While using a pipe wrench and attempting to remove a clean out plug on a continuous miner de-mister, knocked a rub rail off striking left shin. EE reported the incident as minor and insignificant 7 days later. EE sought medical attention 11/05/2019 where EE was placed 'off work' till further notice. Additional information has not been provided.

While making a belt splice and preparing to loosen tension on a chain come-along, EE had left index finger pulled into the come-along when the free end of the chain moved unexpectedly. The pinch caused a laceration to the left index finger that required sutures to close.

While making a belt splice, the splice board was sliding off the platform. As EE reached to push it back onto the platform, EE's partner inadvertently struck EE on the left ring finger tip with a splicing hammer. The impact caused a fracture and lacerations that required sutures to close. Released to return to work and reported for next scheduled shift.

While cleaning a belt drive with a rake, turned and struck left elbow against a steel I-beam. EE continued working and finished the shift. Diagnosed as a bone chip type fracture along with a ruptured bursa sack in left elbow. Limited to a "No use of left upper extremity" restriction.

While attempting to loosen a bolt on a ram car speed reducer using a ratchet and a cheater bar had his left middle and ring finger tips caught between the ratchet and frame when the wrench slipped. The pinch caused a laceration that required sutures and a fracture to the tuft of the middle finger. He was treated and returned to work to his assigned duty.

While changing miner bits struck the bit wrench with a hammer and a piece of metal 1/8" in diameter pierced his glove and became lodged in the palm of his left hand. The metal was removed by a physician and the wound closed with sutures. He was initially restricted from returning to full duty to keep the area clean and prevent infection.

While using a utility knife right-handed slipped and cut the back of his left thumb lengthwise. The laceration also damaged the tendon and required sutures to close. Precautions against infection prevented him from returning to work underground for at least 7 days.

While tightening bolts the injured was holding a wrench while a co-worker was tightening them with an 1/2" impact. The wrench spun in his left hand striking the back of the hand and leaving a laceration that required sutures to close. He was treated and released to unrestricted duty reporting to work his next scheduled shift.

While changing ram batteries, a cylinder lock would not open. He tried hitting the hinge with a hammer. As soon as the hammer struck, the lock sprang open, striking his right thumb. X-rays revealed a non displaced fracture, he was splinted and returned to work without restriction.

While changing miner bits with a hammer a piece of the hammer broke off striking his left palm near the base of the thumb. The fragment remained in the wound. He was transported to Springfield Memorial ER where the fragment was removed. After a follow up visit to his physician 8/23/2010 he returned to unrestricted duty.

While cutting open a damaged repair on a jumper cable, his knife slipped and cut into the top of his right hand between the knuckles of the index and middle finger. Transported by private vehicle to St. Johns Hospital in Springfield where the wound was sutured and he was released to unrestricted duty.

While trimming out a small piece of mine belt the employee cut through the belt easier than expected lacerating his right index finger with a utility knife. The wound required sutures to close.

Employee was using rachet to tighten rope clamp for belt extension when he felt a pop in right wrist. Pain was only momentary so he never reported accident before leaving for the week. The next day he stated that his wrist was swollen and causing discomfort so he went to doctor who diagnosed strain to wrist and placed him on restricted duty for 4 days.

Injured employee was changing bits on a cont miner. He removed the glove from his right hand to get a better grip on his hammer. When he struck the removal tool a piece of steel "shot" off the removal tool and struck him in his right hand, causing the steel to become embedded between his knuckles.

EE WAS TRIMMING CABLE SO IT WOULD FIT IN PLUG WHEN KNIFE SLIPPED AND CUT THROUGH SHIRT LACERATING FOREARM. SUTURES WERE REQUIRED TO CLOSE WOUND. RELEASED TO WORK WITH NO RESTRICTIONS.

EE was using a short bolter wrench to beat broken drill steel out of drill pot and caught finger between pot and wrench. First-aid was rendered and he continued work. The following day swelling persisted and he sought medical treatment. Diagnosed with chip fracture to right index finger. He was released to work underground. No lost days or restricted duty.
