Employee was handling a steel plate when employee slipped and lost control of the plate and caught a finger between the plate and the deck of the dredge.

The employee was kneeling down to measure for a new A/C unit when something bumped or pushed the guard into a terminal of the line reactor and caused to arc and burn the employee on the arm and side.

Miner was cutting a poly grease line with a knife. The knife slipped and the miner cut his left thumb, resulting in 4 stitches.
Miner was performing maintenance (welding) inside of feed hopper. Miner inside hopper needed a bolt to fill a hole; asked miner on outside of hopper for bolt. Miner on outside of hopper tossed bolt to miner inside hopper; miner inside hopper missed bolt and chipped front tooth in process. Accident is reportable due to follow-up visit at orthodontist office.
Miner was taking a light off light pole, unhooked the brace the light swung around and hit miner in right ear and head.
Miner was washing out plugged feed tube from sand tank to sand screw from discharge end with water hose. When material broke free, hose whipped out of pipe and struck miner in face resulting in five stitches to left eye.

Miner was attempting to tighten the tracks on an 88-B Bucyrus Erie Dragline while utilizing a 3/4 ton come-along. The hook slipped off the pad while under tension and came back to strike the miner in the face, resulting in sutures.
Miner was removing gear reduction assembly from head pulley shaft when it shifted unexpectedly and smashed his right hand between the motor mount and head pulley shaft.

CHANGED OX BOTTLE ON UNIT 36 PICK UP. EE WAS SITTING IN BED OF P.U. DRIVER OF TRUCK TOOK OFF TO PLANT AND BOTTLE FALL OUT RACK AND ONTO EE S RIGHT KNEE. TONS WAS NOT DRAINED PROPLEY.
EE WAS CLIMBING OFF TAIL END OF #1 CONVEYOR AND HIS RING CAUGHT A SKIRT BOARD AND LACERATED HIS LEFT RING FINGER. 22 STITCHES.

EMPLOYEE WAS WORKING ON WASHING SCREEN. IT WAS WINDY AND A PIECE OF RUST BLEW INTO HIS EYE.

EE WAS STANDING ON CONCRETE PIER, OPENING #57 BIN. HE SLIPPED ON SOME ICE AND CAUGHT HIMSELF WITH LEFT ARM AND PULLED MUSCLE IN LEFT SHOULDER. DIDN'T INFORM ME OF HIS INJURY UNTIL 11-29-95.
EE WAS REMOVING DRIVE SHAAFT & DIDN'T SUPPORT WEIGHT OF SHAFT. WHEN HE REMOVED LAST BOLT, DRIVE SHAT DROPPED & SMASHED FINGER.
INJURED WAS AT NOAH END OF PIT PULLING UP BEHINDOUTY BY HAUL TRUCK BEING LOADED. WHEN INJURED TRIED TO STOP HE SAID HIS BRAKING FAILED AND COULDN'T STOP. THE RESULT WAS RAMMING THE REAR OF OUT OF HAUL TRUCK #542 SHOVING RADIATOR BACK INTO ENGINE. DURING TIME OF INVESTIGATION NEITHER DRIVER SAID THEY WERE INJURED.
WORKERS WERE TO DISCONNECT A DEACTIVATED POWER CABLE & CONDUIT OUTSIDE THE POWER HOUSE TO ONE PIECE OF EQUIPMENT BEING REMOVED. WORKERS ENTERED THE POWER HOUSE UNAUTHORIZED TO REMOVE CABLE FRO M INSIDE OF POWER BOX TO THE OUTSIDE OF BUILDING. DURING THIS PROCESS WORKERS CONTACTED INSULATED CABLE CUTTERS WITH A HOT LEAD AND SIDE OF BOX RESULTING IN BURNS.

EE WAS ASKED TO SHOVEL MATERIAL UNDER 2 CONVEYOR WHILE OWNER EE WERE CHANGING SCREENS ON WASHING PLANT HE CAME INTO MY OFFICE AT 730 AM AND SAID HE HURT HIS BACK SHOVELING HE SAID HE WOULD GO HOME AND PUT ICE ON IT I OFFERED TO TAKE HIM TO DR BUT HE DECLINED THE OFFER.
EMPL WAS SERVICING UNIT 542 HE WAS GREASING BRAKE CHAMBER LYING UNDER MACHINE. HE BUMPED HOSE UNDER MACHINE & DIRT GOT INTO LEFT EYE. DR TOOK OUT TINY PIECE OF METAL FROM LEFT EYE.

EMPL CLAIMED HE HURT HIS BACK OPERATING HOIST LEVER IN UNIT #401 STACK TRUCK HE SAID LEVER WAS POSTIONED TOO LOW BETWEEN SEATS IN UNIT AND CAUSED STRAIN ON LOWER BACK TO OPERATE OTHER EMPLS SA Y UNIT IS IN GOOD SHAPE AND PROPERLY DESIGNED
