Mine Maintenance Employee lost balance and fell backward onto dipper floor when pry bar slipped out of leverage point, injured hip and shoulder.

During an attempt to remove rigging from a hook on a forklift, the operator extended the boom to assist the rigger. Unfortunately, this action caused the rigger's hand to become trapped between the knuckle of the mat and the item they had just lifted, resulting in three broken bones in EE's right hand.

Mill Ops Employee tripped over feed cart track, rolled ankle.

when entering the lube room a 1" shackle fell and stuck EE on top of right foot behind steel toe protection. EE does recall where it fell from when EE entered the lube room.

MIS Employee sustained laceration using cable splicing knife to open a tablet box.

Tire Tech pinched hand with torque gun during maintenance activity on LDR1.

Mill Maint Employee sustained rope burn to their hand while changing out a return idler on 5 Belt XCC.

HT135 Operator rough load injury by SH23.

Liners fell unexpectedly, striking worker's leg. Causing bruising and soreness.

While removing a catwalk by the plenum, a forklift fork became caught under the platform and lifted it slightly, grazing a welder's shin and causing a small cut above the left ankle.

Changing tire on skid steer, employee was using a ratchet strap to wrap tire to aid in seating the bead for inflation. Once the bead was set and able to receive air the rachet strap was released and the handle sprung back and caught the employee in the right forearm causing a 2" laceration that required 5 stiches.

HT135 Operator pinched/lacerated finger by stuck/malfunctioning boarding ladder.

The injured employee was attempting to cut the lower mud ring on a gear half that was positioned on a concrete floor, with the split line partially overhanging a small concrete retaining wall. The employee reached with both hands under the gear to cut the mud ring, and the gear shifted, striking them in the hard hat and both hands.

Mine Ops Employee experienced heat dehydration/heath exhaustion.

Mine Maintenance Employee struck/dragged across ground approximately 6 feet by SH22 hoist ropes during maintenance activity.

Mine Tech Employee mentioned a "twinge" in their leg to a co-worker approximately 30 minutes prior to the end of their shift. About an hour after their shift, Employee reports the pain their leg had intensified to the point that they could barely walk, at which time they contacted their supervisor to report it.

Mill Maintenance Employee reported back pain upon lifting an acetylene bottle.

EE was organizing lower tire yard. While moving wheels to scrap, EE had stacked Wheel #@1 on top of wheel #2, when wheel#3 that was standing next to the stacked wheels fell into wheel#2 while EE's left hand was in the line of fire, resulting in a broken left ring finger above the first joint.

On 3/24/25 at approximately 11:37 AM a mayday was initiated for an employee who passed out. CPR was initiated and the employee was shocked with an AED multiple times. WVFD arrived on scene and took over CPR but was unable to revive the employee. The scene was secured and legal notifications were promptly made.

Employee lost footing while looking for rocks/minerals while taking a break from the rock wall logging work being performed. The employee used the rock wall to brace from EE's fall and received a laceration to EE's left pinky finger that resulted in three stitches.

MIS Employee felt a popping sound while ascending stairs, felt pain in leg.

Employee's right index fingertip was amputated inside the pin hole of the Tube Press 6 ram when the Overhead Crane raised the yolk out of the candle eye.

Employee sustained laceration to finger during maintenance activity on Filter Press #2 in the Mill.

Driller helper had right foot on the drill rod bin skid while marking the drill pipe. When EE stepped down off the skid, 8", EE rolled left ankle. After alerting the mine safety team and following their protocols, they went to a clinic where x-rays were negative. They diagnosed EE with a sprained ankle and put EE on restricted duty.

On 1/16/25 at 11:40am worker was putting pins on a shovel track. As worker stepped down the worker stepped on a rock making rt. ankle twist. ERT on site assessed the injury and recommended a medical evaluation. Diagnosis of a rt. ankle sprain. No fracture. Minor restrictions have been recommended. Employee follow up with clinic in 2 weeks.

Mill Maint Employee struck/injured by angle iron during maintenance activity.

Mill Maint Employee rolled ankle on uneven concrete at Lower Moly Feed Pumps.

Training Dept Employee injured finger attempting to change Dozer D6-4 dipper height; dipper slipped and smashed left point finger.

Dozer D10-23 Operator reported pop in back and immediate pain.

HT204 caught fire; as Mentor was exiting cab they sustained a burn to their right forearm. Employee had only First Aid treatment; however, they currently have restrictions due to and is undergoing treatment mental issues following the incident.

CLP Employee sustained small laceration to left index finger by 50-inch gasket when dropping in dumpster.

Mine Tech Employee reported pinching sensation in knee when exiting Light Vehicle ENG28. Likely aggravation of pre-existing condition.

As a coworker was climbing into a crew van, EE dropped EE's cup of coffee into the boot of the injured employee. The hot coffee caused a burn to IE's right foot!

Mine Maintenance Employee sustained arm laceration by Loader wire rope choker sling.

Mine Maintenance Employee sustained hand laceration through leather gloves while assisting with maintenance on Dozer D10-24.

HT222 Operator lost their footing while descending the boarding ladder and twisted their ankle.

Mill Ops Employee struck/injured by high-pressure water hose nozzle in the rougher basement.

Mill Services Employee working in the Mill Ops roughers basement climbing to unhook a hose caught on equipment/machinery fell backwards from A3 to A4 transition chute from 4+ ft elevation, landing on a water addition line.

Electrician working on GL2 belt reported sharp pain in leg after tripping or "stepping wrong" on step of stairs while reaching for rope switch. Potential exacerbation of pre-existing injury/condition.

SX-EW Employee was splashed in the face and eyes with sulfuric acid diluted solution.

HT229 Operator experienced allergic reaction to hand sanitizer (itchiness and difficulty breathing). NOTE: Shift started at 6:30 PM on 7/12/2024.

Mill Services Employee got lime splashed in right eye while cleaning at the lime plant.

Slipped down stairs, dislocated shoulder which needed to be placed back in.

HT136 Operator injury, stepped in hole walking to van at end of shift and rolled ankle. NOTE: Shift started at 5:30 PM on 06/23/2024, incident occurred at 6:24 AM on 06/24/2024.

RTD3 Operator felt a pop in their left knee as they were exiting the cab. Pain came and went; reported the following day.

HT226 Operator fell while performing pre-shift at Butte Fuel Dock.

Mine Tech Employee got debris in eye due to wind.

HT221 Operator had unknown debris blown into their eye from the AC vent.

While employee was washing at the 2-3 transfer point the pressure from the hose was too much. EE let go of the hose went to run away to avoid getting hit, causing EE to slip and fall.

Dump Truck CD01 Operator felt unknown foreign object blow into their eye from heater vent. Incident was late reported on 03/25/24 by the Employee to their supervisor.

Employee reported shoulder pain after tripping on a piece of flooring that was sticking up. First Aid injury fully released 04/24/24. Requested to reopen WC claim 07/29/24, started PT 08/28/24, completed 10/07/24. H&S notified 10/18/24.

LDR23 Operator injured when contact with unknown occurred under the water level of lower grind floor ditch.

GR25 ladder broke/fell off while Employee was descending. EE was hanging on to the grab rails with both hands and hit rib cage on the grate as EE fell and landed on feet.

HT153 Operator reported back injury after SH11 dipper opened up on the back end of the truck, causing the bed to lift up and come back down.

Mill Ops Employee on the way out of the mine site lost control of motorcycle when deer jumped in front of them.

HT127 Operator injured left arm during operation of boarding ladder.

While changing forklift operators during suspended load operation, one of the operators inadvertently pressed the park break release switch while grabbing a hold of the grab handle when descending - causing the forklift to roll and the load to shift. Injured employee was caught in between pipe flanges - resulting to an injury to their wrist and thumb.

HT132 Operator reported rough load injury at SH22.

SH8 Operator sustained rough loading injury after hitting a hard spot. The seat was discovered to be damaged allowing forward and back movement of about 3-4 inches.

HT205 Operator reported back pain from rough load at SH11. Shift started at 6:30 PM on 10/12/2023.

CLP Employee changing a PLO bag at the bagging station reported feeling a pop/sharp pain in their right shoulder when they reached up to attach the rubber band that holds the neck of the bag onto the feeder.

Tailings Employee hit and injured thumb while hammering nails into 2X6 boards.

Skid Steer Operator had back pain after the vehicle hit a dip and slammed onto the ground.

Employee walking the berm on the Tailings dam and digging out cyclones slipped on wet slimes, fell and injured shoulder.

Maintenance Employee was walking to pick up a light vehicle from the downline for repair when they stepped in a tire depression and twisted their ankle.

Employee exposed to H2S gas on 7/5/2023 reported feeling ill on 7/11/2023.

As Employee was walking out of building through exit doorway, they rolled their ankle on the grating outside the door, causing injury.

Employee cutting thick plastic wrap with a box cutter toward themselves cut their thumb.

Leach Employee picked up portable generator and strained back.

Employee stepped down approximately 10" onto grating, stepped on wash-down hose causing their ankle to roll.

Employee reported eye irritation from debris that fell behind their safety glasses.

HT213 Operator hit bump in haul road causing back pain.

Haul Truck Operator reported that EE's lunch bag shifted as EE was getting into the Man Van at the beginning of shift and aggravated an old back injury. EE took pain medicine and EE's back was feeling okay. After the medicine wore off during the shift EE's back started hurting. NOTE: Reported back pain at 2:45 AM on 03/09/2023; shift began at 6:30 PM on 03/08/2023.

CLP Employee working near the vent valve at the big boiler felt a splash on their face, wiped their face with their gloves on and felt burning.

Dozer D11-4 Operator slipped on muddy step and fell on push arm, causing injured ribs.

Utilities Employee strained shoulder while moving AC unit. NOTE: On 03/07/2023 the Employee was given an order for physical therapy treatment which Employee started sometime shortly thereafter, at which time this injury changed from a First Aid to a Medical Treatment.

Employee tripped over a lifting eye welded to the crusher doughnut and fell, sustaining injury. NOTE: Shift began on 02/22/2023 at 5:30 PM; incident occurred on 04/23/2023 at 4:30 AM.

Employee burned their hand on the bottom of a glass coffee carafe.

Employee was walking/looking down at hand-held device and tripped over forks of forklift. EE was evaluated by WVFD and made no claim of injury. On 4/4/2023 EE reported swelling of right knee and pain in left shoulder since the incident date and was seen at the clinic and given restrictions. EE has not yet been released but resigned on 4/4/2023.

HT Operator reported rough load injury to back and neck from boulder hitting canopy from dipper.

Employee flushing water through a caustic line was sprayed in the face when they disconnected the hose.

Employee rolled ankle stepping down from concrete pad to dirt. Please note that Employee's shift began on 02/15/2023 at 6:50 PM and the incident occurred on 02/16/2023 at 2:40 AM.

HT Operator experienced rough road injury when the hoist lever was hit accidentally causing shaking of the truck.

An MPC driver attempted to connect EE's 3" off-loading hose to an above-ground tank when it slipped out of EE's hand and came back, hitting the driver in the face.

Employee injured their back while lifting a bevel toe skirt liner weighing approximately 45 pounds at 2 Chute XCC.

Employee reported feeling a pop in their hip after sitting down in place when their foot rolled off a rock they were stepping on while straddling and holding a 4-inch pipe to put it in a 4-inch fusion clamp.

Employee walking into work at Mine Ops tripped and fell on the grating in front of the Engineering Building, resulting in a laceration that required stitches. Employee was released to return to work without restrictions.

During the LOTOTO stage of Haul Truck Operator's walkaround, their finger was smashed between the ladder handrails and the ladder release latch.

Haul Truck Operator called a Supervisor to the LSP Tiedown at the beginning of their shift on 9/26/2022 and reported an incident they said occurred on 9/20/2022 at the Plan 9 Tiedown when their ankle rolled, causing them to fall and injure their knee.

Employee was operating HT 202 and was loaded from Shovel 11 and reported they were feeling some back pain.

Rubber Tire Dozer Operator hit a bump and the seat bottomed out twice, causing pain their lower back.

While removing drill steel from AC9, a shim was being used. The shim slipped and lacerated the employee's left index finger approximately two inches.

While pulling cable on south 4, Employee felt a pull/pop in shoulder.

Employee was stung by insect twice in Brown House at Butte Fuel Dock.

Employee hit a hard toe while operating a Rubber Tire Dozer; arm rest jammed their left elbow up, causing pain in their shoulder.

Employee felt pain in their left arm, shoulder and neck due to haul truck jarring.(NOTE: Employee's shift started at 6:30 PM on 08/25/2022; the incident occurred at 12:30 AM on 08/26/2022.)

At the Primary Crusher an XCC Belt Crew Employee was handing 40-pound beveled toe liners into the chute when they felt pain their lower part of their back and had to drop the chute.

A mantle liner was being prepared to be hoisted to another position - rigging from the crane caught one of the ears on the mantle, knocking it off the cribbing onto employee's foot. Foot was not amputated at the time of incident.

Employee's shift started on 08/02/2022 at 5:30 PM. The incident occurred on 08/03/2022 at 12:40 AM. Shovel Operator's knee buckled as they were descending the Shovel stairs, resulting in a fall and injuries to their knee and ankle.

Haul Truck Operator lowered full bed of material down on truck, causing pain in their back, neck and shoulders.

Mill Ops Employee walking in the Moly Plant under the Larox slipped on the steps and fell onto the grating below, injuring their knee.

Haul Truck Operator reported being struck twice during their shift on the canopy side of the inside of the bed by the Shovel bucket.

Rubber Tire Dozer Operator working on the N3 near SH8 stated their back was hurting and was evaluated by the WVFD. Operator returned to work and was seen at the Clinic the following day and they were released to work with restrictions.

Employee lost footing on rocky terrain and fell backward to the ground, causing injury to right forearm.

Operator of loaded Haul Truck traveling to dumping assignment reached to turn on their flashers to go around a rock spill in the roadway and felt a sharp pain in their right shoulder. NOTE: Shift started at 6:30 PM on 05/23/22; incident occurred on 05/24/22 at 12:30 AM.

Employee was carrying a sample bag in their left hand over their shoulder when it slipped off their back, causing pain in their left shoulder and neck area.

While using an impact gun to tighten bolts on a haul truck u-joint, employee reported a foreign object/irritation to the left eye.

Haul Truck Operator leaving the SW4100 empty hit a bad spot on the dump and jarred their back.

Haul Truck Driver Trainee felt a pinch in their neck and back after the truck hit a dip in while they had their head turned looking out the window.

IE was using a pocket knife with a 3-1/2" blade to cut a rubber mud flap for better fitment on a pickup truck. While making the cut, the IE repositioned the mud flap and the knife cut the IE on the inside of the left thumb resulting in 4 stitches to close the laceration.

EE was loading tires on the tire truck at the lower tire storage yard, while walking back to get in the vehicle, EE stepped on a loose rock causing EE to twist the left ankle.

Haul Truck Operator hit a rough spot coming off the dump and felt a pinch in their back. NOTE: Operator's shift started at 6:30 PM on 03/22/2022; the incident occurred at 1:06 AM on 03/23/2022.

Employee was assembling a machine in the workshop under normal conditions and procedures. While tightening bolts with a wrench (standard process, no unusual conditions) felt a "pop" in the right hand shoulder. Minimal pain was felt immediately however the pain grew worse over the next hour. Employee was then driven to medical services for treatment

Haul Truck Operator was rough loaded at Shovel, causing pain in the lower back and hip area.

Employee tossed a shovel full of mud to the side and twisted arm/elbow.

CLP Employee working night shift was processing elevated arsenic material through the direct feed system. After Employee awoke for their next shift (2/22/22 starting at 6:00 PM), they noticed a rash forming on their body.

Warehouse Employee with pre-existing condition in left shoulder was putting boxes away when Employee felt pain in left shoulder.

Employee was cleaning inside of the PLV; they looked up towards the top of the hole when the atmospheric vent blew some sort of material in their eye.

Drill Operator reached up to put water truck hose back up on the reel and heard a pop in their shoulder.

Employee was moving equipment on the grind line when EE stepped on a piece of uni-strut, causing their ankle to roll.

Operator of Loader sustained injury to left shoulder while loading haul truck due to steering being stiff.

Employee in light vehicle driving from pit to mine shop got out of the vehicle to remove a rock out of the roadway, which caused back pain that increased after they began running equipment a short time later.

Driver of haul truck was backing under shovel and was not square to the house. Shovel Operator loaded the haul truck anyway, creating a rocking motion which caused pain in the Driver's back.

While getting into a pickup, employee slipped and jarred ankle. No pain initially felt. About six hours later started to feel pain.

Dozer started leaking hydraulic fluid. Employee pulled the dozer to a safe location, Witness #1 noticed flames on the right side engine compartment. Witness #1 called the "Mayday" and instructed Operator to activate the fire suppression on the dozer. NOTE: Incident occurred 12/7. Injury reported 12/16.

Employee was air-arcing on the revolving frame beneath a P&H 4100A shovel. Hot slag entered through the employees protective coveralls and fell down into EE's steel-toed right boot. The employee sustained 2nd and 3rd degree burns on the top of EE's right foot.

While unloading ANPAN, driver was using rubber mallet to loosen material from side walls of pneumatic trailer when they bent down about 1/4 bend to strike the pod and felt a dull pain in back.

While being loaded by the shovel a boulder fell off and hit the dove tail of the haul truck. Operator began to feel pain in lower back.

Operator was descending down a ramp with a fully loaded Bulk truck at about 15 miles per hour following a water truck. Operator had both the retarder and jack brakes applied but started to slide and over corrected and slowly turned the truck over.

Employee was operating haul truck that was being loaded by shovel, when loaded felt pain in neck and lower back.

Boulder fell onto dove tail of haul truck while being loaded by a shovel, causing pain in employees lower back.

Employee was driving haul truck when they hit a bump, causing pain to their lower back.

Shovel operator went to grab water bottle and left foot got stuck in the foot rest, twisting left knee.

Employee reached into bed of truck to grab tool box when they felt a sharp pain in their stomach.

Employee was installing nut on liner bolt when liner shifted, smashing their right index finger between the bolt and liner.

While employee was backing into SH12, they made contact with the shovel bucket. After the impact employees shoulder and neck began to hurt.

Employee was stripping a communication cable with a knife and the blade cut through the cut resistant gloves resulting in a minor cut on their left thumb.

Employee was working at the 2250 level changing expansion joints. When they came back to the shop for lunch they began to show signs of heat stress.

A rock was dislodge by a pickup causing it to fall into a crusher where miners were working. 1 miner was stuck on the head while wearing a hard hat. The miner was knocked unconscious. Miner received first aid for the incident, and returned to work.

Working outside in temperatures over 100 degrees for multiple days, employee started feeling hot and tired, employee was moved to truck to sit in ac and cool off then started cramping and feeling nauseous

Employee lacerated hand when trying to cut zip ties.

HT216 operator was traveling empty to shovel when truck hit a soft spot in the haul road and reported lower back pain to supervisor.

Employee was using a soft face sledge to remove the blade wing shroud from a dozer.

Employee was washing slurry from floor under moly cleaners during regular shift when they slipped and fell hurting their back.

Operator injured while troubleshooting the delivery pump #6 in the ECC E room.

Loader was loading haul truck when large boulder rolled off dirt in bed and hit hack of the bed. Employee felt pain in lower back.

Employee injured knee and ankle when foot went through a hole in the grating of a sump at the SX.

Employee reported back pain after MOP bus hit a bump too hard.

Employee was descending haul truck steps when their rubber boot got hung up. The employee reached for the handrail to stop their fall and spun around backward falling onto the ground.

Employee felt a pain in lower back while operating a skid steer.

Employee's knee popped when swinging sledge hammer in an attempt to remove a lower bucket pin on the loader

Employee was traversing stairs. Their boot caught grating causing a fall and injured right knee.

Mechanic while using a come-a-long to adjust idler block, come-a-long slipped and free spun with tension on it. This action caused the employee to strain left shoulder.

2/16, Employee injured knee when exiting vehicle. This incident was a later report due to the employee didn't notified supervisor of their injury until days later via email. Employee was seen at the clinic on 03/04 with RD as the diagnosis.

EE was using rotary/demo hammer to remove tile. The spade bit needed to be repositioned. EE held hammer in lap to reposition bit. Hammer slipped & EE reacted by grabbing handle and hit trigger. The chuck closed and EE's glove caught in the chuck and spun. EE's right 2 middle fingers were lacerated by the glove material tearing when caught in the rotation.

A boulder was loaded into the bed of the truck, jostling the truck and cab. Operator felt pain in back.

Employee injured back while helping to stage scaffolding.

Employee slipped and fell, landing on shoulder while doing daily floor checks.

On the above date and time trainee and trainer on 209 was backing under the bucket at shovel 11. Neither the trainee or trainer heard a shovel horn, and the bucket tripped dropping the muck in the bed. The trainee was still backing up, and put the truck in neutral and quickly set the park brake.

Haul truck operator backed up to berm to dump load. The bed would not come up due being loaded heavy on the canopy of the truck bed.

Shovel operator tripped the dipper near the dove tail resulting in rough loading. Haul truck operator felt some pain in their back.

Employee slipped while descending stairs and twisted right ankle.

Employee felt "pop" in their knee while walking up to the grind floor.

Small feed end shell liner rolled into opening on rollers and started to fall. Employee put arm/hand under to attempt to push it back up. Liner fell completely off striking employees arm between liner and roller beam below, breaking EE's arm.

Employee cut left thumb while cutting off twist-tie from tools.

Employee experienced right lumbar strain after dipper made contact with HT.

Reporting employee stated the involved employee mentioned they were having back pain at the beginning of the shift starting at 6:00 pm.

Employee lost balance and EE's body twisted slightly causing a sharp pain in EE's right side middle back area.

While being loaded at the loader and the knuckles on the bucket hit the top side of the haul truck. Employee did not feel like they needed medical attention at that time. On 8/23 employee complained that their back was worse and employee was taken to the clinic the morning of 8/24/20.

Employee reported that while working on a pump they put on rubber boots and felt a burning sensation on legs and feet

Employee reported that while being loaded by shovel, the shovel had a swing divider failure causing the bucket to hit the bed of the haul truck.

Employee assisted in the welding repair on 8/4 at 16:30 - the next day employee reported to the supervisor of the possibility of a flash burn to EE's eye.

Employee told the supervisor was experiencing pain in the right hand. According to the employee, has been experiencing the pain for a long time. Employee requested medical treatment upon returning to work on Sunday, 8/2/2020, and was taken to the clinic.

Employee went over the discharge line and twisted right knee.

Employee was checking at the A-train tails sampler, when EE went to leave EE mis-judged the last step. Employee slipped and fell twisting EE's left leg, causing EE to fall.

Employee & partner were in the process of separating the motor & gearbox of #2 flocculent delivery pump with a come along to pull them apart. Employee was waiting for partner to return, looking toward the truck when the motor & gearbox suddenly separated under the pressure of the come along, the chain of the come along caught employee's hand & smashed it against a nearby pipe.

At approximately 6:35pm the operator of HT152 was getting loaded at SH23, during the backing up process the operator of HT152 came in contact with the dipper of SH23 causing the dipper to trip releasing the material into the back of the truck. Incident is under further investigation.

Operator let the clutch out on blade 18 and it lunged forward whip lashing EE hurting EE's back.

Operator backed over a boulder in D-10-25 and jarred self when it hit the ground hurting EE's shoulder.

EE was changing out the petron barrel under grind line 3. The barrel EE was changing out was half full of petron. To replace the barrel EE put the lid on existing barrel and tried to roll it out of the way. During this process the barrel slipped and caught EE's ring finger on the left hand and pinched it against the back wall.

Employee stated EE felt pain in the neck area starting at 11 AM on 6/8/2020. Employee believes the pain was caused by sitting at the truck desk for long periods of time. Employee called Supervisor on 6/10/2020 and stated was going to the clinic for treatment.
