Employee stepped from the engine hood cover to the cab access platform and sprained right ankle.

Operator slipped dismounting a motor grader, lost grip on the ladder and fell on the grader blade.

Employee had to restack liners because the original pallet was damaged and needed to be replaced on another pallet, an extra task that can be avoided if the original pallets are better managed. When moving the concave liners from a damaged pallet, one of the liners shifted and crushed a finger, causing an open tuft fracture of right index finger with nail damage.

Employee was cleaning under the lime plant and fine material from the screw conveyor was coming down and the employee believes they inhaled some of the airborne lime. The case did not go recordable until 09/23/2025 when the employee was given restrictions.

After getting a bucket of material, the loader loading the haul truck backed up and turned too quickly causing the bucket of the loader to make contact with the truck bed. This caused the haulage operator to suffer a back muscle strain. Case went recordable on 09/13/2025 when the operator was given restrictions.

Employee was connecting a flange to a pipe and was struck in the mouth by one of the bands.

A Blasting Operations Technician was opening a bag with a utility knife and cut their leg. This caused a laceration that required stitches. The employee returned to work the next shift.

Mine maintenance employee walked into Lube Truck Chock Holder on front of truck striking with hard hat. This caused bruising to face, nose, and a chipped tooth. Employee returned to work next shift.

Heavy equipment maintenance employee opened roll door for heavy equipment fueling and used more force than necessary. Hand bumped into top of door frame causing some bruising of right hand/bones restricting ability to squeeze and lift with right hand.

Employee was working on grader replacing cutting edges. The employee over torqued a bolt causing it to break loose. The bolt hit the underside of employee's hardhat ricochet to employee's forehead striking employee above the right eyebrow, creating a laceration. The laceration needed 5 stitches to close.

Left index finger cut by knife requiring 6 stitches.

Multiple oversized boulders were loaded onto haul truck HT107, creating a rough load event. This sudden impact resulted in the operator sustaining a back injury severe enough to require a mayday call over the radio. No vehicle damage occurred; however, the operator's injury required immediate medical evaluation.

During Fuel Lunch Line at the Fuel Island, Maintenance Lube tech was tasked with guiding Haul Trucks for fueling. When crossing the pedestrian barrier to complete ground guiding they encountered uneven ground which resulted in losing foot placement and rolling an ankle resulting in injury to the left ankle.

Employee was carrying a liner with another employee. The liner almost dropped and the employee attempted to catch the liner before it hit the second employee. The liner caught the employee's right pinky finger between the liner and feeder and the finger was pinched. The pinch lacerated the finger, which required 3 stitches to close.

Operator closed grid box of Liebherr T-264 Haul Truck during pre-operation inspection and pinched their right 5th digit between the latch of the panel and the panel itself.

Removing fuel filter from a loader with filter pliers, felt pop in right elbow causing strain, restricted duty

Passenger van (Ford E-350) carrying crew members was dropping off an operator to the haul truck which was staged at the fuel island. When backing out of the service lane, the van made contact with a concrete barrier protecting the fuel pump area.

Employee claims employee got debris in eye after starting heater in light vehicle.

The haul truck was lodged in mud and being removed by the maintenance team.

Working alone

The technician was using a tool to hold pressure up on a plow bolt to prevent the bolt from spinning while removing the nut. The tool slipped and the technician's hand contacted the spinning bolt head causing a laceration.

Employee was working in wet and muddy conditions. Employee slipped and twisted EE's right knee while trying to catch EE's balance resulting in a sprained knee. Employee was evaluated and put on light duty.

A technician was pulling a final driver cover. As the technican was raising the hoist to remove slack from rigging material, the hoist continued to raise causing the lifting link/bolt to break out of the cover. The lifting rigging struck the employee in the wrist resulting in a laceration and contustion.

Haul truck operator was being loaded by contractor shovel trainer operator. Shovel operator didn't hoist the bucket high enough and contacted the dove tail of the haul truck.

Loss of traction while descending the steps of the shovel.

Diesel diagnostic mechanic was working on air stem of inner dual tire. Ice/muck material that was frozen to underside of haul truck bed melted from haul truck surface and released due to gravity and fell on head, neck and shoulder.

While walking out to trash can, right foot slipped on ice while coming down with left foot. Left leg kicked out from underneath to the left. Employee sustained a contusion to left knee. Employee is scheduled at orthopedic specialist for further evaluation.

Incorrect procedure followed to dislodge a rock lodged in the rear duals of a haul truck.

Employee was moving a large toolbox from the shop bay to the toolbox storage locker. The toolbox then struck an electrical cord protective cover, which made the toolbox off balance causing it to tip over. The toolbox landed on the employee's foot. Employee was diagnosed with a contusion.

Diagnosed with 4th and 5th distal phalanx tuft fracture from pinch point incident (tuft fracture is chip of bone)

Incorrect tooling used for the task and incorrect technique used.

Employee was using a come-along to help install chains on a Liebherr T264 at the mine site. While installing the chain, EE slightly tweaked EE's back. EE continued the remainder of EE's shift until 7:00pm and returned home for the day. The following morning EE came to the site to report soreness in EE's back.

While exiting a crew van a miner's left knee made contact with a van door that resulted in injury to the miners left knee cap.

Replacing return roller on conveyor during shutdown operations. While trying to place roller in hanger other end came out of hanger and roller moved and employee's fingers were pinched between belt jack base and roller. Right hand 4th and 5th fingertips needed stitches and tips of fingers are getting further evaluation by specialist for additional bone or soft tissue damage.

The employee was climbing a permanent emergency ladder on the shovel and it released and when the feet of the ladder hit the ground it caused the employee to loose grip on the ladder rungs causing a fall to the track. EE landed on their left knee on the steel of the track.

While removing liners from Surge Bin. Employee lost their balance as EE was swinging hammer to break free old liners. As EE stepped down to regain balance EE rolled an ankle on the materials that were left on the deck walkway causing a dislocation to right ankle.

While waiting for the loader EE was sitting in driver's seat of service truck and reached right arm back to grab something from behind the seat and in doing this EE has dislocated EE's shoulder.

Finger pinched between hose and guide

During a maintenance activity a miner working in the heavy duty truck shop exited the building outside of bay #8. While walking outside the building the miner stepped onto a rock, resulting in the miner experiencing severe right knee pain.

Two employees were involved in arc flash while attempting to test rotation in a Medium voltage transformer. Employee sustained burns to right side of face.

Driver of Haul truck sustained cervical sprain during a rough loading event where large rock material was concealed in muck of material.

Employee was checking fluids on the top level of a 994 loader in the manhole behind the cab. When EE went to climb out, EE placed hand and the panel closed on EE's hand causing lacerations requiring stitches.

On 9/4/2023, employee was trying to clear a plugged feeder chute with a pry bar, employee's hand slipped off the bar and the tip of a finger struck a rock. Employee did not report the injury until 9/15/2023 and sought medical treatment on 9/16/2023.

Employee was lifting a rock guard when the employee strained a wrist on 8/18/23. The incident became reportable on 9/5/23 after a routine follow up appointment when the employee was given restrictions.

EE received 5 stitches to tip of left-hand middle finger after dropping a core barrel with estimated weight 100lbs on left-hand middle finger, this then got pinched between the Core Barrel and a rock on the ground. EE was wearing gloves, but not the impact type that may have helped reduce the severity of the injury.

While working in a tight space the left hand that was supporting the bottom of a sledgehammer handle was pinched between the handle and surface causing the left fifth finger/pinky fractured. Returned to work same shift.

On June 20, 2023 and employee reported that EE's right knee hurt from bumping it on the step of the lube truck on June 10, 2023 and wanted to see a doctor. The employee reported that EE's foot slipped on the step and EE bumped EE's right knee.

On 6/6/2023, a mill maintenance employee was struck in the hand by a broken lifting eye attached to a pump motor. The employee was prescribed a brace on 7/25/2023 making the case reportable.

Employee's foot sunk into material inside the SAG mill while pushing bolts through the bolt holes for a liner change. This action caused the employee to twist knee suffering a strain.

Maintenance mechanic was using a pry bar while replacing a motor guard on an excavator, when the pry bar slipped from the grip of the mechanic resulting in a muscle strain to the rotator cuff of the right shoulder.

Employee slipped on a concrete floor and dislocated a finger when they caught themselves on a foundation block.

Miner was attempting to step up onto the flatbed of a light vehicle from ground level. The miner directed all weight to the right knee when it buckled beneath them, resulting in tibial plateau fracture.

Technician 1 struck GET with hammer found in workshop (hardened steel) whilst technician 2 applied force to GET with a pry bar, on the 3rd strike a metal fragment dislodged from GET and struck Technician 3 in the left arm (inside of elbow) after piercing 3 x layers of work clothing.

Operator of a Haul Truck was in the process of being loaded by a shovel, operator reported that the dumped load into the back of the truck shook the operator in their seat. Operator reported discomfort and was scheduled for evaluation on 3/20 and treated for a back strain, operator was released with restricted duty. Follow up appointment is scheduled for 3/29/23

Heavy Equipment Operator was dismounting off the last ladder step of a CAT 789, when their backpack slipped off their shoulder causing them to twist as they stepped off the equipment. This action resulted in the operator stepping off in an awkward position resulting in a sprain to the right ankle.

Lower back strain. operator had quick body movement reaction due to loader arm making contact with haul truck bed and/or the ore material loaded into bed of truck at a faster rate when loader bucket was emptied. did not request medical review until 2-26-2023

Geologist reported that using UV fluorescent light caused conjunctivitis.

While using a small ore sample tool, like a smaller version of a spade shovel, employee met some resistance while using tool and after applying more pressure on the shovel and twisting it, removed shovel and felt strain in back.

Heavy equipment operator was asked by a mechanical contractor to help EE lift EE's toolbox into the bed of truck. As they were lifting the toolbox the operator felt a pop in EE's right muscle. Didn't hurt at the time, but when mentioned the following day EE was taken to ER for eval, provided restricted duty (for one day but then missed days for ultimate surgery).

Operator was in haul truck, excavator had placed 2nd load into bed when EE felt truck slumping so EE placed bucket on the HT load to stabilize causing jarring. HT driver drove away, dismounted, taken to hospital for eval. Told to not return to work until evaluated by doctor. Upon evaluation, was told to return to work with restrictions. Had already given 2 wk notice, last day Tues.

Employee was beginning shift for the day and entered a light vehicle. Employee noticed there was debris in cupholder of light vehicle and decided to blow it out with their mouth resulting in debris flying into eye.

After completing office work, the employee exited the office area into the water plant truck bay next to the polymer area when employee slipped on wet polymer resulting in a twisted knee.

Grader operator was in the process of backing up and as they turned in the seat to watch behind them, they felt a pain in lower back that shot up their back to their neck. EE reported the pain and was taken offsite for evaluation. No adverse conditions noted.

While performing inventory of excavator teeth in laydown yard, employee stepped on uneven ground, twisting their ankle. Employee was seen onsite by emergency response personnel, then taken offsite to the hospital. Was provided a boot to immobilize ankle. No scheduled work was missed.

Really small metal object imbedded in eye during maintenance activity. Did not notice till later due to irritation

Shovel operator scooped up a load of material to place in the bed of a haul truck. The impact of the load into the bed caused the windshield to crack, and jarred the truck driver (wearing seat belt). Driver reported back pain; was taken offsite for evaluation. Doctor took x-rays, nothing of note, provided pain medication and told haul truck driver to take 3 days off for recovery.

Front tires of 789 Haul Truck lifted off ground during unloading. When came back into contact with ground caused back strain on operator in cab.

Employee went to an outhouse, slipped on the icy floor, felt like they pulled a muscle but not reported. At the end of shift on 4/9 they reported their back was sore due to not sleeping well. On 4/11 they said that sore back was due to the slip on 4/7. EE was taken to a clinic for eval. on 4/11, released, no restrictions, returned to work. Received physical therapy on 5/12.

Employee was walking down the steps of a haul truck and when EE stepped off the bottom step onto the concrete floor, EE rolled left ankle which resulted in a sprained ankle.

Employee was attempting to start a compressor with a pull cord and struck EE's elbow on the side rail of the pickup truck.

On 11/16/2021, employee was cleaning and moving bolts for an upcoming project. Employee felt strain in EE's left wrist that evening and on 11/17/2021 EE noticed that there was swelling as well. Employee received a strain to left forearm.

Employee was using a bottle jack to lift a water pump and felt something on the elbow. EE looked and found a large bump on the end of it. It was later diagnosed as bursitis.

Mechanic was inspecting parts under the deck of a Pit Viper drill and moved to get a better look and strained EE's back.

Employee was coming down a dozer by stepping onto a ladder in the Climax shop. While stepping down with right foot and holding onto a handle on the upper push brace with left arm, the ladder potentially shifted causing the employee to lose balance and spin around, straining left shoulder in the process.

Removing axle final hub plug with socket wrench tool slipped thumb pinched between tool and rim causing 2cm laceration left thumb requiring a few stitches.

Discomfort and some some limited range of motion in right shoulder due to use when shoveling snow. Repetitive use pushing and some lifting. Working with restrictions.

Employee was stepping backwards off of the ladder of a dozer when EE felt something pop. EE's thigh by the groin area started hurting along with right side abdominal pain.

Not communicating with work partner on task. No locking out the equipment in use. Coring drill

After completing an extensive investigation it was determined on 9/11/2020 after conversations with diagnostic physicians that an employee suffered an arm muscle strain from an unknown cause.

Employee was cleaning up work area at CV-09/10 transfer chute prior to taking a break. Employee placed EE's hand on an I-beam and the chute door that was leaning against another I-beam slipped and smashed employees right ring finger between the I-beam and the chute door. The result was a laceration and fracture to the right ring finger.

Operator was turning on the heater valves of a CAT D10N Dozer and ring finger on left hand made contact with the alternator fin.

While servicing a hydraulic shovel employee noticed low engine coolant, when approaching cap/fill port cap released. Coolant sprayed on employee body causing minor thermal burn to face and esophagus. No permanent impairment.

While exiting a man van, the employee slipped on the step and fell to the ground. EE initially claimed to be okay, but after completing the pre-shift inspection and getting to the cab of the truck, EE felt dizzy.

Employee was performing a workplace exam at the Pit Shop. When EE was inspecting the used coolant tank level behind the Pit Shop, EE slipped on the ice, landing on EE's left ankle, which caused it to roll. Employee suffered a left sprained/strained ankle.

The employee was walking on snow that had recently been ran over by a haul truck resulting in a very icy surface, and slipped and fell, landing on EE's back. EE suffered back pain along with abdominal pain.

A haul truck operator struck a shovel bucket with the truck during an attempt to spot for loading. The driver suffered spinal muscle strains.

A haul truck operator suffered a rough load resulting in strained cervical spine ligaments. The employee complained of shoulder, neck and head pain.

The employee was descending a step ladder in 10 bay of the pit shop. When they reached the second step and stepped down, they missed the first step and fell backwards onto their outstretched arm resulting in a sprained wrist. The ladder also fell to the ground.

Employee was moving a 4 inch x 4 inch wooden block and received a splinter in finger. Employee reported injury on 8.30.2019. Employee refused medical attention. Employee returned to work on the next regular work day on 9.3.2019 and stated that they had visited a personal physician and was prescribed antibiotics. Finger became infected.

A powder employee was working near a highwall when a rock fell and struck them in the back, causing bruising and pain with breathing.

Employee was training another employee on Shovel 2. Trainee over extended the boom under full load, causing the shovel to rock on its tracks and land hard. Employee/Trainer was in the trainer/buddy seat and felt the impact in their neck and lower back. As the night and days went on, pain increased.

The employee was sweeping the floor in the truck shop with a push broom when the employee felt a pop in right foot. Employee was unable to bear weight on that foot and was taken for evaluation. Employee was diagnosed with a "right foot condition".

Employee was servicing a loader and as employee went to climb into the lube truck, felt a pinching in left groin area. As the employee continued with a few more services, the area became more painful the more the employee got in and out of the lube truck.

After completing an investigation, an employee alleges that EE suffered back pain in the evening after completing EE's shift on 04/02/2019 and sought treatment on EE's own on 04/10/2019 and notified supervisor at approximately 8:00 PM that EE went to the doctor and the doctor notified EE that EE had a work related injury.

The employee was climbing stairs and a toe slipped off a step. EE lost grip on the handrail resulting in hyperextension of the small finger of the right hand causing a fracture of the associated bone in EE's hand.

The employee was changing filters on the deck of a haul truck. When employee set a filter down employee assumed a bent-over position. When employee straightened up, employee sustained dorsalgia and muscle spasms in lumbar area.

Supervised contractor was rolling a cyclone vortex into position for a crane pick and in the process tore EE's bicep.

During a probable seizure, this employee sustained a fractured right upper arm and a laceration above the left eye. The event was unwitnessed and occurred during end-of-shift change in a loader that was tied down.

Electrician reported on 12/18 that their knee was sore from using the stairs in the crusher building on 12/17.

Miner cut left thumb with the pocket knife while cutting plastic band.

The employee was trimming a wooden wedge to plug a leak in a tailing line. The knife cut completely through the wood and lacerated EE's hand. EE received six sutures to repair the wound.

Mechanic was bent over using a hand held needle scaler to clean the bucket on a loader, when they stood up they noticed there was pain in their back. Mechanic was saying that when they breathed they felt pain in their back.

An employee was using compressed air to clean bolt holes and got a metal fragment in right eye.

Employee picked up a 67 lb. conveyor belt scraper from a storage shelf and put it on left shoulder. As employee was carrying it to the truck, they felt a pain in their back between and below their shoulder blades.

Employee parked rubber tire dozer, RT01, at the fuel bay and dismounted the machine. Employee climbed back up to the cab and felt something in shoulder pop. No pain or discomfort followed. After fueling the machine, employee climbed up the ladder to remove lockout key and while climbing down felt something tear in left bicep and was in pain.

The EE was walking down stairs and felt left knee give out, resulting in pain.

The operator of a Cat 789 Haul truck received a back injury when the seat in the truck bottomed out while crossing a dip in the haul road. The operator was not permitted to work until seen by EE's personal physician.

Security Officer alleges that EE slipped and fell while shoveling snow at the fuel dock.

Dozer operator parked dozer on the stockpile at the end of the shift where there was a slope. It snowed the evening before and there was snow on the stockpile. The operator walked down the slope to the man van at the end of the shift and slipped on the stockpile.

While clearing the metal bin chute, the employee chose to manually move a rock by self and in the process strained right shoulder.

Employee was packing bags and the bag clamp stuck on the loop keeper and when they put the loop through the keeper, the clamp released and struck a wrist.

Employee was carrying two 20 lb. propane bottles from a light vehicle into the reclaim area. Employee slipped and lost footing causing EE to fall to the ground on right side which resulted in a sprained right ankle.

Despite having plowed and applied scoria in the morning, a mechanic slipped while doing a pre-shift inspection on a tele-handler forklift and fell. Initially EE said was fine but on 2/22 EE said that EE wanted to see a doctor because shoulder was hurting.

The employee was using a bar to help remove a bridge in a transfer chute and suffered a strained back.

Employee was removing crusher liners with a double jack and felt pain in the lower back.

Employee was walking to the crusher building and slipped on the ice and fell, landing on left side and sprained left wrist.

Crew was setting poles; injured employee put their hand too close to the setting chain. When tension was applied to the winch line, the setting chain rolled onto injured employee's thumb causing the thumb to be fractured. Injured employee was taken to the nearest medical facility where minor surgery was performed.

The operator was walking in the tracks the loader that employee had just climbed out of. The loader tires had compacted the fresh wet snow that created a slick icy track and the operator slipped and fell on the compacted snow.

Employee alleges that EE slipped on snow despite having cleared the area and fell down suffering an injured back.

Shovel Operator was digging a hard high wall toe and suffered a lower back strain.

The employee was using a torque wrench and pulling very hard. EE's foot slipped as EE caught self on some all-thread, it lacerated the right small finger.

Employee was walking up the stairs to the liner deck in the Mill when employee felt a pain in neck and became dizzy. At the top of the steps employee had to duck under some caution tape. Employee does not recall what happened after that until some co-workers came to employee's aid.

Employee was walking in the administration building hallway, lost ee balance, and fell backwards striking ee head. There were no defects in the walkway or slippery surfaces that contributed to the fall. A personal medical issue may have contributed to the loss of balance that precipitated the fall.

Employee was pushing material to a loader with a dozer. As ee was backing up after making a push, ee was distracted by grab iron and step that had come loose. Dozer backed over uneven ground that caused the back to break over and come down hard. Employee stated that the impact had jarred and injured back.

Despite having gravel and scoria spread throughout the ready line, an operator conducting a pre-shift examination slipped in the parking ditch at the ready line and fell. Initially the employee did not report an injury. On 1/22/2016 the employee reported that ee back was sore and wanted to see a doctor.

After a thorough investigation and late notification of incident, we have determined that a back injury was sustained. The employee was retrieving his lunch box on the tracks of a dozer. While lifting it he felt and heard his back pop.

Employee was using a long wrench to loosen a nut on a Cat Dozer and felt his shoulder pop. Employee sprained his left shoulder.

Witnesses saw the employee "twitch" on a ladder. He handed his tools down and reported back pain which was ultimately diagnosed as a thoracic muscle strain.

While resetting fuse for A/C unit in cab, shovel operator stuck his left hand inside the housing. When the fan came on the operator lost his balance causing his left pinkie to make contact with moving fan. Contact caused 2 lacerations to the finger requiring stitches.

Employee was grinding during his afternoon shift. On his way home his eye began to bother him. Employee had a piece of metal embedded in his right cornea. An optometrist was able to remove the object.

Affected employee was helping carry a magnet belt. When his partner lost his grip, the employee caught the load and injured his back.

Directly supervised contract employee stepped on a rock when dismounting a ladder and ankle.

Employee had completed a welding task and was in the process of cleaning up area. As the employee began to fold the ladder utilized in the task, he looked up and a piece of slag fell from off of one of the ladder rungs. Report states that: "The slag fell through the top of his safety glasses and ricocheted in to his eye".

While doing cleanup after a job, the employee stepped on a large bolt and sprained his ankle.

Employee removed hard hat and face shield rubbed eye with his sleeve, felt particle in eye, washed it out returned to work, next day eye was irritated went to dr debris was removed (not embedded) rust was drilled out.

Employee was closing a lid on a valve vault, fingers were caught between lid and frame causing a crushing injury to two fingers.

Employee was climbing down off of a motor grader and fell landing on the mould board.

Employee was mixing a bucket of backing material when some splashed on his legs.

Employee was walking down a slope to put a prism on a bench when he slipped/tripped and fell forward spraining his fingers/hand.

Employee exited his truck and slipped on ice while walking to the reclaim.

Employee felt a "pop" in his shoulder while using a banding tool to secure product to a pallet in the warehouse.

Employee was using a hammer to install picks and strained his back.

The IW was removing tile from a wall during a locker room remodel. The piece of tile slid from his hand and fingers down his arm lacerating his arm, requiring stitches.

Employee was driving wedges to hold the shaker screen side boards. When she struck the wedge, the hammer bounced back at a weird angle causing her wrist to twist resulting in a fractured wrist.

Ply board form rubbed against bicep and splintered. Splinter removed by urgent care doctor.

Cleaning curls away from a magnetic drill with right index finger and got finger caught in drill bit.

While raising the basket during mine rescue training, tripod collapsed. As the tripod collapsed it struck affected employee in the left leg. Employee was transported to area clinic and was diagnosed with "knee contusion".

Employee was removing lip shroud protector bolts from a 998H Loader. He placed his hand over bolt head in order to keep it from spinning. While using 3/4" impact wrench from above to loosen, the bolt head pushed through into employee's hand and caused a 2" laceration to his left palm just below the thumb. Employee received stitches.

Injured person was running a chipping hammer in a drop tower where he reported injuring his left shoulder. He went to the doctor where they diagnosed him with tendonitis. He was prescribed physical therapy, Naprosyn, or over the counter equivalent and restricted duty. Reporting doctor concluded the case a pre existing injury and was concurred upon by workmans compensation.

Per initial incident report from supervisor: Mill employee was "lifting bucket full of rock to dump onto the belt, turned a little awkwardly and strained his back". Incident was initially reported as a first aid however a physical therapy report received on 5/16 for a visit from 5/15 indicates ultrasound and electrical stimulation was administered.

Employee reported that he strained his back while welding.

The employee was using a scissor lift and boom lift to put up a support structure for an overhead pipe. He and another employee had positioned the beam with a chain hoist. While they were positioning the beam it started to rotate toward him (roll) the employee let go of the chain and tried to stop the beam from rolling when it rolled over onto his hand.

Employee was measuring the depth of the explosive as it was loaded into blast holes and allegedly fell on his shoulder. The incident became recordable on 08/27/2013 when employee received a medical procedure to alleviate pain associated with osteoarthritis.

Despite continuous application of abrasive material on roads and parking lots, and employee slipped and/or stumbled while exiting a pickup truck and struck his knee on the side of the pickup truck.

Employee tried to pick up a metal pipe that was heavier than she thought and felt a pop in her left wrist.

Employee entered elevator to report for duty in the Crusher Control. Elevator moved approximately one floor and stopped. Employee was in elevator for approximately 45 minutes. Elevator company inspected elevator and determined the door had been slammed shut which resulted in the door open indicator falsely reading an open door which stops elevator movement.

While placing wheel chock on the back of a truck, employee's feet slipped in mud causing him to smash finger between the chock and the rail of the truck.

Employee was washing area with hose, stepped on pant leg causing employee to stumble into a hand rail.

Employee was ascending stairs and felt his knee "go out"

Employee suffered an apparent seizure and fell causing lacerations.

While handling the rod, employee cut his finger on a burr.

Employee was walking with material when the left foot slipped. He caught himself with the right foot, landing in a split, causing a strain to the right groin area.

Field mechanic was attempting to R&R cutting edge blade on back-hoe. When he loosened the first carriage bolt, it dropped down out of the shanked bolt hole. The mechanic used his left hand to push the bolt back into the slot and used his impact wrench to continue to loosen the bolt. The bolt spun and cut the tip of his left middle finger.

Employee reported a foreign body in the right eye from grinding and welding.

Employee notified the job on 12/16/11 that he thought he got a foreign object in the eye while grinding the day before.

Employee was grinding pipe supports when he felt pain in the right eye.

Employee was electrofusing a fitting onto a 2" piece of HDPE pipe. Employee removed his gloves in anticipation of completing weld and shutting down the welding machine. Employee abruptly turned away from coupling when the hot molten around the coupling exploded out from the fusing site. Employee was approx 2' away from area when molten landed on employee.

Employee was stripping concrete forms at station 10 + 94 on the tailing delivery line. The support structure had been poured 10/27/11 the assigned task for the morning was to strip forms in the process of removing the anchor bolts with a pipe wrench the bolt broke free causing the employee to stepped back causing his ankle to roll.

While walking from parking spot to mill offices, employee slipped and fell.

While towing a rod trailer down a decline employee lost control of Skidder it stalled. Employee dropped the blade, as it hit the ground, rod trailer caused skidder to start to jack knife he then chose to unbuckle and jump from skidder. When employee jumped, he landed on his feet with his feet slipping out from under him and then landed on his side.

While "hammer drilling" a 1 5/8' hole thru a concrete wall, the bit got caught on a piece of rebar embedded in the wall causing the "hammer drill" to twist out of the operators hands, causing him to fall forward hitting the "hammer drill" with his mouth and cracking one (1) of his front teeth.

While operating 994, employee felt stiffness in his neck while turning his head.

Employee's dental appliance came loose during loading causing an injury to an anchor tooth.
