IE caught their left arm between a set of equipment doors while trying to enter the intake aircourse causing a fracture to the ulnar bone between their wrist and elbow.
Employee was folding up hog panels to put in trash dumpster when EE's hand slipped allowing hog panel to strike said employees eye.
Employees were unloading metal undercast panels from a 5th wheel trailer into the bed of a coal hauler. The top two panels slid partially off of the stack, a panel got stuck, EE's hand slipped across the end of the metal panel, causing a laceration to EE's left hand. IE was transported out of the mine and taken to a local medical facility where EE received five sutures.
IE stated that EE was entering into the rear seat of a mantrip located behind the front passenger seat with EE's hand on the upright canopy support post. EE stated that another employee closed the door to prepare for departure smashing EE's right ring finger between the door and post. The IE was transported to the local ER for treatment where EE received four sutures in the finger
Employee was preparing a piece of plate for installation; Plate was clamped down on saw horses and when clamps were released the plate slide off the saw horses which came down on employee's lower right leg and made a laceration.
IE was loading a spool of cable into a scoop bucket and felt a strain in their right shoulder. IE continued to work doing therapy on the shoulder. On 12/9/2022 the employee had an arthroscopic procedure completed to clean up a micro tear and bone spurs and began missing work.
An employee from Contractor BB0 was in the process of removing the outer jacket from an electrical cable at Peabody's Columbia Rebuild Center. The next day the employee presented with pain in left wrist...contractor was advised to see a doctor. Was initially given steroids to reduce pain and swelling and no lost time injury occurred.
IE was removing jack stands from a mantrip using right leg to slide the jack stand out from underneath the mantrip. When doing this, IE stated that knee popped with no immediate pain. Over time IE states that pain has progressively gotten worse causing IE to not be able to bear full weight on right leg. IE was taken off work by a medical doctor on 11/3/2021
IE stated they were shoveling coal spillage on the belt conveyor when lower back started aching. IE began missing work on 07/09/2021.
IE stated that while trying to unplug the charging cable from the battery for # 14 scoop, the charging cable was stretched over the battery lids. As the IE pulled on the cable to get some slack to unplug the charging cable, the cable pulled on the lid enough to slam closed on IE's left pinky finger as EE was disconnecting the charging cable plug, causing a laceration and fracture
IE stated was operating a coal hauler. IE stated struck wrist on a sharp piece of wire, while removing their backpack from the equipment. When doing this, IE's wrist came in contact with the wire causing a laceration approximately 1.25" on the inside of IE's left wrist. On Friday April 23, 2021 IE was seen by local doctor and taken off work for sutures that was added.
#4 Roof Bolter was moving to 7R. The IE was helping move the bolter by hanging cable. One employee was tying the wire while the IE was holding the cable. IE stated the cable got tight and broke loose from the tie wire. IE stated the cable snapped IE's arm downward causing a sharp pain in shoulder.
IE stated was pushing a rock, sized approximately 4' in length by 1.5' in thickness, into the miner conveyor. The rock started to tip up into the conveyor, when the IE's hand was pinched between the edge of the rock and to top of the mine roof, causing a laceration to the inside part of IE's left hand index finger.
IE was plastering an intake stopping wall. IE stated IE splashed the plaster into eye.
IE was installing a bent 6' bolt into the previously drilled hole. EE stated as they were pushing the bolt into the hole from the ground, they felt a sharp pain and popping sound in lower back. EE stated they finished the bolting the entry and moved to entry #9. EE continued to bolt 3-4 rows before coming outside to go the medical facility to be checked out.
IE stated that EE attempted to exit the mantrip, IE opened the door and stepped out with EE's left leg. IE then grabbed the top of the door frame with both hands to pull self out and up. IE's right leg was bent as EE was standing up. When EE stood up, EE felt a sharp pain in EE's right knee.
IE stated was outby the unit getting some metal straps for the bolter. IE grabbed 3 of them through the holes in the straps. As IE pulled on them, one of them got caught on something causing EE's hand to get caught between the strap holes causing a laceration to the right hand pinky. IE was brought out and for treatment and received sutures to close the wound.
Employee was throwing cable over and when the employee raised back to the standing position hit head on a brow.
Employee was placing hog panel on the ATRS when EE over extended EE's back muscles
On 11/8/2019 an accident report was filled out stating the IE shoulder has been hurting for the past month. After work comp/doctor visits. The IE believes the incident resulted from hanging a cable on 10/10/2019. It wasn't until 12/09/2019 the worker was taken off work pending an mri.
EE was hanging cable in entry 5 @ 27+50 EE felt burning and a small pop in right shoulder.
Employee was reclaiming belt structure and was stacking belt rails on the back of a mini track. While attempting to place a rail the previously stacked rails moved and pinched the left pinky finger
IE was sealing a return stopping. IE had a bucket of sealant slip out of IE's hand and land on the mine floor. When the bucket hit the floor, a watery sealant mixture went into IE's eye.
IE stated on 8/5/19 that IE was lifting belt structure on or around July 8th, 2019 and felt pain in the groin area that radiated up to EE's left hip. IE was taken to medical facility to be evaluated and was taken off work on 8/7/19.
Employee was in the process of installing top belt framing above their head. The piece of framing got caught on a piece of fire deluge pipe while trying to get it into position. When freeing it up the piece of framing slipped off of the belt rail and suddenly fell, which jerked the employee's arm downward causing an injury to the right shoulder
IE stated IE was roof bolting and had finished drilling with the first steel. EE lowered the drill pot down, inserted the second steel into the 1st steel. When EE raised the drill pot up to insert the second steel into the drill chuck the steel sprung causing the steel to strike EE on the hand causing a laceration between EE's index finger and thumb. IE received sutures.
Employee was building a stopping after a belt move. A block slipped and caught EE's finger between block and rib.(Employee had gloves on.)
IE was dragging 2 buckets through curtain when ee twisted to right and felt sharp pain in center upper abdomen.
IE was pushing an 8' bolt in the top in low coal and felt a popping and burning in right shoulder.
Injured employee sustained a scratched cornea and was prescribed antibiotic eye drops as a result of being hit in the face with a blast of water and debris. The employees were repairing a water leak on a 2" Victaulic coupler on a battery barn deluge system. The system pressure was isolated but was not properly bled off.
IE stated that ee was trying to load a starter box for a belt drive into a scoop bucket. IE wrapped a strap around the starter box and pulled on it twice. When the starter box didn't move, IE attached a strap and used the scoop stinger to pull it into the bucket. IE reported incident on 2-4-19, taken to medical facility where is was taken off work.
IE and 2 other co-workers were placing a hog panel against the rib. A sharp edge of the hog panel stuck into the IE finger. On 12/9/18 the IE went to the Dr. to see if there was any metal in finger. Upon examination the x-ray showed a sliver of metal in left pinky finger. IE continued to work, referred to a specialist on 12/14/18 to have the sliver of metal removed.
IE was in the process of manually resetting a breaker on a battery scoop. A protective cover had to be removed to gain access to the breaker. After it was reset the IE was re-installing the cover, when it slipped and smashed the end of the middle finger
IE was loading bolter with 3' glue when IE felt right shoulder pop.
IE was loading roof mats three at a time into the bed of a coal hauler. As IE was sliding the roof mats into the bed, the bottom roof mat dropped down catching the lip of the false bottom. This caused the top two roof mats to slide forward catching IE's index finger in between the top two roof mats and the bottom roof mat. IE received an avulsion that took 11 sutures to close.
While removing driveline in a CAT 992 end-loader, the driveline fell striking employee in the right leg.
IE stated they went to help seal an intake brattice. IE was sealing near the top of the brattice and turned from getting sealant to place on the wall when a piece of tie wire hanging from a plate went in the side of safety glasses and poked eye.
I.E. was building a crib on Unit #1 in Entry #12 at Spad Location 11+20. While lifting a 3' crib into place approximately 6' above the mine floor, I.E. felt a "POP" in the left shoulder. I.E. worked the remainder of the 6.25.2018 shift and the following shift, 6.26.2018. On 6.27.2018, I.E. was seen by a physician and diagnosed with a Torn Rotator Cuff
IE was lifting cable, and felt a sharp pain in the back. IE continued to work, but pain did not go away. IE was transported out of the mine and taken to local physician for further evaluation. IE received a Full Return to Work with No Restrictions and worked for 10 Days. IE received a 2nd opinion on 5/4/2018, and was taken off work.
IE was carrying the hog panel from the hog panel rack on the roof bolter to the ATRS for the next row when the hog panel they were carrying got caught on the previously installed hog panel. After a MRI and a follow up Dr's visit on 4/25/18, the IE was diagnosed with a rotator cuff tear which required surgery on 4/26/18.
A welded lifting eye broke off while picking a plate up, struck forehead causing laceration requiring stitches
After finishing the shift on 1/8/2018 a continuous miner operator said IE's back was sore. IE said there was not a specific incident but related the soreness to moving continuous miner cable throughout the shift. IE said the soreness escalated after the shift. IE has been diagnosed with a strained ligament in IE's lower back and began missing work.
While cleaning a pan on a screen at the plant, the employee struck hand on the bottom of screen
IE was carrying an open bucket of sealant preparing to plaster a stopping. When ee set the bucket down liquid that was pooled on top of the sealant splashed up and contacted the employee's left eye. The IE was wearing prescription safety glasses with side shields. Treatment included prescription eye drops.
I.E. was changing bits on 239CM on the Right Side of Unit #2. I.E. and another employee went to spin the head of the Continuous Miner. While applying downward pressure to spin the head, I.E. felt a sharp pain across the back of the shoulders.
Employee was moving solid 6" block to build a wall. Employee received a block from another worker and turned with the load bent over and felt a sharp pain in lower back.
Employee was dumping concrete bags onto grate on a gunite pump when another employee tossed a concrete bag on employee's hand smashing it between the grate and the bag. Employee received 6 stitches.
IE said while hanging miner cable with a co-worker, the tie wire broke so IE reached to catch the cable with left hand. IE continued to work but noticed that hand/arm felt different. Note: This employee had a previous injury to the left hand/arm (tear to distal bicep) that required surgery to repair. Surgical repair was performed again following this re-injury.
IE was adjusting the forks on a minitrac. IE lifted on the fork to adjust it. Lifting on the fork rotates the keeper stock located on top of the fork. As the IE lifted the fork they did not realize that their little finger was in a pinch point between the metal stock keeper and the frame. IE received 9 sutures.
IE stated that while throwing over miner cable felt a sharp pain in lower left of back. Having sharp pain radiating down left leg to knee. IE was seen by Dr. on 6/20/17 and taken off work at that time.
