Employee was in the underground shop area picking up rockdust bags to throw in dumpster when EE struck right ear on a sprinkler head on the fire suppression system. This resulted in a laceration to the right ear causing sutures.

Employee was removing panel rubrail from a shuttle car. When the rubrail was being placed on mine floor employee injured left thumb.

Employee was removing curtain from shuttle car reel and felt pain in lower back.

Employee was in the process of attaching bull hose to the unit water tree, the water valve was inadvertently turned on the 1 1/2 inch water valve causing the bull hose to strike the employee in the mouth. The employee received a laceration to the upper lip. The employee received 8 sutures to the injury.

Miner was pulling up 995 continuous miner cable slack with A nylon strap in the deck of A shuttle car. The miner put hammer handle through the strap, the miner had hand on the hammer head when the cable became tight the miners hand was caught between the hammer and the car deck. The miner received A fracture to the left index knuckle.

While helping plant mechanic install belt on pump, employee's left middle finger caught between the belt and pulley resulting in a slight fracture of the finger.

Miner was in process of installing a man door in a recently erected permanent stopping when a block became dislodged from the top of the stopping, coming in contact with his right hand. This causing a fracture to the right thumb.

While removing transmission from D9L fastened transmission to the boom of the mechanic truck. After attempting to loosen the transmission with the boom, employee stepped forward to see why transmission would not come loose. The transmission "popped off" striking the employee in the chest and knocking him against the side of the mechanic truck, resulting in six fractured ribs.

Miner was removing wire rope from shuttle car slide anchor when wire rope unwound from clevis striking miner in the face causing a fracture of his nose.

Driving up road on the way back from 4th 54 belt drive Didn't see roof bolt and made contact causing a cut on the top of his head resulting in 5 staples.

Miner was hanging high voltage cable when cable slipped from his hand striking him in the mouth, causing a broken tooth.

Miner was spreading legs of step ladder when ladder began to fall, miner attempted to restrain ladder and reports lower back strain/sprain. This 7000-1 form is being completed to terminate citation # 8513327. Miner reports the accident occurred on 3-22-13,however did not report accident until 5-22-13. Therefore there were 0 days lost or restricted duty.

Employee removed the bottom steel from the bolter pot to check the suction. As he placed his left hand over the pot, the remaining steels fell from the bolthole he was drilling and struck his left hand, fracturing his middle finger.

Employee was loading rock dust onto a coal scoop. He picked up a bag, turned to put the bag on the scoop and his feet tangled up twisting his lower back.

Pulled cable off the back of the ride and struck my left elbow against a car cable hanger plate and cut it open, requiring seven sutures.

Breaking a band on a bundle of roof bolts and the band flew up and cut right forearm, requiring 5 sutures.

Loading opposite operators side of roof bolter. Reached across supply trailer for a box of glue, as he turned toward the bolter his left forearm contacted a band on a bundle of pins causing a laceration that required three sutures.

Employee was working on the 2nd Southwest tailpiece when the belt framing slipped and struck left leg. The resulting laceration required 7 sutures on the inside of the cut and 11 on the outside for a total of 18 sutures.

Employee was hanging cables when he turned and caught his right ear on a piece of hog panel hanging from the roof. Laceration required 9 sutures.

I was hanging miner cable in #4 entry on #2 unit. I lifted the cable up and felt a sharp stabbing pain in my stomach by my belly button. I didn't think anything about it at first, but after I got home I couldn't move without severe pain in my stomach.

Shuttle Car operator attempting to retrieve sideboard from shuttle car that had fallen into car when he felt a burning sensation in his abdomen, resulting in a hernia.

The employee picked up a belt roller and when he turned to install it he fell to the ground and was having pain in his lower back and left hip. The employee was not taken off of work by a doctor until 10/10/2011.

While loading supplies into a battery scoop bucket, a metal band hit the employee on the right hand causing a laceration. The employee received 4 sutures.

While changing a cat pad on the continuous miner the employee smashed his thumb between the pan and a board.

Lifting wide beam. Mashed hand between beam and 630 mucker, causing crushing injury to right little finger.

Injured was carrying buckets of stopping plaster and felt pain in his back.

While unloading some steel pipes, one of the pipes shifted unexpectedly and pinched the employee's left wrist between two of the pipes. The employee received a hairline fracture.

While trying to open a big airlock door, the employee opened a smaller inner door used to releive pressure on the big door and he slipped and when he tried to cath himself, the small door slammed on his fingers. The employee received sutures.

While trying to open a big airlock door, the employee opended a smaller inner door to releive pressure off of the big door, when someone yelled at him and when he looked back the small door slammed on his fingers. Employee received sutures.

While walking through a curtain, a corner of a piece of wire mesh attached to the mine roof was turned down and the employee ran into it striking him on the left side of his safety glasses and slid to the corner of his left eye and scraped his face toward his left ear. The employee received 12 sutures.

Employee connected two drill steels together and was pushing them up with the drill pot, when the steel popped out and struck him on the right wrist and hand.

As he was roof bolting, injured was holding a bolt board straight while pushing a bolt up and caught his left index and middle fingers between the board and the roof amputating the tips of both.

While moving a Nitrogen bottle the bottle slipped from the employee's grip and slid down his left leg. A sharp part of the cap hit the emplooyee's leg causing a large laceration. The employee received 26 sutures.

While trying to take the cap off of an oxygen bottle the cap came loose suddenly and struck the employee in the mouth. The employee suffered a chipped tooth.

While throwing the continuous miner cable over out of the travelway the employee felt a sharp pain in his lower back.

Employee was loading supplies into scoop bucket, when he cut his right index finger on a bundle of pin plates.

While unloading a 50 pound bag of rockdust the employee felt pressure on the right side his abdomen which turned out to be a hernia. The employee had surgery to correct the hernia on 8-20-10. This did not become a lost time accident until 8-20-10.

While picking up a 50lb. bag of rockdust off of a 4 passenger mantrip the employee twisted around and felt his knee pop.

While trying to load a mandoor into a battery scoop bucket the employee felt a sharp pain in his right shoulder.

Employee was holding up some screen wire on the rib while trying to install a roof bolt when he felt a sharp pain in his abdomen. The employee said the pain worsened as the day progressed. Later that night he went to the emergency room and had to have surgery for a hernia.

While handling a steel pipe the employee slipped and mashed his middle finger on his left hand against the steel pipe and a steel beam. The employee ripped his finger nail off and suffered a fractured finger tip.

The employee was on the rock slopes, at the #3 entry, setting steel. He unhooked a chain from around an I-beam and the I-beam flipped over. His right index finger was caught between the I-beam and the chain. The employee received 9 sutures on his right index finger.

While employee was pulling on miner cable he strained his back.

Employee was re-claiming an old overcast when one of the support beams fell, striking him on his left ankle, causing a fracture.

Employee was pulling a roof bolter cable over toward the mine rib and strained his right shoulder.

Employee was loading a Roof Bolter with roof bolts and strained his groin.

Employee was picking up a metal trailer hitch (tongue) off of the ground in the supply yard and strained his back.

Employee was dragging loops of cable of a continuous miner around the corner of a coal rib and strained his back.

The employee had wrapped a thin cable around the brake handle on a piece of equipment and was going to pull the cable but, while he was trying to hang onto the cable it caught on something and pulled tight and caught his left hand and broke his middle finger. The nature and location of the injury caused the employee's finger to be amputated by the doctor.

Employee had been under a doctors care on his right shoulder since May 2007 for repetitive shoveling. He re-aggrevated the shoulder on 12/4/2007 while shoveling on a belt tail roller. He went off work for shoulder surgery on 2/13/2008.
