain in his lower back.

As employee turned on water to wash longwall shield toes, the hose slipped from his hand, knocked his safety glasses off and struck his left eye, causing severe eye injury.

Employee pulled on cord to shut airlock door and reported that a small foreign particle (dust?) entered left eye. Particle was removed from under EE eyelid and on 1/31/05 EE was treated with steroids for allergic reaction to foreign particle.

During process of repairing crusher in Prep Plant, employee's finger was pinched between sprocket and gearcase. Employee received four sutures to close laceration. Employee was released to return to work with restrictions.

Employee was assisting in off loading a CLA fork (approximately 150 pounds) from supply truck when his left hand was caught between fork and truck bed. The tips of his middle and ring fingers were fractured.

Supervisor pulling on alignment pin when it shifted, pinching his left middle finger between vibrating screen mechanism and pin. Finger was lacerated and required sutures.

Employee was lifting a tool box when he felt pain in his right shoulder. On 11/4/04 he had surgery to repair a tear in his right shoulder and began missing work.

Employee operating D-8 dozer when it started running hot. As he prepared to add more antifreeze, he loosened radiator cap and hot antifreeze came out hitting his upper back and left arm. He was treated for burns and released on restricted duty.

Employee pouring concrete when his legs came in contact with the wet concrete above the top of his protective boots. He continued working until 7/13/04 when he was diagnosed with infection in his legs and was assigned restricted duty.

Employee was removing a bearing and shaft assembly when the shaft fell, striking the employee on the left leg and causing laceration. Sutures were required to close laceration.

Employee was helping repairman to put steel cover back on continuous miner when his left little finger was caught between frame and cover, causing laceration and sliver fracture to finger. Sutures were required to close laceration.

Employee was shoveling gob and rockdust for 10-hour shift. After shift he stated he had soreness in his right shoulder. Employee continued working on regular duties until 5/17/04 after he was diagnosed with possible rotator cuff tear and was assigned restricted duty pending surgery.

Employee was injured while responding (as a mine EMT) to an incident. Employee experienced pain in his right leg/groin after sitting in an awkward, confined position, followed by efforts to help lift an injured employee onto stretcher.

Employee was shoveling into a pile when the pile gave way and the sudden movement caused undue strain on his right knee.

EE WAS SEARCHING IN PARTS DRAWERS LOCATED IN UNDERGROUND SHOP. THE BOTTOM DRAWER WAS STUCK AND WHEN EE GAVE IT A QUICK PULL, HE FELT PAIN IN HIS LOWER BACK.

SOME TYPE OF MATERIAL ENTERED EE'S LEFT EYE UNDER HIS SAFETY GLASSES. HE DEVELOPED AN INFECTION IN HIS EYELID AND WAS ASSIGNED TO RESTRICTED DUTY.

NEW FACE CONVEYOR SECTIONS WERE BEING INSTALLED THE INJURED EE WAS HELPING TO ALIGN STEEL HAULAGE TRACK ASSEMBLY WHEN ANOTHER EE REMOVED MOVABLETRACK ASSEMBLY AND STEEL CONVEYOR CASTING. EE SU STAINED A CRUSH INJURY TO THE END OF HIS FINGER.

EE WAS REMOVING STEEL BELT GUARDING TO GAIN ACCESS TO TAKE-UP ROLLER SENSOR ON LONGWALL CONVEYOR BELT DRIVE. THE SECTION MOVED PINCHING EE'S FINGERS ON HIS RIGHT HAND, CAUSING LACERATIONS TO R ING FINGER AND LITTLE FINGER. EE RECEIVED 20 SUTURES TO CLOSE LACERATIONS.

EMPLOYEE WAS MOVING A PIPE THAT WAS APPROX 20 FT LONG AND WEIGHED AROUND 40 POUNDS, WHEN HE FELT PAIN IN HIS LOWER BACK. EMPLOYEE RECEIVED MEDICAL TREATMENT INCLUDING PRESCRIPTION MEDICATION.

ON 1-20-03 EE WAS PULLING TO RELEASE CABLE REEL PRESSURE WHEN HE FELT PAIN IN HIS LEFT EOBLW. ON 10-27-03 EE HAD LEFT TENNIS ELBOW RELEASE SURGERY AND RETURNED TO WORK ON RESTRICTED DUTY.

EMPLOYEE WAS LIFTING OVERHEAD TO HANG AIRLINE WHEN HE FELT PAIN IN HIS NECK AND UPPER BACK.

EE WAS INVOLVED IN COMPLETING INITIAL STEPS OF LONGWALL MOVE. ONE STEP REQUIRED USE OF AIR DRILL TO ANCHOR WIRE ROPE TO MINE ROOF. EE WAS DRAGGING DRILL AND AIR LINE ACROSS LONGWALL FACE WHEN HE FELT PAIN IN HIS LOWER BACK. EE WAS PLACED ON RESTRICTED DUTY.

DRILL AUGER STUCK IN ROOF. WHEN EE REACHED FOR AUGER, IT FELL FROM HOLE AND PINCHED HIS LEFT HAND BETWEEN TWO AUGER PIECES. THIS RESULTED IN A LACERATION TO THE LEFT HAND PALM AREA WHICH REQUI RED SEVEN SUTURES TO CLOSE.

EMPLOYEE WAS SITTING DOWN AND BENDING OVER PICKING UP LARGE ROCKS WHEN HE FELT A STRAIN IN HIS LOWER BACK.
