On 10/1/2014 the assoicate states that he was removing a rubber rope seal with channel locks, on number 3 filter pan. When he pulled upward on the rubber rope seal he felt pain in his right shoulder. Restrictions were placed on the associate on 10/22/2014
Associate was performing a caustic switch. He was using a valve wrench to open caustic valve on #8 press when the wrench slipped jamming his left pinky finger between the floor and valve wrench handle.
While working in switch gear to restore power associate was using a hammer and chisel to remove insulation and the hammer bounced back hitting him in the tooth. The hammer chipped associates tooth.
EE was installing a Clarkson on the slacker deck in section III when the hammer that he was swinging hit a wedge, struck a pipe and then hit his left hand at the index finger. EE was asked if he needed medical treatment and he stated that he did not.
Employee was installing a blind on the classifier sand shoot. The blind fits tight and required the use of a maul to install. When employee finished task he felt a sharp pain in his lower back. He was placed on restricted duty on 10/04/13
Realigning Y8 coolers. While opening a water valve, the wrench slipped off of the valve handle on the gate valve and he smashed his hand on the gate valve next to the one he was opening. He fractured his middle & ring finger on his right hand. He was treated with cold compresses and buddy tape. He was given a prescription for pain.
Employee was scaling when a piece of scale smashed his ring finger on his right hand. Finger was bruised and swollen. Diagnosed with tuft fracture.
EE was swinging a maul trying to close a valve. He experienced pain in his right shoulder. Pain increased. He was diagnosed with Multi directional Instability. His work duties were restricted on 5/14/12. Noranda could not accommodate these restrictions, therefore, as of 5/15/12 is now a Lost Time Accident.
EE was using a crescent wrench to move the tail stock on the Poreaba lathe to begin machining a valve stem. While applying force to the wrench, the wrench slipped and EE felt a sharp pain in his back. Work duties were restricted. Occupational injury not determined until 6/19/12.
While attempting to replace B7 recycle suction valve pump, ee struck beater bar with an 8# maul. The beater bar spun and struck him on his left index finger cutting the glove & lacerating his index finger. He received 6 stitches. His work duties are restricted.
Employee was manually using a 16# maul to clean scale from the bottom of a tank. Employee later complained of pain in mid to lower back with pain running down his legs and making his legs feel weak. He is diagnosed with lumbar strain and is restricted in his work duties.
EE stated that his shoulder was sore from swinging a maul. He experienced pain in left shoulder & under arm pit. He was evaluated by Noranda medical personnel and it was determined that he be restricted from using a maul with left arm and no overhead lifting with left arm.
EE was pulling a 24" pipe wrench to replace a 2" water valve on #2 filter press and felt a pain in his back. His work duties are now restricted.
EE was building concrete forms when he hit his right little finger with a hammer causing a fracture. He was prescribed oral antibiotics which makes this incident recordable.
EE was cutting the cloth off one of the leaves for #1 press when the knife he was using slipped, cutting him on the right forearm.
EE was rolling blinds in Evap. He picked up his impact wrench and as he did so, and felt a pain in his left shoulder. On May 7, 2010, ee was given the following restrictions: No overhead work, limit lift to 50lbs with left hand.
EE was opening a valve with a maul. He smashed the pinky finger of his right hand between the maul and the valve handle. His finger was slightly swollen. On March 9, 2010, EE saw an orthopedist and it was determined that his finger was fractured. He did not miss any days of work and he was not restricted.
Installing pin in the bucket of a front end loader using a 10# hammer. When he applied force, he felt pain in his right shoulder causing him to drop the hammer. He was then placed on restrictive duty pending MRI. Results of MRI revealed arthritis. GA could no longer accomodate restrictions because of the indefinite nature of his condition. Last day worked was 6/8/09.
EE reported that he hurt his left thumb & wasn't sure when the injury occurred. Earlier, he was reaming out the stab-in to the north wash header using a manual reamer and a maul. He stated that he didn't feel any pain while performing the job. Upon completion of the task, he felt pain in his left thumb. Was sent to medical for evaluation. Restricted work duty.
Employee was working at the cold end of calcination. While using a maul to open a valve, he smashed his right index finger between the maul & the breaker bar. Finger was x-rayed and found to be fractured. He is scheduled to see an Ortho hand Specialist on Friday, October 3, 2008.
While preparing to take AG-5 out for a turnaround, EE was reaming out AG-5 suction with a maul & reamer. As he hit the reamer with the maul, a piece of metal chip flew off and struck him in the left thigh, where it lodged. The metal was not visible but he had a 1" laceration.
EE was attempting to open cold condensate valve at #1 Power Boiler to add condensate to steam drum when valve broke loose; his small finger on left hand got smashed between valve wrench and a pipe, causing a fracture & laceration to his finger. Required surgery.
Employee was cutting a tree branch with a pocket knife, when the knife slipped, and cut his left thumb. The laceration required seven stitches to close.
An operator was securing a wire nut to a one inch cable using a socket wrench. The wrench slipped off the nut causing him to smash his finger between the wrech handle and a pad eye. Nurse in GA's medical department cleaned wound and applied ointment and gauze. X-rays revealed tuft fracture to right ring finger.
Using a mall to break open a valve, employee felt pain in his right shoulder/upper back and was treated with first aid measures. The employee's injury failed to improve with first aid treatment. Upon further medical evaluation, it was determined the employee experienced a torn rotator cuff that required surgical repair.
Working under #3 filter cutting metal support brackets, EE was in the process of chipping slag off of beams, when a piece of foreign material flew in his left eye. Medical treatment was required.
A mud rig had been installed on tank 68 to drain caustic to the west unit sump. While taking the mud-rig off the bottom of tank 68, his hand slipped off the wrench and hit the 2" ball valve on the bottom of the tank. Caustic was released, splashing under his face shield onto his face. He was wearing mono-goggles and a face shield at the time of the incident.
Employee was descaling the 6 to breaker hopper. He had a #2 gun bit in his hand scraping off the side of the hopper. When he was pushing the scale down, he jammed his right thumb between the shaft of the bit and side of the hopper. He was taken to medical and from there, sent to get x-ray. Fracture of finger was confirmed.
Employee was performing a routine task of putting Tank #69 back in service. He was attempting to knock out the short entry nozzle to inspect it. He was in the process of swinging the maul to unscrew the locking collar. He jammed his left ring finger between the maul handle and the locking collar. Diagnosed with contusion & tuft fracture of finger.
Employee was using hand tools(wrench) on Tuesday, 3/21/06, when he injured a tendon in his right thumb. It was determined that the employee had lacerated his right thumb at home on 3/19/06, but did not properly treat or report this off the job injury. On Tuesday, this injury was aggravated by pulling on a wrench, requiring medical treatment and restricted use of right hand.
EE was attempting to tighten packing on an angle valve with a pair of channellock pliers. The packing stud nuts were tight & when he exerted more force, he felt a "pop" in his middle left side of his right hand palm & his little finger on his right hand. He continued to work his regular job, but still feels the "popping" sensation in his right hand.
Employee was cleaning his fingernails with a knife, when the knife slipped, stabbing him between the thumb and index finger of bis left hand. He was taken to medical, then to the doctor where he received seven stiches to close laceration. Employee returned to full duty following treatment.
Employee was attempting to place west weigh belt on the roller using a 2 x 4. When prying up on the belt, the 2 x 4 raised up, causing employee's finger to be pinched between 2 x 4 and metal frame. Laceration to left middle finger from front of nail to tip of finger. Received several stiches.
Employee was reaming out 2" stabbin on tank 65. Said it felt as if something in his arm twisted. Complained of numbness and burning. Brought to first aid, was given pain medication and ice pack. One hour later, didn't feel any better. Transported to St. James Hospital. Dr. Fraiche restricted his work on 4/4/05. Right elbow contusion.
Employee was greasing the southwest pony on 101 Gantry Crane when the seal between the grease gun and the button head fitting leaked and sprayed grease on the left side of his face. Some of the grease got in his left eye. He also had pain in his right thumb later, resulting in outpatient surgery. He was released with restrictions on 4/5/05.
While swinging a maul, right indes finger got caught betwen handle and piece of iron. Employee was provided medical treatment for his contusion.
