Employees were replacing protective wing shroud on the left side of the bucket on the 6020 excavator. While attempting to put the shroud in place, the shroud dropped unexpectedly pinching EE's fingers between the shroud and edge of the bucket. The shroud weighing approximately 40 pounds fell 10 inches before striking the employee's left 3rd finger causing fracture and laceration.

Mostrando 50 de 72Employee was performing routine maintenance on the dragline using hand tools to remove dirt shoot from in front of the drum of the dragline. While lifting the dirt chute up, the employee's finger got pinched between the chute and frame of the upper platform.
Employee lifted leg to step on a tie rod to get to steps. When EE lifted EE's knee it popped before stepping on the tie rod.
Employee was placing machinery teeth on a pallet to be moved with the skid loader. A tooth fell off and landed on the ground and employee decided to pick up the 100+ pound tooth and place it back on the pallet while the skid loader was still in motion. Employee felt a pop in lower back and had instant pain
While the contractor, smucker builders, crew was using a brut post grabber to set the 16th pole of the day for a pole building construction, the 8"x8"x32' pole slipped in the brut pole grabber. It then fell forward knocking down the last pole set, which landed on the brace of the next pole back. the 2x4 wood brace broke and stuck EE in the back of the head as EE moved away.
Employee was lifting a ladder to support PCR on dragline.
Employee was removing a piece of drag rope from inside the shovel of the 7450 dragline. While moving it over EE walking shaft, EE lost control of the drag rope and it struck EE in the arm causing a contusion in left upper arm.
The employee was working in the Logan breaker picking breaker rock. While unwedging a large rock that became wedged on the picking table between the divider and the side of the table, the employee put left hand on the side of the table to brace self near the feed chute and a rock came in and hit the tip of the ring finger
Employee was dumping tractor trailer in bagging plant hoppers. They reached up to pull tailgate latch and it would not open. So they pulled harder to get it open and strained a shoulder. Employee did not seek treatment or have lost time until 6/15/22.
Employee was in the process of placing the protective cap on an empty acetylene cylinder. When EE went to tighten the cap, it became stuck and employee tried to jar it loose. While in the process, the cylinder started to tip over and pinched finger between the cylinder and storage cap. Caused laceration to left middle finger requiring 2 stitches.
While picking a lump of coal off the Cleaning Plant picking table, a rock of unknown size collided with Employee's Right Pinky Finger
REPLACING GREEN WRAPPING AT THE BAGGING PLANT. EMPLOYEE PLACED RIGHT HAND ON THE CENTER CAP AND LEFT HAND UNDER THE WRAPPING ROLL TO LIFT IT UP. THE CENTER CAP FELL OUT AND THE ROLL FELL SMASHING A FINGER BETWEEN THE ROLL AND A PIECE OF IRON CAUSING THE TOP OF LEFT HAND MIDDLE FINGER TO CUT OFF NEAR THE CUTICLE.
Employee was on floor welding plug hole for plates in 777 haul truck box, employee needed more lead so they crawled to suitcase welder, grabbed it with right hand and gave it a yank.
HURT BACK WHILE PUTTING TOP SEAL BELTS BACK ON THE BAGGER.
When lowering a pump into the pit, the excess hose likely slid down over the highwall and snapped the portion of the hose laying on the bench tight, which caused the portion hanging over the edge to jump over some rocks that stuck out from the highwall and straighten out across the bench, snapping the hose back towards the pickup and striking employee in the leg.
Dirt fell off of hard hat when taking it off at the end of the shift while cleaning up. It got into EE's eye causing an infection.
Re-installing screens to AC vibrating screen deck, a steel hold-down strap sliced hand due to a jagged edge along its length when passing it to another employee.
While cleaning and prepping rail cars, EE felt a strain in EE's lower back while bent over. Employee reported the accident on 4/16/2019. EE did not seek medical attention until 8/14/2019.

Employee was lifting grate and pain went into right groin.
Injured employee and another employee were taking 4 lengths of square tubing off the rack to be cut in half. Injured employee was bending over to lower the injured employee's end to the ground and hurt back.

Lifting suction pipe. First thought had pulled a muscle in lower groin, but 2 weeks later it popped out.
while moving broken baffle out of coal line, a sharp piece of angle iron came in contact with employee's top of hand causing a cut.
Employee was moving an almost empty can of welding rods to replace it with a full can. When picking up the can a sharp edge cut finger.
Crouched down to remove a light fixture from 7450 dragline and hold it until the bolts were removed. Upon standing, felt pain in lower back.
Preparing to fuel a piece of equipment from the fuel truck. When starting the pump, fuel oil squirted from a hole in the hose, striking employee in face.
Employee hurt back from lifting teeth for DeMag while building up the base.
Employee was getting out of cab of truck and grabbed handle to turn to go to back of truck. He stepped on back plate/battery box cover to bend down and lift ring off gate handle. He then pushed gate handle down and upon standing up had sharp pain in lower back and right leg.
Was putting a jack under the truck and bent down to pick up an air hose and pulled his back. He did not receive medical treatment until 4/28/15.
Rock picking: removing a rock from the coal and pulled shoulder.
Employee was grinding rust and slag from a chute. He was using a 4" grinder and wearing tight fitting, clear goggles. When he removed his goggles, debris from the top of the goggles and/or forehead fell into his eye.
Broken side cutter was removed from demag bucket with welding truck crane. Hook-on cable got welded in point sheave. Welding partner lowered winch causing slack on the reel. Employee yanked on broken piece which came down on his finger because of slack.
Moving a drill steel rod. Rod was on the ground, when being lifted it shifted landing on employee's ankle.
While reaching into re-attach broken chain line, employee got his finger against a sharp piece of plastic on a return idler resulting in laceration.
After moving CAT 375 excavator, employee climbed down off the machine. He was holding the hand rail with his right hand, twisted while stepping off, and felt his right shoulder pop.
Employee was house cleaning, sweeping the floors. He was wearing gloves and reached into wire mesh screening to move it and cut his right middle finger.

He pulled pin to remove hook and pulley, block spun back and hit his arm.
Employee was picking rock from rock table and another rock hit his finger.
Lifting bag to add lime to water treatment system and he felt something full in his back.
Pulling air hose from under equipment and lacerated left hand on sharp object.
EE states that he was installing a grate in floor at the truck garage, he was removing a piece o metal grating off the truck when the metal grating slipped and struck 2 fingers on his hand.
