Employee was helping push a flock tote across the floor when his foot got caught between the flock tote being pushed and an adjacent pipe coming through the floor, compressing his foot from the side. Employee was wearing metatarsal boots.
Injured and co-worker bent over and lifted the sheave about waist high and put on pump shaft. Employee felt pain in lower back. He continued to work until September 30, 2012.
Pulling torch hose and bumped right elbow on discharge chute of #5 Froth Cell at corner of channel bracing for actuator.(Hit corner of 6" channel bracing.)
ON APRIL 9, 2012, INJURED CLAIMED TO HAVE STRAINED HIS LEFT SHOULDER WHILE PULLING ON VALVE HANDLE ON PRIMARY COARSE SUMP LOCATED IN BASEMENT OF PREP. PLANT. INJURED DID NOT MISS WORK, BUT CO NTINUED TO HAVE PROBLEMS WITH SHOULDER. INJURED HAD SURGERY TO REPAIR ROTATOR CUFF, AND BEGAN MISSING WORK ON JUNE 10, 2013. THIS INJURY WAS NOT REPORTABLE UNTIL THIS DATE-(JUNE 10, 2013).
EE was cutting a 10 inch pipe that was propped up on another pipe. The pipe he was cutting rolled over on his right foot and bruised it.
Injured stated that he tried to loosen radiator cap on 100 ton locomotive, when hot steam and liquid contacted right wrist, causing burn to wrist area. Injured was in process of checking engine coolant level because engine was overheating.
Injured claims to have been lifting a piece of metal 3/8" x 12" x 36" long during clean-up when he felt pain in lower back area. Injured threw piece of metal in end loader bucket and started driving 773 rock truck for approximately 45 min. when pain got worse and he called for help. Supervisor was notified and ambulance called.
Injured and one other person were carrying 1/4 x 4' x 45" piece of metal up small flight of stairs to belt walkway, when metal slipped causing weight to be shifted to injured EE, putting him into an akward positioin.
Injured was dropping steel rope (3/8" x 120 ft long) which was run through a shive block to the ground from top of load-out bin when slack slipped in rope, catching finger against handrail causing injury.
INJURED AND CO-WORKER WERE CARRYING A PIECE OF PIPE, WHICH WAS "L"-SHAPED, AND APPROXIMATELY 6" X 48" IN LENGTH (WEIGHING APPROXIMATELY 75 POUNDS), UP THE STAIR-WAY BETWEEN 6 AND 6 1/2 FLOOR IN PREP PLANT WHEN CO-WORKER'S END OF PIPE SLIPPED CAUSING INJURED TO FEEL PAIN IN HOW LOWER BACK.
WHILE ASSISTING / SUPERVISING BELT CREW INJURED BENT OVER TO HELP AND HIS HAT FELL OFF. HE WENT TO GROUND LEVEL TO RETRIEVE HAT AND WORKER DROPPED BELT CLAMP STRIKING EE ON TOP OF THE HEAD WHI CH REQUIRED 8 STITCHES.
WHILE LIFTING COAL SAMPLES, EE TURNED AROUND AND TWISTED, INJURING NECK/UPPER BACK. DID NOT WORK ON 8-26-02.
CUTTING BOTTOM OF WASHER OUT, BENT OVER TO PICK UP PIECE OF METAL. LOWER BACK/HURT LOWER BACK TWISTED STARTED MISSING WORK 3-11-02.
REBUILDING PUMP AND DROPPED WEAR PLATE ONLEFT HAND. CEASED WORK 2-25-02.
ACCIDENTALLY PLACED FINGER BETWEEN FROTH CELL PADDLE AND EURETHANE BLOCK AFTER PIN CAME OUT OF ONE END OF PADDLE.
TWO EMPLOYEES WERE LIFTING A 1/4" X 6" X 6' BEAMABOUT 6' OFF THE GROUND TO WELD INTO PLACE. ONE COMPLAINED THAT NECK WAS HURTING AFTER THAT. HE MISSED WORK 8/10, RETURNED 8/13 AND CEASED WORK ON 8/15/01 AND HAS NOT RETURNED TO WORK TO DATE.EMPLOYER QUESTIONS THE VALIDITY OF THE ACCIDENT.
INJURED WAS THROWING SCRAP METAL & PARTS OUT OF HOIST DOOR BY HANDON 5TH FLOOR OF PREP PLANT WHEN HE GOT A PIECE OF METAL IN FINGER, WHICH LATER BECAME INFECTED. WENT TO DR. ON 4-27, NOTIFIED FOREMAN OF INJURY ON 5-8.
INJURED CLAIMS TO HAVE BEEN PULLING WATER HOSE ACROSS 3RD FLOOR OF PHASE I BUILDING. HOSE HUNG UP CAUSING INJURED TO BE JERKED SIDEWAYS & TWISTING HIS BACK. BEGAN MISSING WORK ON 9/13/00 AFTER DOCTOR VISIT.
INJURED WAS REMOVING A PIECE OF BOLT FROM COAL CHUTE. WHEN HE FELT PAIN IN LOWER BACK. INJURED WAS LEANING OVER IN CHUTE AND JERKED ON BELT.
INSTALLING SIDE ON DRIP PAN UNDER SILO FEED BELT WHEN PIECE OF METAL SLIPPED AND CUT RIGHT KNEE.
INJURED AND OTHER EMPLOYEES WERE CARRYING METAL SHELVING TO PUT IN TEMPORARY STORAGE AREA WHEN A PIECE OF SHELF SLIPPED OUT OF HAND CAUSING TOP OF HAND TO BE LACERATED BY SHELF BRACING.
