Employee was trying to take off a bolt that was stripped. Employee used a prybar to secure the bolt to prevent spinning. The bar slipped when the impact was applied and employee caught his thumb with the spinning bolt causing a laceration to his right thumb.

Employee was tightening bolts on a valve when the wrench slipped off bolt and struck employee above the right eye.

Employee was stripping wire with a razor knife. The knife slipped and he cut his left hand.

Employee was pulling electrical wire up on cable tray on conveyor. The rope they were using to pull with came undone. The cable fell down and struck him on his hand causing a hairline fracture to his right index finger.

Employee was at the B4 stacker belt preparing the end of the new belt by cleaning up the cables when the utility knife slipped resulting in a laceration above the right knee.

Employee was removing a jam nut on the "A" deck at the primary crusher, when his left hand was crushed between the wrench and the spider shell, resulting in a contusion to this left little finger.

While tightening fitting on a hydraulic hose on the 536, the adjustable wrench that the employee was using slipped causing him to hit his body under his right armpit. He felt a sharp pain.

Worker was standing on steel ring driving bolt out with 20 lb. sledgehammer. The hammer deflectd off ring and hit worker int he foot knocking him off his feet. He fell onto ring striking rib cage and fracturing 6th right rib.

Setting speader bar for fusion machine pipe saws and got hand pinched (two dingers-1 w/laceration requires sutures).

Employee was changing idler frame and was trying to knock out bolt using hammer and pin. Employee missed pin striking left index finger.

In the belt splicing process next to the R4 conveyor system, employee was stripping rubber off of cables to prepare for splice. Using razor knife to clean off rubber from cables, knife slipped off of cable and entered Rt. thigh above knee pad.

Employee was working on cell maintenance changing out the capblocks and triangle bars. He got a chipping hammer and began to chip the melted capblock off the dogbone so that the new capblock would sit flush in the dog bone channel. It was at this point that a piece of debris went into the employees right eye causing him irritation and discomfort.

Employee was in the process of driving grade stake nails for an asphalt project, when a piece of the nail head struck him in the right hand.

EE was tighting a nut on a conveyor belt section when the socket slipped causing her to fall backward. She tried to catch her fall with her left hand. When her left hand made contact with the ground she dislocated her wrist.

Employee was utilizing hammer to try and straighten hook on cathode conveyor on No. 2 machine. When he struck his ring finger with the hammer, taken to clinic

Employee was swinging a 16 lb sledge hammer to knock track shims into place when he felt pain in his right elbow.

Employee was attempting to clear a frozen 1/2 air line under pressure, when the ice broke out of the air line. It struck him in the face and a piece of ice knocked his safety glasses off and hit him in the left eye causing a laceration to his cornea.

Employee was driving out a pin with a hammer and it slipped and struck his left leg below the knee.

The employee was in the process of shutting a bonnet valve. The bar the employee was using slipped off of the valve and smashed his right ring finger against another near-by valve.

Removing hoist pedestal retainer plate. Last two bolts broke off causing the retainer plate to strike employee in the chest. Part of the prodecure was to insert two pry bars as bolts were removed. Along with the retainer plate the pry bar hit employee in the right arm causing a puncture wound in the elbow area and a cut above his right eye.

Was prying with a mill bar against the mounting ring and cyclone header while the crane was hooked to the picking eyes because the header was tilting and needed to be moved a little for removal. Bar slipped causing a laceration to the underside of his chin in the fleshy part.

Employee was stripping wire with razor knife and knife slipped and cut right thumb requiring sutures.

Employee was using a pocket knife to gage the depth of a piece of rubber. The pocket knife folded onto the little finger of his right hand resulting in a laceration.

Employee was moving a piece of grader edge with an 18" pry bar. The bar slipped, striking employee in the face resulting in a laceration to the side of his nose.

Employee was removing a digging point when he hit the side of the point with a pin punch and a piece of metal flew off and hit him in the arm.

Employee and partner were removing pillow block bearing from pulley assy. Injured employee was holding housing bolts from spinning from bottom of bearing assy. After bolts had been loosened and nut removed partner began striking housing with hammer after second strike with hammer bearing separated and pinched injured mans hand between floor and lower bearing half.

Cutting plastic pipe with utility knife and cut left index finger.

Employee was assigned to clean up from belt splice at the tail of the S-12. He was gathering rubber strips and placing them in a trash bag, the strip was entangled so he started cutting strip with belt knife and struck his left upper leg causing a 2" laceration.

While stripping a conveyor belt for installation the employee received a slight cut on his left leg above his knee when another employee who was also stripping the belt knife slipped upward grazing the other employee.

While employee was mounting a tire on a wheel, the top bead did not fully mount right. Employee grabbed a bead hammer and struck the bead once, the bead still did not fully mount so employee struck the bead again. As he hit it a second time, the head on the hammer came off the handle striking the employee above the eye.

Employee was hammering T-posts with a 16 lb hammer. The hammer glanced of the post causing his right index finger to strike the post lacerating his finger which required sutures.

Trk 112, retorque left rear inside wheel using RAD 30 torque tool. Holding RAD-30 with right hand @ trigger, left hand @ tube of tool. Caught left thumb between tube and wheel - reversed tool to pull hand out. Reported to tech, called Security, transported to hospital.

Employee was cutting wood with a saw. The blade was dull and he wrapped his left hand around the base of the saw to push better and the saw pinched his little finger requiring sutures.

Employee was changing a flat tire on a flatbed utility trailer using a Hi-Lift (handyman) jack. While lowering the trailer on the jack, the employee momentarily let go of the jack handle causing the handle to spring back, catching and smashing the employee's hand between the handle and the body of the jack (fractured bone in thumb).

The repairman was using a 16lb double jack hammer to loosen the corner shroud when the hammer made contact with the shroud a peice of metal broke off and flew up under the face shield that he was wearing and struck him in the face.

EE was washing a truck when the monitor spun as he turned on the water. He tried to hold it, jarring his back and sending a sharp pain down his back. He then complained of dull lower back pain. He was taken to the ER and removed from work. RTW 3/14/06.

Employee was rolling a pipe over with a strap wrench when the wrench slipped striking his face.

Employee was replacing snubber pin on 13 shovel dipper. After welding on the keeper he chipped away the slag from the weld and a small piece went into his right ear.

Employee was operating a chain hoist and a foreign object entered his left eye.

Employee was using a hammer to help open a sump gate on Sag 2 sump when he felt a pop in his lower back above his left hip. He continued to work but later after using the skid loader, he felt tingling in his toes. He was taken to the dr. given a prescription and taken off work. Currently on going.

Employee was installing communication jacks. Employee was holding the terminal with her left hand and, with her right hand, attempting to use a punch to cut the wire. The punch slipped and punctured the middle finger of her left hand.

Employee was using a doublejack hammer to drop a liner inside a ball mill after the bolts had been removed. When the liner dropped, the balls caused the liner to roll towards the employee and the liner struck the employee above the left knee.

Employee was prying with a bar and stepped back and felt a pull in his back.

Employee was pulling on a prybar when it slipped and struck him in the mouth, lacerating his upper lip and requiring sutures.

WHILE INSTALLING A BOLT ON THE MAN HOLE COVER TO THE C FILTER, EE'S HAND GOT CAUGHT BETWEEN THE NUT & THE PLATE THAT WAS BEING BOLTED DOWN, PINCHING THE PALM OF HIS LEFT HAND. THE EE WAS TAKEN IN FOR TREATMENT REC'D 2 STITCHES & RELEASE TO WORK.

Employee states while pulling out rods, he slid his hand up the wrench. The wrench reversed and crushed his finger.

EE WAS PULLING CABLE AT THE 22 SHOVEL WITH CABLE TONGS. THE TONGS SLIPPED OFF WHEN HE WAS PULLING AND HE FELL LANDING ON A LARGE ROCK WITH HIS LEFT UPPER LEG. IT HAD JUST FINISHED RAINING AND IT WAS WET AND MUDDY. HE FINISHED THE SHIFT, BUT WHEN HE CAME IN THE NEXT MORNING COMPLAINING OF PAIN, TAKEN TO THE ER. DOCTOR DIAGNOSED WITH A DEEP CONTUSION AND TO STAY OFF WORK UNTIL 8/9.

EE received injury to hand when trying to catch bracket that fell out of bench vise. A mechanic was using an easy out to extract a bolt that had broken off in bracket & when it slipped out of vise, EE reached out to catch it. The tap punctured palm of rt hand causing immed. swelling. EE taken to hospital, referred to specialist to check for bone or tendon damage-none
