Moving a welding machine using a forklift. He stopped the forklift, set the parking brake, and turned it off after he lowered the welding machine onto the ground. As he was unhooking a stabilizer chain the welder shifted on the ground pulling the chain and the tip of his index finger was mashed between the chain and the forklift mast. Was wearing leather gloves.
Employee was moving a piece of grated flooring material and it brushed against his forearm, causing a laceration.
Employee was removing a guard from a pump in the basement area when it shifted and the corner hit his left arm causing a gash requiring stitches.

WHILE TRYING TO COUPLE 8" PIPE TOGETHER, EE EXPERIENCED PAIN SPANNING HIS BACK AND RIGHT LEG. EE WORKED UNTIL 3-1-02, BUT COULD NOT DEAL WITH THE PAIN. THIS PROMPTED MEDICAL ATTENTION, AND IT WAS FOUND THAT HE HAD A TORN MUSCLE IN THE SCROTUM. REPAIRS AND RECOVERY ARE UNKNOWN. ORIGINALLYFILED AS A 'FILE ONLY', BUT HAS NOW BEEN DETERMINED AS AN LTA.
EE WAS ROLLING UP TARP ON HIS TRUCK & PULLED DOWN THE TARP BAR TO HOOK UP THE CHAIN & SEEMED TO HAVE CAUSED A LOWER LUMBAR STRAIN IN HIS BACK
