While helping to hang CM cable and waterline, employee felt a pull and strain in right shoulder. Employee continued to work until Dr. pulled them for surgery on 06/21/2024.

Employee was welding on a chute at the stoker loadout. Employee was in a basket attached to a CAT telehandler. The basket was raised six feet off the ground when it unattached from the telehandler falling to the ground. Employee sustained a fracture to the left tibia.

Employee changed oxygen tank at a permanent station. Attached hose to tank proceeded to open valve on oxygen tank. The regulator failed and blew apart the hose attached to the tank came off as well. Employee was struck by debris from the regulator. Laceration to the left cheek and left orbital fracture.

Employee was setting timber when they bent over to pick up wedges a timber fell striking them on the head above the left ear.

Alleged repetitive trauma to both arms and hands, carpal tunnel.

Carried coats to plant, set them on a belt crossover. Turned to leave area when right foot caught a small pile of coal fines. Employee fell to the ground (same level) onto their right hip and elbow.

Employee approached and entered equipment doors at the slope bottom to exit the mine. Employee closed one door and turned to find the other door near them closing. Employee reached out to catch the door. Another employee was already inside the air-lock opened the out-by set of doors to exit the mine.

EE was unrolling felt liner and picked one end up to go over the dirt pile. EE felt a twinge in left shoulder. EE returned to work but did seek treatment on 01/07/2023. The x-ray showed no fractures or significant findings. Noted and EE returned without restrictions.

Individual was cleaning and raking along MN4 beltline XC96 when they tripped on wire rope falling to the ground. Put their arm out to catch themselves causing a fracture to the Greater Tuberosity.

Helping change ram car batteries. The safety chains were tight, so the car pushed the battery into the rib to take tension off the chains. I.E. reached in to loosen the chains when they became tight again pinching their right index finger between the battery and the safety chain. Caused a fracture and laceration that required stitches to the I.E's right index finger.

Employee was helping another start the face scoop. Employee sat down in the cab and leaned forward to pull on the breaker handle when employee felt a pain in lower back. Through company investigation, information received January 6th, 2023 it was determined this incident need be reported.

A roof fall occurred at South Submain #1, Entry #4 in the intersection of crosscut XC #20. There was no injury or property damage with this occurrence.

2LMN Unit Intersection 10+50 1R XC Actively bolting a brow on a blow through that was 4 to 5 feet in length. The brow broke up causing multiple pieces of shale to fall. Employee was inserting pin into previously drilled hole when employee was struck on the back and legs knocking employee to the ground.

A roof fall occurred at NSM XC44 Entry 5. There was no injury, a ram car that was out of service was partially covered with material from the fall.

A roof fall occurred 1RMS #2 Beltline, XC 16 Entry 7. There was no injury or property damage with this occurrence.

A roof fall occurred 1RMS # 1 belt XC 38 entry 1. Entry had a clay vein running along left rib line joined by other clay veins in the intersection. There was no injury or property damage with this occurrence

A roof fall occurred 1RMS XC 31-1/2 Entry 1. Entry had a significant clay vein running with the entry. There was no injury or property damage with this occurrence.

A roof fall occurred along 2L #2 beltline XC16 Entry #4. Entry had clay veins running with the entry. Additional clay vein in the crosscut joined the first in the intersection. There was no injury or property damage with this occurrence.

A roof fall occurred along 1R #2 beltline XC #9 Entry #2. Entry had a clay vein running along right side rib line through the intersection. A second clay vein entered the intersection from the left rib joining the first forming a cantilever in the intersection. Rubble from the fall compromised a Kennedy stopping. There was no injury or property damage with this occurrence.

While removing top structure from overland belt the employee dropped the structure pinching right thumb between the structure and structure railing. This resulted in a fracture of the right hand.

An established intersection with no limestone present and bolted with fully grouted 8' bolts, had a clay vein measuring from 5' to 10' thick through the middle of the intersection. The roof failed along the clay vein. Initially discovered at 2:03 pm and verified by management at 2:37 pm.

While helping build a solid block wall bumped EE's left elbow. There was no visible wound, swelling or discoloration at the time. Reported swelling and inflammation 4/29/2021 and sought medical treatment for cellulitis. Began missing work after Diagnosis of and treatment for infection due to Septic Bursitis 4/30/2021.

While reaching over a ram car to tighten a battery plug, felt a pop below EE's left calf muscle and EE's leg began cramping. Went to Midwest Occupational Health Associates the following morning and was released to unrestricted duty. Became Lost Time after hospitalization 3/17/2021 with pulmonary edema due to a blood clot from the strained area.

While picking a piece of 1/4" steel plate measuring 17" X 25", and taking a couple steps, felt intense pain in EE's right upper arm. X-rays revealed that EE's right shoulder was dislocated. The shoulder was put back in place and EE's arm placed in a sling. EE began missing work Thursday, July 23, 2020.

While walking near a co-worker setting 6' props, was struck on top the right foot when a prop fell unexpectedly. The impact caused fractures to 2 Tarsal bones in EE's right foot. Treated and released to restricted duty.

While getting out of a tractor, hit head against the top in an area of low height. EE stumbled in the cab and fell, twisting left leg and felt a pop in left knee. EE continued working. A follow-up MRI revealed damage to the left knee that required surgery. EE began missing work when the procedure was performed 5/11/2020.

A roof fall occurred at XC#75 MS#13 belt line. The belt had been removed and was under planned rehabilitation. Personnel had withdrawn from the work site. There was no interruption to production. There was no injury or property damage with this occurrence. The area has been cleaned and bolted according to plan.

A roof fall occurred along MW#1 belt, XC #31/station 35+45, entry #0. Limestone had 2 slips that contained quartz, 1 in the middle and 1 larger slip in the heavy side of the cantilever. There was no injury or property damage with this occurrence. The fall was approximately length 18' X height 11'.

While roof bolting, EE's drill steel became clogged. As EE reached to remove the steel, shale fell from the roof hitting EE's hand and driving EE's thumb into the drill pot. The impact caused a contusion to EE's right thumbnail. After shift EE went to an urgent care facility where X-rays revealed a fracture to the tip of EE's thumb.

While using a pipe wrench and attempting to remove a clean out plug on a continuous miner de-mister, knocked a rub rail off striking left shin. EE reported the incident as minor and insignificant 7 days later. EE sought medical attention 11/05/2019 where EE was placed 'off work' till further notice. Additional information has not been provided.

A roof fall was found at 08:00 AM in a 3 way intersection of entry #5 MS10 XC14 station #22+84.95 intake travelway. No injury or property damage occurred, travelway is being re-routed and access was propped off. Fall was approximately 30'x20'x10' area was mined in Oct. 2002.

While moving miner cable, felt a pinch in lower back. They went to pick up cable again and felt additional pain in back and into legs. They called for help to get out of the mine. They returned to work without restriction. They contacted a physician after allegedly aggravating the injury while sweeping with a broom and began missing work 10/25/2019.

While making a belt splice and preparing to loosen tension on a chain come-along, EE had left index finger pulled into the come-along when the free end of the chain moved unexpectedly. The pinch caused a laceration to the left index finger that required sutures to close.

While making a belt splice, the splice board was sliding off the platform. As EE reached to push it back onto the platform, EE's partner inadvertently struck EE on the left ring finger tip with a splicing hammer. The impact caused a fracture and lacerations that required sutures to close. Released to return to work and reported for next scheduled shift.

A roof fall occurred along MW#4 Belt, XC19, Entry#4, Sta 135+47. The fall is approximately 40' Long, 50' Wide and 20' Thick. It is dangered out and 2 rows of breaker props have been installed on all accessible sides.

A roof fall occurred along MW#4 Belt, XC6, Entry#4, Sta 116+66.88. The fall is approximately 30' Wide X 40' Long X 15' High. It is dangered out and 2 rows of props have been installed on all accessible sides.

While walking ran EE's head into a low brow. EE experienced pain in EE's neck, right shoulder and right arm. EE sought medical treatment 12/6/2018 and was diagnosed with cervical disc disorder and released to perform all job functions and work with caution. Began missing work 7/22/2020 after C4-5 cervical disc replacement surgery.

While installing a 6' bolt and holding the bolt as the pot was raising, the bolt bent, pinching the right thumb at the base of the nail. The pinch caused a laceration of the nailbed and fractures to the finger. Employee was treated and released to restricted duty, no use of right hand.

While roof bolting, reached to remove a plugged drill steel from the roof. As they reached, shale and rock fell around the steel, knocking the injured to the ground. The rock and drill steel struck their right calf and left a laceration that required sutures to close.

At the completion of a job, EE's left arm got caught by a belt roller on the plant feed belt causing abrasions. EE attempted to knock belt skirting down against the belt with a hammer and missed, allowing coat sleeve to be caught between the top roller and the belt. Transported to Springfield Memorial Medical ER for treatment where EE was admitted for observation.

Our employee is a truck driver. Had just finished dumping coal, went to walk back to push in tailgate valve to relock the tailgate, when employee slipped in mud and fell on left side. Driver did not wish to seek medical attention immediately. Thought had just bruised side.

While roof bolting and removing a steel from the roof, employee was lining the steel up with the drill pot when a piece of shale struck employee's hand mashing it against the pot. The impact caused a fracture and laceration to the right index finger.

While preparing to start the roof bolting machine, was struck on the head and left cheek by falling shale. The shale, approximately the size of a dinner platter, struck the edge of the canopy as well, allowing the sharp material to bounce into the operator's left cheek and leaving a laceration that required sutures to close. EE was treated and released to unrestricted duty.

While standing on the roof bolter and reaching to connect curtain to an S hook, had a sharp pain in lower back and down both legs causing body to jerk. Had experienced similar pain for 2 weeks and sought chiropractic treatment along with the use of an inversion table. Diagnosed as sciatic inflammation, work related illness.

While roof bolting, EE helped move a piece of shale that had struck EE's partner. The shale then flipped over while moving and struck EE on the right ankle. Initially reported as a first aid only 10/09/2017, EE began missing work after an MRI detected an osteochondral defect ankle sprain and corrective surgery was performed 01/29/2018.

While roof bolting in old works, a piece of shale fell and struck employee on the head. Employee felt a sharp pain described as a "Stinger" in the neck and shoulder. Employee did not seek medical attention until the following day. Initially reported as a first aid only on 10/2/2017, became a lost time after surgery to repair herniated disks 2/12/2018.

While inspecting a work area and trying to pull a rib down, was struck on top of the right foot by the rib when EE couldn't move away fast enough. The material landed on top of EE's tarsal guard resulting in a closed fracture to a tarsal bone. Restricted to sitting work only until EE's next evaluation.

While cleaning a belt drive with a rake, turned and struck left elbow against a steel I-beam. EE continued working and finished the shift. Diagnosed as a bone chip type fracture along with a ruptured bursa sack in left elbow. Limited to a "No use of left upper extremity" restriction.

A non injury roof fall was found at 7:00 AM on 8/8/17 at XC 4 entry 3 2rMS #1 belt. The fall was in a four way intersection that had been mined December 2014 and supported with 8' resin bolts. The fall will be cleaned up upon approval of clean-up plan.

The 20 person elevator at Williamsville intake shaft became inoperable due to wiring issue, all underground personnel were brought out of mine until car was back in service at 2:50 PM
