Mostrando los 46Employee was pushing the rubber basket in the pre-cleaner housing on a D11R when his left hand slipped and he struck his left wrist on the adjacent bolt.
Injured employee was handling a sixty inch long, 6 1/2 inch diameter piece of drill steel used to anchor the highwall miner when he slipped and fell. The end of the employee's left pinky finger was mashed between the steel he was handling and the steel structure of the highwall miner.
EE was carrying tool tray onto D11R dozer, right elbow popped and started burning.
Employee was handing employee a string of insulators. They slipped out of his hand and hit injured in mouth damaging his 2 front teeth.
Employee states he was changing a Western Star tire, he was rolling the tire and it almost fell over; he grabbed hold of it to keep it from hitting the pickup causing him to hurt his back.
Company notified by area hospital that a claim was being filed two (2) days after the alleged incident occurred. Employee returned call on Thursday afternoon and stated that he hurt his back lifting truck hood. "Report filed under protest."
Employee was stepping down from a ledge and his foot went into about 10 inches of mud. While trying to take a step he pulled his knee trying to dislodge his foot. This event was not reported for 3 days.
Employee was picking up 2 full boxes of Pentex 3/4# boosters when he felt "it pull in lower abdomen & back".
Employee was lifting a full box of Pentex 227 primers out of day box on a truck, when he felt his shoulder blade begin to "burn".
EE stated he was changing a tire; as he was lifting the tire, he felt a pain in his side.
Employee was standing on top of truck under powder bin. While holding onto the safety rail, he attempted to push open the door to one of the truck's bins and twisted his knee.
When shutting door on Cat water truck, employee alleges he hit left elbow with door.
While loading materials in hole there seemed to be a chemical reaction causing black/green smoke to come out from hole.
Injured employee was going up a retrievable boarding ladder on an endloader when the ladder gave 4-6 inches causing his left hand to get caught between a handrail and a platform resulting in a laceration requiring sutures.
Employee stated that he hit left knee on front bumper of coal truck during pre-shift inspection on 07/10/06, but did not seek medical attention until 08/11/06; the diagnosis code description was "dermatitis". There were no witnesses. Employee did not stop work for the alleged injury. The incident/injury is being disputed by management.
Injured employee was climbing off the roof of the B E 495 shovel when his left knee struck a unistrut clamp causing a laceration to the knee. The laceration required five sutures to close. Employee immediately returned to work after medical treatment.
Employee was shoveling dirt into drill hole. He felt pain in his groin area. Saw doctor on 6/29; given no work restrictions until lab results.
EE started that he was cleaning the truck. He accidentally sprayed Armor All into his right eye.
Employee was installing drawer slides in a service truck when he mashed his right hand between drawers. He went to local hopital ER on 2/21/06 after right little finger kept swelling up. He was diagnosed with a fracture of the first digit of the finger. Employee returned to work the same day and has lost no time.
Employee stated that he was moving a 55 gal drum of oil, the drum slipped from metal surface, smashing his hand betwene the drum and a metal pole.
EMPLOYEE WAS LIFTING BOX FROM FLOOR OF MAGAZINE TO LOAD ONTO TRUCK. HE FELT A POPPING IN HIS BACK BELOW SHOULDER BLADES. SAW ER PHYSICIAN & WAS GIVEN LIGHT DUTY (LIFT NO MORE THAN 10 LBS) UNTIL 9-6-05
EMPLOYEE STATED THAT HE CUT HIS RIGHT HAND WHILE MOVING OIL BARRELS.
employee was riffling a coal sample, lifting buckets, when he felt a "pop" in his back
Injured employee was standing behind dozer track as it was being aligned to be installed. The end of the track was elevated. The end of the track fell striking employee on the left leg.
Employee was transporting bed wear strips with a fork lift. Two wear strips fell off. He placed one strip back on the fork lift. He picked the end of second strip up and twisted his back as he was trying to get it onto the fork lift.
Employee was assisting another employee dispose of reject coal. He lifted a barrell of reject coal and felt a sharp pain in the groin area
EMPLOYEE WAS ENTERING THE CAB OF A CATERPILLAR 988F ENDLOADER. AS HE SAT HIS LUNCH BOX ON THE CAB FLOOR, HIS HARD HAT FELL OFF. HE ATTEMPTED TO CATCH THE HARD HAT WHILE BENDING OVER. HIS MOUTH CONTACTED THE SUPPORT GUARD SURROUNDING THE JOYSTICK CONTROL. THE CONTACT DAMAGED A FRONT TOOTH.
INJURED WAS OPERATING GROVE 60 TON CRANE. HE HAD GOTTEN OFF OF THE CRANE TO CONFER WITH THE MECHANICS ABOUT A BOOM REPLACEMENT ON ONE OF THE LOADERS. AFTER CONFERING WITH THE MECHANICS, HE WEN T BACK TO THE CRANE. AS HE STEPPED UP ON THE 1STACCESS STEP HE FELT A POP & PULL IN HIS RIGHT LEG. HE CONTINUED TO WORK & COMPLETED HIS SHIFT. HE SOUGHT MEDICAL ATTENTION THE FOLLOWING DAY.
INJURED EMPLOYEE WAS REELING UP A FUEL DELIVERY HOSE THAT WAS DRAPED ACROSS THE CRANE OUTRIGGER. WITNESS EMPLOYEE WAS HOLDING THE FUEL NOZZLE AT THE END OF THE HOSE AND WHEN HE CAME TO THE OUT RIGGER HE DROPPED THE NOZZLE. THE NOZZLE SWUNG IN AN ARC FROM THE REEL STRIKING THE SUBJECT EMPLOYEE'S RIGHT ANKLE. THE WIGGINS NOZZLE WEIGHS APPROX 15#.
EE & CO-WORKER WERE REMOVING DEBRIS FROM MAGNET. AN END LOADER BUCKET WEAR PLATE WAS ATTACHED TO MAGNET. EE ATTEMPTED TO PULL WEAR PLATE AFTER A ROPE WAS ATTACHED BACK UP THE BELT TO PREVENT I T FROM FALLING INTO CHARGE CHUTE. WHEN THE POWER WAS REMOVED FROM MAGNET & AS HE PULLED, WEIGHT OF WEAR PLATE PUSHED HIS LEFT ARM DOWN ON A PIECE OF PLYWOOD PUT UNDER MAGNET TO CATCH OTHER DE
EE ALLEGES HE WAS PULLING FUEL HOSE THROUGH MUD AND TWISTED HIS BACK. HE DID NOT SEEK TREATMENT UNTIL 11-11-02, HIS FIRST DAY BACK TO WORK FROM THE WEEKEND. HE REPORTED A NLT INJURY AFTER HIS SHIFT ON 11-7-02, THE LAST DAY IN HIS WORK WEEK.HE REPORTED TO HIS SUPERVISOR 11-13-02 WITH A LIGHT DUTY SLIP-DECLINED.
EMPLOYEE WAS HELPING REMOVE A DRIVELINE OUT OF A COAL TRUCK. HE WAS LIFTING DRIVELINE OUT FROM UNDER TRUCK AND ALLEGED THAT HE STRAINED HIS BACK AND NECK.
MAN WAS SHOVELING. DRILL CUTTING INTO HOLE ON H CUT 3 DRAGLINE PANEL. MAN STATED HE TURNED AND FELT SOMETHING POP IN HIS BACK. HOSPITAL DIAGNOSED A SLIGHT MUSCLE PULL. CHIROPRACTIC TREATMENT.
TANKER TRUCK DRIVER WAS ON TOP OF UNIT CLEANING DOWN THE INSIDE OF HIS TANKER. WHILE WORKING HE FELT A DISCOMFORT IN HIS BACK. INCIDENT REPORTEDON 6-17. EE WENT TO DR. ON 6-25. PRESCRIBED LIGH T DUTY ON 6-25.
EE WAS HELPING SHOVEL OPERATOR APPLY GREASE ON P&H 4100 SHOVEL STICK WHERE WELDING REPAIRS HAD BEEN MADE. WAS USING 5-GAL BUCKET OF GREASE WHEN HE BENT OVER & OVEREXTENDED RESULTING IN PAIN TO LOWER BACK. CONTINUED TO WORK REMAINDER OF SHIFT & WORKED FOLLOWING DAY. DIDN'T REPORT TO WORK JUNE 9 DUE TO BACK PAIN. SUFFERED MUSCLE SPASMS @ HOME ON 9TH & WENT TO HOSPITAL BY AMBULANCE.
MOVING METAL GRATING GOT RIGHT HAND CAUGHT BETWEEN GRATING AND BRACING. CUT TWO FINGERS ON RIGHT HAND.
EMPLOYEE WAS WORKING ON A ROCK SPRING FOR A RD 800 MACK COAL TRUCK. HE HAD SPRING LAYING BETWEEN FORKS OF A FORKLIFT IN A RAISED POSITION. HE GOT UNDER SPRING TO TIGHTEN CENTER BOLT, SPRING FELL ON HIM, STRIKING AND FRACTURING RIBS NO 6 AND 7.
EE WAS REMOVING A GUARD FROM THE COLLECTING BELT ON THE BASEMENT FLOOR. OTHER EE'S WERE WASHING DOWN THE FLOOR ABOVE. AS HE WAS REMOVING THE GUARD, A RUSH OF WATER CAME FROM ABOVE. HE TRIED TO GET OUT OF THE WAY AND AS HE STOOD UP, HIS HEADHIT A PIECE OF CHANNEL STEEL SUPPORTING THE REFUSE BELT. HE SUFFERED A LACERATION ACROSS THE BRIDGE OF HIS NOSE AND A BLACK EYE.
FEET STUCK IN DEEP MUD. SPRAINED R ANKLE REMOVING FOOT FROM MUD.
EMPLOYEE, WITH THREE OTHER MECHANICS, WERE CONDUCTING PREVENTATIVE MAINTENANCE ON A CAT DOZER. ONE OF THE MAINTENANCE PERSONNEL HAD PLACED A SLATE BAR LEANING AGAINST THE LEFT SIDE PUSH ARM. H E WENT TO PICKUP TO MOVE A WOODEN CRIB BLOCK. ASHE RAISED BLOCK, HE BUMPED THE BAR CAUSING IT TO FALL. IT STRUCK TOP OF HIS LEFT THUMB, CRUSHINGTHUMB.
EE CLAIMS HE PICKED UP A TURNDOWN TO PUT ON A LINE TO A SUMP. WHEN HE PICKED UP THE TURNDOWN TO PUT IT IN PLACE HE HAD TO TWIST WITH IT TO GET AROUND A 4" LINE THAT WAS IN HIS WAY. WHEN HE DID THIS HE FELT A SHARP PAIN IN HIS RIGHT LOWER BACK.
NOTE: THIS REPORT IS FILED UNDER "PROTEST". EMPLOYEE STATED HE HURT HIS RIGHT SHOULDER WHILE PULLING HIMSELF UP BOARDING LADDER ON CAT 988B LOADER. HE ALSO STATED THAT HE INJURED THIS SHOULDER 6 MONTHS PRIOR WHILE TRIMMING TREES AT HOME. MEDICAL RECORDS SHOW EE HAD LAST SEEN DR ON 9/11/00 FOR TREATMENT OF SAME SHOULDER & THAT HE POSSIBLY COULD HAVE A PARTIAL TEAR OF ROTATOR CUFF.
EMPLOYEE WAS REPLACING A SCREEN IN THE REFUSE SIEVE BOX. INJURED WAS LIFTING UP ON ONE END OF THE SCREEN IN AN ATTEMPT TO GET THE SCREEN PROPERLY ALLIGNED. AS HE WAS LIFTING AND PULLING, HE FE LT A SHARP PAIN BETWEEN HIS HIP AND GROIN AREA. ON 9-28, HE FELT A KNOT IN HIS GROIN AND SAW PHYSICIAN. PHYSICIAN DIAGNOSED HIM WITH A HERNIA IN THE GROIN AND SCHEDULED A REPAIR OF HERNIA.
EMPLOYEE WAS WORKING ON ADJUSTMENT IN THE HEIGHT OF THE SEAT IN THE KOMATSU LOADER. AS HE WAS MAKING THE ADJUSTMENT HIS RIGHT HAND SLIPPED OFF THE ADJUSTMENT KNOB. HE WAS BOUNCING IN THE SEAT ATTEMPTING TO GET THE RIGHT ADJUSTMENT. THE END OF HIS RIGHT MIDDLE FINGER ENTERED THE AREA OF THE SCISSOR MECHANISM AND WAS MASHED AS HE PLACED WEIGHT ON THE SEAT.
WHILE REPLACING DRIVESHAFT ON FUEL TRUCK, SHAFT SLIPPED FROM GRASP AND DROPPED ONTO RT WRIST. THIS CAUSED A KNOT TO RAISE ON THE RIGHT WRIST. THIS MECHANIC WAS WORKING WITH A HELPER.
