Pipe was stopped up and the employees arm was in the pipe trying to clean to remove gob. When the gob came lose the employees arm was in the way causing the gob to break his right wrist.
The employee was replacing the hoist cylinder on a D8T Dozer. As he had the cylinder hoisted in the air, a bearing cap came off and struck him in the face and mouth. The injury required stitches in his upper lip, no other medical treatment needed.
EE was placing a 1/2 x 29 1/2 x 48" piece of plate in the fuel bin when plate twisted and caught left hand ring finger between plate and the plate on platform.
PULLING 6" LINE UP TO WELD WITH CHAIN FALLS AND MONORAIL SLIPPED BACK, CAUSING 6" PIPE TO FALL ON FOOT.
EE WAS WORKING ON A LIME/WATER MIXING TANK. HE STEPPED INTO A HOLE THAT CONTAINED WATER AND THELIME MIXTURE. HIS FOOT WAS COMPLETELY SUBMERGED FOR ABOUT 5 SECONDS. AFTER ABOUT 5 MINUTES HE NOTICED HIS FOOT FELT LIKE IT WAS BURNING. HE REMOVED HIS BOOT AND FOUND HIS FOOT AND ANKLE HAD MULTIPLE BURNS ON THEM. HE THEN RETURNED TO OUR SHOP AND WAS TAKEN TO THE HOSPITAL FOR TREATMENT
EE STATES HE EXPERIENCED PAIN IN HIS BACK AS HE WAS TRYING TO OPEN THE BY PASS FLOP GATE FOR THE DRYER.
EE WAS MOVING 1" HANDRAIL RODS INTO PLAT TO BE HOISTED UP.
EE WAS LIFTING 2 BAR WITH HALF THE WEIGHT RESTING ON A CHAIN. THE CHAIN BROKE AND BAR FELL JERKING HIM TO THE GROUND JERKING HIS NECK.
EE RAISED UP FROM JERKING ON SOME CHUTE WORK HIT HANDRAIL JAMMING HIS NECK.
INJURED STATED HE PULLED MUSCLE IN HIS CHEST WHILE RIGGING MACHINE.
INJURED STATED THAT HE WAS PREPING A PEICE OF STEEL BEAM TO LIFT AND IT FELL OVER STRIKING HIS RIGHT LEG.
