While operating the telescopic forklift, the miner opened the left-hand door, high wind pulled the door from the miner's grip, broke safety strap, the door hit the frame and shattered the glass creating flying debris. Debris was removed from beneath the eyelids, an ointment applied, the mine returned to work.

While installing a pin in the bucket cylinder of the PC2000, the Miner had their foot positioned under a pallet board. The pin came loose from its rigging and struck the pallet which in turn struck the Miner's left foot behind the steel toe shoe.

While walking from Miner's Epiroc d-65 drill to talk to the operator of another drill, the Miner got dust in right eye which caused Miner to rub eye and dislodged contact. Miner's eye became increasingly irritated and Miner sought medical attention on the second day. Miner was diagnosed with a scratched eye and has been treating for the same.

While removing a rock from the rock picking belt, the miner lost grip of the rock causing it to fall and strike their right toe beyond the steel toe protection. A blood blister developed the next day and the Miner sought medical attention and had it drained the following day. Infection developed and the Miner was admitted to the hospital two days after it was drained.

While adjusting the arm of a new pair of safety glasses, the glasses slipped beneath the Miner's thumb while applying down pressure causing the opposing hand to swing up and towards the Miner's face while holding the glasses. The arm of the glasses struck the Miner's left eye causing a tear.

While positioning the D65 Drill breakout table on the hydraulic press, the table slipped between the two support beams of the press. The Miner's gloved left ring finger was caught between the table and the press support causing a fracture and laceration that required 4 stitches.

On 1/11 Miner maneuvered a leaf spring set onto a dolly in the shop. On 1/12 the Miner experienced pain in the groin area and went to the ER following work. An X-ray was taken and EE was diagnosed with a strain. After several weeks the Miner was still experiencing pain and had it reevaluated. On 3/13 it was diagnosed as a hernia and received corrective surgery on 03/23.

While servicing the S190 Portable Screen, the Miner removed the cap of the radiator to check the coolant level. The Miner received burns to hand when the coolant ran into Miner's glove.

While servicing the Terex Vibratory Screen, the miner was positioning self from one side of the screen to the other when the screens eccentrics rolled causing EE to pinch finger.

While shoveling near the crusher station at the preparation plant, the Miner was struck on the left shoulder by an unknown object or debris. The Miner was seen by a doctor and was diagnosed with a bruise on the shoulder.

While the Miner was removing a mine timber from the feeder area, EE strained EE's back.

While pressure washing the interior of the preparation plant, the miner pulled the hose to gain more slack. The hose caught an a loose piece of metal on the floor above the miner causing it to fall and cut the miner's left arm.

While working on the feed hopper at the plant, the Miner scraped EE's left arm against the edge of a sharp piece of metal and cut EE's lower arm. EE went to the hospital where EE received two stitches.

While mounting a new motor on a frame in the cleaning plant, employee was attempting to insert the bolts from the bottom of the frame while another employee held the motor. When the motor was lowered, the employee inadvertently had tip of thumb on top of the frame were it was pinched.

While repairing the support brackets for the utility body underneath a service truck, the employee attempted to remove the bolts from the bracket. Dirt and rust fell from underneath the truck and struck the employee on the forehead and fell underneath safety glasses. Dirt or rust went into the employee's eye.

EE assisting in removing a cable from a 4600 drag line. The cable came loose from the drum and struck EE on the bridge of the nose. The nose area was swollen and bleeding. EE was taken to the local emergency room for evaluation and treatment.

Plant operator was replacing a roller on overhead conveyor. The roller brackets were not correct for the roller being replaced and the roller came out of the brackets striking the operator on the bridge of EE's nose. At present, we believe that the operator sustained a broken nose. EE is in the process of being examined by an ENT specialist.

Welder was making a repair with some pre-cut pieces of steel. While lifting one of the pieces of steel into place the welder complained he felt a strain on his right side. His foreman sent him to the ER for evaluation.

Employee went to foreman on 8/23 and stated he had hurt his back on Sat. 8/20 lifting a 5g pail of oil while doing a service. The employee left on the morning of 8/23 after telling the foreman of the incident. The accident was not reported until three days after the alleged incident. Employee states there were no witnesses and not sure how it happened

While lifting bags of magnetite from a forklift to a catwalk in the plant the employee strained his abdomen. He went to the Hazleton General Hosp. to get examined at the end of the shift.

While placing a sheet of steel to be welded as a bed liner into the R90 rock truck, the employee felt a strain in the groin area. After several days a bulge developed. Upon examination by his family doctor it was found that he tore his groin muscle.

While unloading a down the hole drill hammer and drill bit form a pickup truck, the employee caught his thumb between the the two. This resulted in a cut on his left hand that required stitches

After A vigorous morning of activity of up and down movement, employee reported a feeling of discomfort in his feet. Employee later reported that he saw his family doctor, but felt relief when he "cracked" his back. This prompted the employee to seek treatments from his chiropractor.

While reeling in the hose of the fuel truck, a kink in the hose uncoiled and the nozzle struck the employee's finger.

WHILE INSPECTING THE GOULDS 3 X 3 PUMP FOR CONDITION, THE EMPLOYUEE STRAINED HIS BACK WHEN HE TRIED TO ROLL THE PUMP ON ITS SIDE.

When replacing an air conditioning line on the H-241 backhoe, dirt fell from a higher level and struck employee in the head. The dirt went behind his safety glasses and into his eye. Later the eye became infected and he needed medical attention.

wHILE PULLING ON THE LEVER TO ENGAGE THE POWER TAKE OFF PTO, THE EE SPRAINED HIS WRIST.

WHILE LIFTING THE SHROUDS FOR A H-185 BACKHOE BUCKET, THE EMPLOYEE FELT A PULL / TIGHTNESS IN HIS CHEST.

WHILE REMOVING THE REAR WHEEL SEAL FROM THE TRUCK AXLE, THE WARE SHARPENED METAL SEAL UN EXPECTANTLY SLID AND FELL HITTING THE LEFT HAND THUMB.

WHILE CHANGING A HYDRAULIC OIL LINE. EMPLOYEE REACHED UNDER THE PIECE OF HEAVY EQUIPMENT TO RETRIEVE A DROPPED TOOL. OIL RUNNING FROM THE REMOVED HOSE DRIPPED INTO THE EYE.

EMPLOYEE WAS CARRYING A PIPE UP STAIRS AND HE SPRAINED HIS BACK.

EE WAS CLIMBING STEPS TO FRONT-END LOADER AND FELT A "POP" IN HIS KNEE.
