Tech was climbing up back of 637 Scraper between transmission and rear fender to change a pump. Tech had grease on bottom of foot and he slipped and fell backwards. As he turned to catch himself, he fell to the ground approx 4 feet. He fractured his left wrist trying to catch himself.

When removing hydraulic filter, strained shoulder.

Employee complained of muscle spasm in his back while operating a 637 Cat scraper in tandem with another operator. Employee was transported to the ER where he was checked out and taken off work. Ongoing.

Employee drove his 775E haul truck over rough ground at the dump location and strained his arm as he held onto the steering wheel.

The employee was standing on a rock about 12" off of the ground loading secondary rounds in a larger rock behind it when his boot slipped off of the rock twisting his left knee.

Employee was attempting to pull a piece of four inch pipe on the stockpile when the pipe got caught between drip and other pieces of four inch. The employee was then thrown off the ATV which then fell over on the top him.

Employee was lifting a roll of skirting and felt pain in lower abdomen. EMPLOYEE WAS ON OVERTIME (15 HOURS)

While trying to move a starter sheet, employee felt pain in lower back.

Employee was stepping out of forklift, when he twisted his left ankle. This was not determined recordable until medical review on 3/19/2007, employee will be required to have surgery.

Employee was removing conduit in the central tank house and some airborne sulfate dust got into his eyes. Employee was taken to local clinic and was given a prescription for his eyes and released to full duty.

Employee was in the process of removing an impeller from a pump in the shop when he suffered a laceration to his left index finger and a small cut to his right index finger. Employee was taken into the ER room and given a tetnus shot and anit-biotics. He was refered to a specialist and scheduled to have surgery to repair the tendon on his left index finger (3/9/07) On Going

Employee was traveling to the SX on the access road when an oncoming contractor vehicle swerved to miss some deer on the road colliding head on with the employee. Employee was transported to the hospital via ambulance. Employee suffered from whiplash and contusions to her chest area. She was given pain medication and taken off work for two days.

Employee noticed that a valve on cell 20 was not close all the way. He went back to re close the inlet valve on cell G 20 when he splashed electrolyte that was on the floor and electrolyte got on his clothing and saturated them and electrolyte got on his right calf on his right leg.

Employee was climbing down from the bed of the CC-152 boom truck when he started descending the boom truck his right foot slipped off the rung, causing his right leg to hang up between the rungs on the dismounting ladder.

Employees were moving a 4x8 sheet of diamond plate, when the wind caught the plate and one employee lost his footing and dropped his end, causing diamond plate to fall on top of left foot

Employee was putting grating back in place in the mill by the feed trunion air jack when the grating slipped. His left ring finger was lacerated.

Employee was operating a skid steer loader with a scraper attachment. She was back dragging fine crushed material when the scraper blade caught on some structure and brought her to a stop. The abrupt stop resulted in pain to the neck and shoulder area.

Employee was in the process of retrieving a rope, that had been thrown over the bank, for cable removal. He came up from the bottom of the bank and when he pulled on the rope rocks came down from the bank and hit him. The employee lost his balance and fell causing injury to his shoulder. He was taken to the clinic.

The employee was stepping out of a loader and slipped on the damp ground turning his ankle.

Employee using a exit ladder which was up in retracted position on the side of a new P & H Shovel when the weight of the employee caused the ladder to extend down approximately 8 feet causing sprain to his left ankle. Employee was placed on restricted duty.

The employee was stepping down from a Loci and stepped on a loose rock along the rib turning his ankle.

Employee was working under the pan feeder of Crusher 2 clearing rock off a concrete ledge so a skid steer loader could scrape the material out of the pocket. The skid steer scraper attachment made contact with her left ankle causing a laceration.

Employee was trying to take off a bolt that was stripped. Employee used a prybar to secure the bolt to prevent spinning. The bar slipped when the impact was applied and employee caught his thumb with the spinning bolt causing a laceration to his right thumb.

Employee was coming out of the front gate when he turned his ankle. He was taken in to the er. Employee was taken off work. On going.

Employee was pulling on rubber belting to straighten it when he felt a pop in his groin.

Employee was working in ball mill while other employees were using a jack hammer to remove a bolt. When they hit the bolt, feed fell between his safety glasses and eye. He flushed his eyes, but by Wednesday, 2-21, his eye was irritated. He was taken to the doctor, a small speck of metal was removed (not embedded), given antibiotics and returned to work.

The employee was attempting to clear a ball jam in the chute for the #4 mill. He put his hand in to the chute to move a loose ball and the following balls rolled forward trapping his hand in the chute. While trying to dislodge his hand with his other arm, he scrapped his forearm.

EE was installing conductors into conduit. He was pulling in a downward motion from above his head to below his waist pushing conductors into the conduit. This motion exerts force int he elbows and shoulders. The strain/sprain is in both elbows where the forearm and tricep ligaments attach. (EE was placed on 4 days restricted duty: he had personal days scheduled prior to the

Employee states over a couple of months he has experienced pain in shoulder and arm as a result of lifting.

Employee was tightening bolts on a valve when the wrench slipped off bolt and struck employee above the right eye.

Employee was climbing down the ladder of his haul truck when his foot slipped. He did not feel any pain at the time and went home to sleep. When he awoke his lower back was hurting.

Employee was stripping wire with a razor knife. The knife slipped and he cut his left hand.

Employee was operating a dozer, another employee was operating an end dump and was given the signal to dump by the spotter. The spotter then realized that the dozer had moved but the material was already falling on the dozer. A rock landed on the dozer operator's hand, causing a laceration.

Employee removed the choker from the crane lowering the impactor at the crusher. The impactor shifted causing it to fall and caught his ankle between it and the old headnut on the floor. This resulted in a contusion to his left ankle.

Employee's scraper came to a sudden stop when the slack was released after the hook up. Afterwards, employee began to feel some discomfort in his neck and head. He was transported to the ER where he was checked out. He was taken off work. RTW 2/21/07.

Employee was pulling electrical wire up on cable tray on conveyor. The rope they were using to pull with came undone. The cable fell down and struck him on his hand causing a hairline fracture to his right index finger.

Employee was attempting to adjust the forks on the forklift when his hand became caught between the mast and the fork causing a contusion to his right hand.

Employee was removing used ripper points from the back of his service truck. After tossing the next to last ripper point and reaching for the last ripper point, the employee felt something impact his left tricep. He did not see anything and continued to reach for the last ripper point when he noticed blood trickling down his arm.

Employee was walking down the stairs at the mill maintenance building when she slipped and fell landing on her back. This resulted in a strain to her neck and back.

Employee reports while descending haul truck ladder, felt pain in back of knee and fell.

Employee was stung by a bee as she walked to the front gate. The bite site of right arm got infected and was treated. Ongoing.

Steel plate was pressed down and tacked in place. Employee then took position on top of plate to continue welding. While sitting on plate performing job duties, the tacks failed causing a sudden release of stored energy jarring employees lower back. EMPLOEE DID NOT START FEELING PAIN UNTIL 2/20/07 AND REQUESTED TO BE TAKEN TO THE CLINIC.

Employee was loading pulley onto truck using forklift when pulley rolled off of forks and smashed employee's fingers between bearing housing and torch box.

Employee was pulling 3a shorthead crusher and while exiting the stairs by the 2b shorthead he turned at bottom of stairs at ground level and stepped on pin on step bearing plate causing sprain to right ankle. He was looking up towards crane at time of incident.

Employee was moving transformers down a set of stairs assisting other employees. Transformer was manuvered sideways catching employees pinky in a pinch point causing a fracture. No days away. No restrictions.

Employee was on the lower rungs of a ladder in the mill grinding area when he lost his balance; his face impacted the ladder causing him to fall to the ground. This resulted in a minor cut to the chin and a strain to his right shoulder.

During shift change Employee was moving toward the back of the 140-0 van to be seated. The individual driving the van began to move while employee was still upright causing them to fall. The driver of the van didnt stop to make sure employee was in the seat.

Employee was driving water truck w/air seat, truck went over bump and employee's hard hat hit roof of cab causing him to bite down hard and fracture tooth

Drill string hung up and then dropped, smashing employee's right thumb between water swivel and chuck jaws.

Employee was at the B4 stacker belt preparing the end of the new belt by cleaning up the cables when the utility knife slipped resulting in a laceration above the right knee.
