EE was hanging curtain when EE felt a strain in EE's back while reaching up.

While straightening a 6' 3/4" glue bolt, the operator felt a sharp pain in their right shoulder.

EE was replacing the ripper head jack on the continuous miner, the excessive lifting and pushing resulted in a burning sensation in EE's left shoulder. EE reported that EE's shoulder was burning. EE continued working until 9/27/19 which time EE went to a doctor. EE performed regular job duties until shoulder surgery on 11/21/19.

EE was assisting to lift a roof bolter drill head approx. 24 inches off the floor into a personnel carrier so that it could be transported to the surface when EE experienced extreme pain in EE's lower back. The drill head weighed approx. 140 lbs. The injury occurred at the end of the section supply track way.

The employee was physically pushing ½ of a 16 foot airlock door to open it when severe pain occurred in their lower back area. The metal airlock doors are located at 12 crosscuts, inby the no. 5 return air portal and separate the main intake and return air courses. The doors are 16 ft. wide. A lot of air pressure is present at this location.

EE was attempting to remove some draw rock off top of cont. miner. EE grabbed a piece of rock measuring 30"L x 24"W x 5" thick & pulled it toward body. When rock reached edge of machine EE lost grip, rock dropped down & caught front part of ring finger on rt hand against edge of machine severing most of the flesh toward the bone. EE was working in no. 6 entry w/2 co-workers.

The employee was changing belt conveyor top rollers which requires heavy lifting and pulling, while in this process ee suffered severe lower back pain

Employee bumped his right elbow on the corner of the center control panel while sitting down on the Johnson Ind. 4 wheeler. Resulting in swelling and bruising to the elbow.

EE had installed a 54" steel channel on the 1st bolt in the row he was installing. Once the bolt was installed the steel channel was not properly positioned, he grabbed the steel channel w/R hand to pull the channel toward him, his R ring finger became lodged between channel and roof resulting in traumatic damage to the end of ring finger. EE said machine was no part of injury.

While attempting to put a cover back on the roof bolter, he dropped the cover and it hit on top of his foot behind the steel toe.

While removing metal guard from headdrive, employee mashed his pinky finger between guard and headdrive resulting in 2 cuts requiring 14 stitches.

EE WAS MOVING THE MINER CABLE IN #6 ENTRY. WHEN HE GRABBED THE CABLE TO PULL IT TO THE RIB, HE FELT SEVERE PAIN IN LOWER BACK.

EMPLOYEE CALLED ANOTHER EMPLOYEE ON APRIL 9,'97 CLAIMING TO HAVE PICKED UP A ROCK INJURING HIS BACK. HE WORKED THE ENTIRE SHIFT AND DID NOT REPORT ACCIDENT TO BOSS OR MINE FOREMAN.

THE VICTIM WAS LIFTING MINER CABLE TO BE HUNG WHEN HE FELT PAIN IN HIS LOWER ABDOMEN. HE SAID OCCURRED ABOUT 1 WEEK BEFORE CHRISTMAS BUT DID NOT REPORT INJURY TO BOSS UNTIL 12-26-96 AND CONTIN UED TO WORK. EMPLOYEE DID NOT WORK 12-30-96 AND REPORTED THE INJURY TO ME ON 12-31-96 2 WEEKS AFTER THE INJURY OCCURRED.

EE WAS INSTALLING SHUTTLE CAR FLIGHTS WHEN HE PICKED ON UP HE STRAINED HIS BACK.

EE WAS CHARGING A BEARING ON THE TAILROLLER. HE HAD THE ROLLER TIED OFF WITH A CHAIN. AS HE WAS PUTTING THE NOW FRING IN PLACE THE CHAIN SLIPPED CATCHING HIS FINGER BETWEEN THE BEARING AND THE TAILPIECE THE RESULT INJURY IN 1 BROKE FINGER AND 1 BADLY BRUISED.

HE WAS TRANSFERRING A SPARE SCOOP TIRE FROM RAILRUNNER TO SCOOP BUCKET, THE TIRE FELL OUT OF R.R. AND BALANCE OVER TO HIS LEG. HIS LEG WAS BRUISED.

LOADING ROCK DUST [50 LB BAG] TWISTED BACK.

HE WAS HANG MINER CABLE AND HURT HIS BACK.

HE WAS LIFTING A 50 LB. BAG OF ROCK DUST.

EMPLOYEE WAS THROWING A BAG OF ROCK DUST FROM TRACK DUSTER TO LOADER BUCKET, TWISTED AS HE THREW ROCK DUST.

EMPLOYEE WAS HANGING A FEEDER CABLE STRAINED BACK

THE 3 BELT WAS BROKE AND WE WERE PULLING IT BACK TOGETHER WITH CHAIN, WHEN IT BROKE AND HITTING HIM IN THE BACK.
