Working under crusher motor. Was loosening motor base bolts when open end wrench slipped off and struck EE in the upper lip. Received stitch in upper lip.
WHILE USING A 4-LB. HAMMER TO STRAIGHTEN OUT THE ANGLE IRON ON A 70-FT. PORTABLE CONVEYOR, EE MISSED THE ANGLE AND STRUCK HIS LEFT THUMB. 5 STITCHES WERE REQUIRED TO CLOSE THE WOUND.

IN AN ATTEMPT TO UNPLUG THE NORTH CHUTE A LARGE CHUNK FO ROCK FELL FROM A SIDE HITTING THE DI-N-BAR, CAUSING THE BAR TO SLIP FROM HANDS AND HIT EE IN THE JAW. SUFFERED A BROKEN TOOTH ON LOWER RT. JAWLINE.
EMPLOYEE'S WERE REMOVING TABLE SHOE'S FROM SPOKANE CRUSHER WITH A SLEDGE HAMMER WHEN A SLIVER OF STEEL FLEW OFF AND EMBEDDED IN EMPLOYEE'S FOREARM.
