Mostrando los 19Employee was using an 8# sledge hammer to operate a valve. After srtiking the valve handle employee felt pain in his right shoulder. The employee was given perscription pain meds on 11/12/08.
Employee strained his lower back while swinging 8 lb sledge to open a valve.
Employee was using a hollow drill stem to adjust a turnbuckle to make adjustment to vertical drilling rig. He threw the drill stem instead of walking it around, it came around and struck him behind the left ear. The resulting laceration required three staples to close the wound.
Employee was rodding out a drain valve. When rodding was finished, he turned and started walking away. The drain valve began spewing hot slurry and employee was sprayed on the right hand, left elbow, waist and back of neck causing second degree burns.

EE was closing a block valve. Valve was almost closed and EE gave it jerk. The wrench slipped off, hitting his index finger between and the valve. Finger was fractured and required sutures and splint.
EE was closing a discharge valve using an 8# sledge hammer. EE was swinging left and right and twisting at the wasteline for about 5 minutes continuously. EE began feeling pain in low back about 5 minutes later.
EE WAS SETTING UP THE IMPACT GUN FRAMEWORK. HE WAS USING A PRYBAR TO LIFT THE COUNTER WEIGHT, IT SLIPPED AND HIT HIM IN THE LOWER LIP, CAUSING APPROX. 2" LACERATION. SENT TO SPOHN MEMORIAL WHERE HE RECEIVED STITCHES.
EE STRUCK VALVE WITH HAMMER AND FELT SOMETHING HIT HIS FACE. EE HAS 1/8" CUT ABOVE [R] EYEBROW. BLEEDING HAS STOPPED. APPEARS THAT A PIECE OF METAL REMAINING UNDERSKIN. GUARD UNABLE TO REMOVE WITH FORCEPS. EE SENT TO NORTH BAY ER. METAL REMOVED UNDER LOCAL ANESTHESIA.
Employee was opening dump valve on #9 pair with a wrench. Wrench slipped and caught little finger between wrench and handrail.
EE STATES WAS REMOVING A HYDRAULIC HOSE, WHILE PULLING ON THE WRENCH, FELT A PULL TO LEFT GROIN AREA, BURNING SENSATION, DIAGNOSED AS A HERNIA, SURGERY PENDING.
Employee was using a 6' scaling bar and noticed a sharp pain in right shoulder.
Employee strained right shoulder while opening a valve, using a valve wrench. Employee was in a squatting position when injury occurred.
EE was tightening up some bolts when the wrench he was using slipped causing EE to fall backwards. He landed on some hydrate and caustic causing a 3rd degree burn to his left buttock. No restrictions or lost work days.
EE removing scale inside multi-deck tank. Scale fell bouncing off rake striking EE on lower lip causing laceration. Required five sutures to repair.
EE closing 8 inch gate valve, wheel wrench slipped catching left thumb between pipe and wrench. Result fracture.
EE standing under heat exchanger working. Right foot on lower slope foundation and left foot on elevated foundation, about 12" drop. 4" cold roll steel about 12" long used for counterwieght lying on upper foundation. EE accidently bumped counterwieght with hammer causing it to roll off landing on top of right foot. Resulting in fracture.
Employee using scaling gun on precipitator tank barrel. Lost control of tool allowing it to move downward.Employee's left 4th finger was pinched between gun and pipe flange.resulting in slight fracture.
