Employee was loading a training roller into the back of a mantrip. When roller got stuck IE lifted roller heard and felt a pop in their right arm
IE was installing a roof bolt. While holding the bolt in the drilled hole, the bolt inadvertently moved, bending the bolt causing EE's hand to get caught between the drill chuck and the drill guide. This caused a laceration that required sutures.
Employee's left heel was caught between the relay bar clevis pin and the headgate debris plate, causing an avulsion to the heel requiring surgery.
Employee was unthreading a water hose from the wet dust truck while the hose was stretched. When the hose was undone the water hose sprung back striking the employee in the cheekbone causing a contusion just under the right eye. After further evaluation from an ophthalmology specialist, IE was diagnosed with a closed fracture of the right orbital floor on 7/9/2024.
IE was moving steel pipe on a metal pallet that needed to be rearranged to a smaller length pipe. The stack gave way. Subsequently, the IE's hand was caught in a pinch point resulting in their left thumb being injured. The employee was wearing impact gloves at the time of the incident and was taken to the Hospital for further evaluation.
IE was working on a belt splice. As EE was pushing on the end of the belt, EE started to slip. EE grabbed the belt chain to prevent a fall. EE felt arm pop.
Crew was loading a techseal bag (4000lb) on to the teachseal platform. Two of the upper straps broke. The crew was repositioning to use the lower straps. A strap pinched the right pinky finger of the IE, causing a crushing laceration to the finger. IE received 5 stitches.
IW was assisting in installing a draw bar on the pump monorail. IW was helping hold the drawbar in place as the trolley was pushed into position. IW had hand on the monorail for balance when the trolley wheel pinched IW's finger.
IE was climbing down from belt structure during the section belt move. IE had both hands on the structure chain & stepped off the structure. The IE contacted 2nd chain & hook caught on the inner seam of pant leg. IE stepped off the structure the hook caught IE. IE transported to the ER for treatment and received sutures to close the wound.
IE was hanging cable with two drill steels. One came apart IE in mouth chipping one tooth and knocking one out.
Miner checking HG Sprocket area for rib bolt. Upper & lower doors open, upper door put back, not seated. Injured contractor put the lower door in place, reached up & grabbed the pin/anchor tab. Tab slipped off the lug pinching finger between tab and lug. Wound was cleaned & EE refused further trtmnt. On 3/26, we found that EE visited the ER after the incident & received sutures.
Employee went in to collect seal samples that needed to be tested. EE drove in to 9 East as far as ee could, then lifted the box & carried it to truck. EE didn't seek medical attention until June & was diagnosed with a bilateral inguinal hernia on 6/14/18. The mine was notified of the June 28 diagnosis as work related on July 6.
Taking tilt cylinder pin out from eye where it attaches to head frame. Took bucket off, backed scoop up, had head frame tilted forward. Pin still in the cylinder eye and one side of bracket. Scoop was shut off. Coworker exiting scoop and heard the head frame hit the ground. Coworker went to front of scoop and saw IEs foot was under the head frame. IE had 3 fx'd bones in foot.
IE was struck in the mouth with a hook from a ratchet strap, resulting in a broken tooth. The employee did not have any treatment until 3/26/17. That information wasn't received until 4/5 & it wasn't clear whether or not the treatment was for the broken tooth or cleaning required before the dentist would do other work. The cleaning was required due to poor dental hygiene.
Person was installing roof bolts. EE had put bolt in roof and was using tension gun to tighten bolt. EE put tension on the bolt and they tightened it up and in the process ee had hand on top of roof mat which tighten left hand to roof smashing fingers. EE broke one finger requiring surgery and required sutures in another.
Employees were packing a bottom frame down the ramp of an overcast when the injured employee slipped and dropped the frame on foot. Employee was wearing metatarsal boots and it was determined that the employee fractured metatarsal bones. Received medical treatment on 11/8/16 making this reportable.
Miner was performing pre-shift exam of the NM beltline. At the rock dust transfer tank break, the dust pipe came apart at the elbow. The pipe struck the miner in the forearm resulting in a fracture.
IE stated that EE was assisting 2 other employees lift a metal side board for a feeder. When placing onto a flatbed of truck, it slipped--smashing finger on bumper of truck
EE injured shoulder roof bolting. EE pulled on a roof bolt in the tray and felt pain in shoulder subsequently diagnosed as a dislocation and major tearing of the labrum, which has been diagnosed as degenerative prior to this event allowing the dislocation. EE had been prescribed PT in March. Company notified on 4/20/16 of first appt being the 1st week of April.
Employees were opening the metal door on a chain bucket, when the injured got latch unhooked EE headed to go help the other person unhook their latch. The door was on the down grade side of the cross cut and the employee was standing in front of it when it opened unexpectedly falling on EE's foot.
IE was getting out of mantrip & after he got out, turned back to reach for something. Another employee was exiting and stepped on IE hand--bending his finger back
Employees were moving cable. The cable fell from the roof and struck the injured employee in the forearm, resulting in a closed fracture of the ulna. Employee received medical treatment (a cast) on 8/18/15.
Employee was closing a manual door. The material used to seal the ventilation device would not roll over to let the door to close. The employee was kicking it and pushing the door closed when it came off the hinges and fell on him. The company found out on 9/24/15 that the employee contacted the Dr. to request physical therapy on 9/8/15, which he was prescribed.
EI was wearing safety glasses bolting when the jaws rattled shut. IE opened jaws and got hit in the face and eyes with hyd. oil. His eyes were flushed UG and on the Surface. IE's condition worsened after returning to work and he was taken to see the optometrist. He received minor corneal abrasions and swelling requiring prescription drops to treat the condition.
Employee claimed a shoulder injury on 9/8/14, did not seek medical attention 3/18/15. Company denied the claim based on time lag between incident & medical request. Claim accepted 9/17/15 by counsel for bicep tendonitis and T2 SLAP tear surgery. Company notified by counsel of acceptance on 10/23/15. Filing as an Occ Illness per definition and medical findings.
IE was washing down and noticed a leak coming from a pipe. While placing a magnet over the leak, material shot out striking him in the face. IE received first aid treatment but went home and his condition worsened. Went to the ER and was diagnosed with a corneal abrasion. He went to ophthalmologist the next day, was given prescription eye drops and patched his eye.
Injured employee was trying to separate two connected drill steels. He pulled the steel toward himself to spring the steel to steel connection. When he pulled on it, the steel to steel connection broke releasing the stored energy causing the steel he was holding to come back and strike him in his mouth causing a fractured tooth requiring dental treatment.
Employee was helping an operator load a D-Box into the bucket of a scoop. They had the D-Box pulled 1/2 to 3/4 in and were chaining D-Box to scoop for transporting. Employee held tension on a chain to keep it from falling off a hook. His left hand was on the push-plate and as the plate was pulled back his left hand was caught between plate and back of bucket.
Miner was handing crib to another employee on top of the HG drive. He was the 3rd person in the chain. When injured, he didn't get the crib raised soon enough and pinched his finger between the crib & conveyor stop. Employee was wearing gloves and holding the crib by the ends. Employee fractured his L middle finger requiring medical treatment.
Employee was trying to place an end cap on vent tube. He was face to rib and tube (do to closeness of last row of support) he placed cap on end of tube, it stayed on for a moment then fell, Employee dropped his head to avoid being struck in face but not in time it struck the end of his nose.
Hanging 24" T in face of 5mw section, Tube was not completely wired tight, tube fell hitting EE in head.
Employee states he was feeling pain in the lower groin area. There was no specific accident or injury, but since he's a material handler he felt the problem was from lifting, moving, and carrying supplies over a period of time. He was referred for an evaluation and was diagnosed with a hernia which was determined to meet WC guidelines and accepted as an occupational illness.
Employee states he is experiencing neck and shoulder pain which he relates to heavy lifting and carrying at work. Employee was evaluated under Colorado Workers Compensation guidelines for cumulative/repetitive trauma, and employee does meet the criteria for an OI claim.
Employee reported injured back while shoveling feeder.
Employee states he was cleaning the tailgate and was struck by a piece of coal on the first knuckle of his index finger on the left hand. The injury required stitches. Employee was released for his next scheduled shift.
Employee states he was moving and stacking rolls or rib mesh between two Burrell cans. A roll of mesh bumped and dislodged the can. The employees back was turned, and the can struck him on the back of the head. Employee received a cut on the back of his head which required three staples. This 7000-1 was originally opened via the MSHA website on 3/18/11.
Employee states while he was pulling himself up onto a high-voltage trailer, he felt a pop in his left knee. Employee received medical treatment and it was determined he would require knee surgery. This 7000-1 was originally filed through the MSHA website on 1/31/2011, but we are not able to access it.
Employee states he has pain in his left shoulder, elbow and wrist which he feels results from repetitive work such as putting cribs in the tailgate. EE was sent for an evaluation and it has been determined he meets the CO Workers Comp Guidelines for an OI.
Employee states he was releasing a pinch clamp on a dust pipe Y-Branch. When the clamp released it overextended his hand causing a laceration between his thumb and index finger on left hand. 7 sutures were required.
Employee was loading a hose onto a wet dust truck when the dust shifted in the hose causing the loose end to come back and strike the employee, leaving a laceration needing 4 sutures on his right ear
Employee reports right shoulder strain, which he believes is the result of repetitive motion of lifting/carrying during the normal course of his work. Employee has requested medical care and a claim has been accepted as an occupational illness.
Employee was removing a portion of a block stopping. He stopped to inspect the roof, he placed his right hand on the ledge of the opening. One of the blocks fell, landing on his right middle finger. He received a contusion/crush injury to the right middle finger. He was able to return to work for his next scheduled shift.
Employee was helping install Kennedy rails for a seal form. He was holding the free end of the rail as another contractor was striking it to embed the other end into the rib. Rib fell out pulling the free end from his hand and striking his left wrist.
EE was hauling Kennedy rails in the truck and the load shifted. EE went to move the rails when they shifted again and cut his left arm. Wound was treated at E.R. location and EE RTW the same day.
Employee strained their back while carrying a 45# roller from a storage pile to the truck.
EE was packing structure out of the 17L cutback belt entry when EE started experiencing pain in his right knee
He and another employee were dragging & pulling a mini-conveyor from the belt line to a truck, employee felt strain in groin left side. They received assistance from 4 others to finish moving conveyor and load in truck.
Employee states he was helping to take a staple out of a Longwall shield when the staple started to move, striking the employee just above the left eye. He received 13 sutures to close the wound.
Employee states he was carrying a top roller in the beltline when he set down the roller he knocked over a spreader bar causing a rock to fall striking him on the left ear. He received 10 stitches and was released to full duty that same day
Employee states while unloading structure weighing approx. 70-80 lb. he stepped off a 2-foot ledge and felt a pop in his right knee.
