Mostrando los 6EE was attaching 6" lay flat suction/discharge line to fill water truck approx. 6' in length with cam lock fitting.
Driver had loaded with ash and was slightly overweight so he had opened the tailgate to lighten the load. A chunk of ash impeded the gate closing fully so he tried to pull the lump out of the way. As the lump dislodged the gate slammed shut and got the 5th finger (pinkie) of his right hand caught between the gate and the bed.
EE AND HIS HELPER WERE CARRYING A PIECE OF PIPE. INJURED STOPPED TO CLEAN OUT ONE END OF THE PIPE WHEN HELPER STOPPED AND FELL. THIS CAUSED THE PIPE TO BOUNCE AND STRIKE EE IN THE CHIN. THE LA CERATION REQUIRED 2 STITCHES.
EE WAS CLEANING SECTION OF FUEL PREP SILT HOPPERFOR MAINTENANCE. USED GLOVED HAND TO WIPE SILT AWAY SO AMOUNT & TYPE REPAIRS NEEDED COULD BE DETERMINED IN PROCESS, CUT HAND. APPROX 1 1/2" IN L ENGTH. REQUIRED STITCHES.
