While moving lubrication oil barrel, barrel slipped from one employee's hand and pinched other employee's finger.

When attempting to move a skid steer bucket, on a table, for repairs, employees left hand got caught between the bucket and table, injuring his hand.

Pulling table bushing out of a drill and aggravated his hernia from July, 2011. Went to see a doctor on September 27, 2011.

Pulling on heavy dragline chain to get it in place. Employee did not seek medical advice until approximately a week later and then was diagnosed with a hernia. After a follow up consultation with another doctor on 7/1/2011, employee in the near future is looking to have hernia surgery.

Employee pulled too hard, on a 7' long 4x4 piece of hardwood that was being used to create a lever to close gate on crusher, receiving sharp pain in groin area.

HE BENT DOWN AND PICKED UP A 6X6 TIMBER AND AGGRAVATED AN OLD BACK CONDITION.

HE WAS HOOKING BUCKET UP TO FRONTEND LOADER & HE HAD CABLE STRUNG FROM LOADER TO BUCKET & HE TURNED & BRUSHED CHEEK ON CABLE.

HE WAS INSTALLING A REAR-END DIFFERENTIAL ASSEMBLY IN A TRUCK THERE WERE NO ILL EFFECTS UNTIL 10 DAYS LATER (WHILE EMPLOYED BY JONES & BRAGUE IN 1979 HE DEVELOPED A HERNIA IN A DIFFERENT AREA OF HIS BODY WHICH WAS REPORTED)

HE WAS MOVING A3DRUM OF OIL AND DEVELOPED A HERNIA.

THERE WAS NOT AN ACTUAL INJURY DATE. THE MAN HAS A HEREDITARY PROBLEM WITH HIS BACK IN THAT THE VERTEBRAE ANDLIGAMENTS HAVE DETERIORATED OVER A PERIOD OF TIME. MR. THOMAS DID STRAIN HIS BACK W HILE MOVING AN OIL BARREL WHICH HE SHOULD NOT HAVE BEEN DOING. THIS AGGRAVATED THE PROBLEM WHICH HE HAD IN THE PAST.
