Gramercy Operation Metal/Non-Metal

Employee was performing housekeeping activities at the alumina tail pulley. These activities require the employee to pull a hose from the north side of the tail pulley to the south side. During this process the employee felt a pain in the middle of their back.

Employee was opening a manual valve on the bottom of the density tank in section 3/ calcination #5 filter when EE felt a pain in EE's back.

Employee was working on a diesel pump (west side alley by #1 settler) and tripped on a piece of two by four (wood) while walking. Which caused them to fall.

After employee removed a 90 (pipe) on east surge basin pump. Wasps flew out and stung employee on EE's right hand.

At approximately 5:20am, while employee was conducting routine cleaning of spigots; a spigot came loose. This resulted in caustic contacting the right side of the employee body (right eye, right arm, right chest, right lower side of back, and right buttock).

Employee was adjusting the cylinder head on a Clarkson valve on #1 secondary in precipitation. The weight of the cylinder head shifted and smashed EE's left index finger.

While doing normal operations by the south high pressure back pressure to bring up the plant, employee had a hole in their left boot causing caustic to get inside. Their left great toe/foot started to hurt.

The employee was assisting a co-worker in positioning the track mobile onto the tracks in alumina shipping. While EE's hands were resting on the top rail, the track mobile slipped off the tracks, causing EE's hands to drop onto the bottom handrail and resulting in an injury to this right wrist.

Operator was attempting to unplug the west silo (the south leg) with a hooked water hose, as employee was moving the water hose in and out of the opening, they started feeling pain in Left Lower Side of Back.

Employee was moving around buckets of bolts by 9 and 10 heaters in digestion. They felt some discomfort in left lower side of back after they moved bolts.

An operator was in the process of completing a check on top of the bag house at the Hot End in calcination, when the employee fell and hit EE's left thumb.

Operator reported that operator's right shoulder was hurting from beating on tanks tonight and operator couldn't lift arm.

Operator was opening a 2-inch valve to catch sample when EE swung the mull and miss the valve handle and hit EE's right little (5th) finger.

The evaporator operator was moving an air tool when they felt a strain in their back.

Employee was heating up a keyway to remove from a valve bushing. In the process of doing so the key popped and hit the employee in their right forearm.

Security Officer was at the main gate conducting a routine vehicular inspection. During the process the employee was struck by 18-wheeler truck causing injuries to the right leg.

OPERATOR WAS CLOSING SCREEN BOX WHEN TANK 104 OVERFLOWED AND MADE CONTACT WITH THE OPERATOR.

While reassembling the centrifuge at #3 steam turbine the operator smashed EE's Left Pinky Finger.

Employee was exposed to a caustic leak on the Mez deck in precipitation.

Employee reported to the supervisor on 02/16/2025 that EE was in section 4 shipping unloading rail cars on 02/14/2025 when EE slipped and fell on liquid product on the ground while EE was walking towards the hook of the cable during the pulling up of the next rail car. Employee stated that EE sustained injury to left side and leg.

Operator was changing cloth on the # 3 filter and struck their L Index Finger with the hammer.

Operator was attempting to fix leaking packing on #7 washer under flow pump by loosening the nuts on the packing gland. EE got sprayed in EE's face with mud and liquor. This incident occurred in Clarification.

Employee was cutting rope for filter cloth and accidentally cut Left 4th finger.

Employee was breaking off some rail cars and as EE was walking, EE's foot shifted in EE's boot. EE continued to work and making the switch. After going home and taking their sock off, EE saw that their foot was swollen and tight after about two hours.

Employee caught left heel under a wheelbarrow while taking scale to dump into T4.

Employee stated that EE walked by a pump and felt something spray EE's boots. EE didn't notice immediate signs of caustic exposure, EE proceeded to EE's tasks. Employee later began feeling an irritation where EE's boots touch EE's legs. Sent to medical for evaluation.

Employee was closing the door off the fire extinguisher and sustained an insect bite to right arm.

While scaling, a piece of scale fell overhead and punctured through EE's right glove.

Employee was in the process of washing down in the mud thickener area utilizing a water hose, and attempted to pull more slack out of the hose to wash a further distance away, and in doing so lost EE's footing and started to fall forward to the ground, in an attempt to not fall face down EE changed EE's positioning of EE's body while falling and landed sideways on EE's left ankle.

WHILE CLEANING THE SCREEN BOX, LIQUOR MIXTURE (CAUSTIC) GOT INSIDE MY CHEMICAL SUIT AND THEN INTO MY BOOTS. MY RIGHT LEG STARTED TO BURN.

PUMP WAS WASHING, OPERATOR WAS WALKING BY WHEN GASKET BLEW.

EE WAS INSTALLING MOTOR WHEN IT SLIPPED OUT EE'S HAND AND SMASHED EE'S RIGHT THUMB AGAINST THE HUMMER SCREEN.

Employee reported the incident on 06/13/2024 to EE's trainer, stating that while EE was training in batch precipitation on 06/12/2024, from 7:00am until 3:00pm after arriving at home later that night EE noticed irritation on EE's right ring finger.

Employee was removing bolts to install skirt on the DLA hopper when the wrench employee was using was caught by the conveyor and pinched the right index finger between the wrench and a hopper bracket.

Employee was adjusting 1B filter cloth when EE's hand slipped off the cloth causing EE to fall backwards into the handrails, resulting EE to hit EE's back against the railing.

Employee was performing a job task at the filter acid tank on 05/02/24 and did not report injury until 05/04/24. Employee stated that employee's left finger was swollen.

Employee jammed right thumb while catching sample from press feed.

Employee complains of pain to right wrist after changing air filters on #4GT.

Operator was attempting to close A7 lowers valve to fill tank with liquor for storage. In the process on doing so, the weld broke on bushing of valve free spinning beater contacting worker on right hand cutting their index finger through the glove.

Employee pulled a muscle (right elbow) assisting with lifting a small pump onto a cart.

Employee reported during the 5pm shift huddle that they have blisters to their feet(bi-lateral) due to extreme walking during the past week.

Employee was closing the discharge valve on the H1 East Flash pump. After closing the valve, EE pulled the eliminator off the pinon gear. The air hose disconnected and hit EE above EE's left eye.

Maintenance personnel were removing a valve in the HpBp area near v5. When removing the valve, a restriction that was in the upper side of the pipeline gave way causing a fan of hot material to spread out contacting the employees.

Employee carried bearing to CV3 head pulley. After dropping bearing off all was good. EE reported to the supervisor that EE ended up with the middle of EE's back hurting as a result of the previous day's task.

Employee was using 20k hydro-blast flex lance to clean inside of 6 inch pipe. The employee pulled the lance out of the pipe under pressure, allowing the tip of the lance to come back and strike employee in the right forearm, causing a laceration to the arm.

Operator was pulling on 1B cloth to realign on roller. While pulling on cloth, operator slipped backwards, their hard hat and goggles fell off. When the cloth picked up the "cake" again, it then splashed in the liquor sending caustic on their face and eye.

EE WAS WASHING DOWN AT THE WETCAKE LOADOUT AREA. EE'S HOSE GOT STUCK AND EE PULLED IT TO FREE IT, WHEN EE DID, EE FELT SOME DISCOMFORT IN RIGHT LOWER BACK. EE ATTEMPTED TO WORK THROUGH IT BUT THE PAIN BECAME WORST

Employee was descending the stairs on the cold end of calcination when they stepped onto the ground where the concreate was cracked and twisted their ankle.

EMPLOYEE WAS DOING HOUSEKEEPING AFTER JOB WAS COMPLETE AND EE WAS MOVING A VALVE USING THE ELECTRIC HOIST TO REMOVE THE BEARING MOUNT ASSEMBLY. WHILE USING THE PULL CHAIN TO MOVE THE HOIST TOWARDS THE DOOR, THE HOOK SLIPPED CAUSING THE TOP OF THE MOUNT TO FALL TO GROUNG AND STRIKING THE LEFT FOOT.

Operator was working on A2 PAP, swinging a maul when operator felt a sharp pain in back.

Employee was mopping the floor when EE slipped and fell. Incident was originally classified as a first aid. On 9/14/2023 worker went for a follow up visit to GCOM after complaining of pain and was placed on off duty until next follow up appointment.

While employee was putting together a SEN for tk 76 to be put back in service, worker was tightening fittings and pipe wrench slipped striking EE's left ring finger, immediately felt pain.

While trying to move a large suction hose by #5 filter, employee felt a burning pain in left shoulder. EE immediately reported into the ERT and was taken offsite to GCOM for assessment. The incident was classified as a first aid injury. The following day the employee stated that the pain had intensified. EE was further assessed and placed on restricted duty.

While descending step ladder with a steam hose, employee felt a strain in their back.

An employee working in the Warehouse reported to the supervisor that was feeling ill. Employee experienced heat related illness.

The Employee was hosing down around the #4 Settler Underflow pump when EE was contacted with mud on the top of EE's nose.

Employee was descaling 2/3 Hopper when a piece of scale fell and hit the gun. The gun in turn struck the employee in the face area

Employee was attempting to open tk 67DOV with a sledgehammer when they felt discomfort in their wrist

Employee was walking inside 0 washer replacing rake arm cables and accidentally stepped in hydraulic fluid that was on the floor and which had leaked from a bobcat. In doing so EE's left foot slipped forward causing EE to fall.

Employee was descaling the #9 press when spicket broke off and EE was contacted with caustic on EE's face and body.

Operator was using a maul to open the manual beater on tk35 DOVw when they felt a pop in right elbow and pain.

Employee was going down the stairs by Kiln #3 when EE slipped, sustaining a leg injury.

Employee was opening the gate on the pan feeder. The gate was jammed. Employee forcefully pulled down to release the gate. Upon attempting to free the gate, employee smashed left pinky finger between the valve wrench handle and the panfeeder frame.

Employee was lifting chain wheel to place on valve when EE felt a pull in EE's lower left side of EE's back

Employees were in the process of draining the South Wash Liquor Tank. Upon turning on the air on the tank, the material (caustic and ore) came out of the top of the workers.

Employee was getting off work in Alumina Shipping. After employee clocked out, employee was walking down the tracks near railcars that were being unloaded and when crossing over tracks when employee slipped and fell. Medical determined that employee had 2 broken ribs.

Maintenance GR personnel was on top of Tray # 3 assisting with the disconnect of torque mechanism. EE began removing the bolts that secure the torque mechanism. Upon the removal of the last bolt, the torque arm suddenly released and pushed the torque mechanism and mounting plate. The force of the release caused mounting plate to strike employee in the leg.

Employee smashed their left index finger with hammer/maul.

Supervisor caught index finger between valve and steel structure while rolling it over so that it could be hydro-blasted. Resulted in fractured index finger tip.

OPERATOR WAS IN THE PROCESS OF CHECKING SEN. WHILE USING MAUL TO MAKE ADJUSTMENT, EE MISSED TARGET AND SMASHED RIGHT INDEX FINGER ON THE LADDER STAND.

EMPLOYEE WAS WORKING ON THE CANYON SUMP TO PUMP WHEN EE WAS SPRAYED WITH A WATER LIQUOR MIX FROM SUMP CAUSING BURN TO EE'S LEFT ARM, LEFT CHEST AND NECK.

Employee was tasked with performing routine housekeeping duties to wash down the area underneath the filter press. As the employee completed the wash down of the south side, EE laid down the hose and started walking around the press to the north side. The employee walked approximately 15ft and lost their footing. This resulted in multiple fractures to the left ankle.

Employee was walking down 5 filter stairs when EE stepped down awkwardly causing a shock up EE's leg. The employee then stated EE had numbness in right leg to EE's foot.

Employee was welding a bracket on a 24 inch valve when eyes was exposed to a welding flash due to the failure of auto lens.

Employee experienced a heat stress related incident while working in the department. Employee began to feel hot and weak. Employee was taken to the medical department and then transported offiste via ambulance for further assessment. Incident was reported to MSHA Case number 03486624

While getting ready to leave work when EE stated that EE had a hole in EE's boot and noticed a caustic burn on the bottom of EE's left foot.

While cleaning under tank 38, employee picked up large chemical hose to move out of the way and felt a slight pain in lower back area. It did not bother them at the time but did intensify during the night. when they came to work the following day they immediately reported the incident and was taken offsite for assessment.

Employee was unplugging spout with a maul and while swinging the maul EE's left hand contacted a support beam that was nearby. Upon further inspection after removing glove EE noticed a laceration on left pinky finger.

While shearing a small piece of 1/8" metal plate, the employee got 1 finger on each hand smashed = (2 fingers). EE was taken to Medical for evaluation and then off Site for further evaluation.

Employee was scaling inside #2 tray when employee felt a burning sensation on right index finger.

While placing gratings back in the East Hopper after descaling, EE's right ring finger got caught between hopper and the grating which caused a deep cut.

Operator cleaning/back flushing washer upon completion they came to supervision and stated that they had pain in arm. Employee sent off site; evaluated and referred to ER for suspected dislocation. Arm was not dislocated; employee fully released. Employee complained of same pain on 07/21/2022 and went under the care of a specialist and is now restricted.

Employee became overheated while working.

While working in the Clarification dept, the employee reported that EE was not feeling well. EE was taken to the medical dept where EE was assessed and taken offsite for further treatment. The employee was experiencing heat stress related illnesses.

Employee was replacing the short entry nozzle on tank #41, while taking the nozzle out, liquor came out the line. Employee was contacted on EE's body.

Employee was picking up and placing derailer in back of buggy. Felt pain in back.

Host client failure to properly label steam lines. Employee hooked up a hose to wash down and hooked to a steam line.

While removing a hose from a steam rig using a maul, operator's hand struck the bolt that sticks up on the coupler for the Boss fitting on the steam hose. Operator felt discomfort and reported the incident to supervision.

Operator was preparing to leave for the day when EE took off EE's right boot EE notices a small burn on EE's right lower Leg.

After closing the screen box on zero cone pump, employee set the maul down when EE felt a sharp pain in EE's back.

Operator was working in the area when operator bent over to pick up maul. A beam broke free and contacted the employee on shoulder.

Operator was using the swivel vice at the control room lean-to putting together a short entry nozzle. While tightening the 90 piping it slipped and hit EE in the ear, resulting in a small cut.

Employee was closing the V2 1st from the North PRV drain. As employee came down the ladder and stepped onto the platform, twisting left ankle.

Employee engaged a 9-ton come along when a welded lug on the press shell gave way. The detached parts of the lug struck the employee in the chin/neck area resulting in a laceration.

Upon stepping over a ditch, employee felt a sharp pain in the back side of right foot (bottom of heel)

Employee was removing guard and got right hand pinky finger smashed. While pulling belt guard employee's finger became pinched between the guard and the jack bolt for the motor

Upon trying to take the water off the slurry injection pumps employee slipped and tried to prevent EE's fall with EE's left leg and filled boot with caustic

Employee was setting up an eliminator on a valve on the south primary feed tank suction and the eliminator fell off the valve smashing employee's finger.

Miner was unhooking and moving a hose when the miner sustained a caustic/chemical burn on leg/thigh. Follow up appointment was held today and surgical intervention was recommended by the burn unit.

Employee was working in Batch when EE's lower leg was splashed with caustic. Left untreated, has developed into a severe burn requiring significant treatment.

MINER WAS USING A PIPE WRENCH TO OPEN A VALVE WHEN THEY SMASHED RIGHT RING FINGER BETWEEN THE WRENCH AND VALVE CAUSING A CONTUSION & FRACTURE TO THAT DIGIT.

Plugged flange line broke loose exposing miner to caustic liquor. Chemical burn to multiple body parts as result of incident.

Using a pry bar inserted into the notched holes of the barrel of a capstan fitting to open a gate. Employee applied force with the pry bar it bent and rotated causing the employee to lose their balance and fall onto the lime loading grating. During the fall EE's right arm braced the fall. EE's arm struck the grating and slid into the opening causing a fracture to the right ulnar.

This incident was just reported to the Safety/HR team as a result of an internal investigation which was held on 11/4/2021. 2 Employees/Miners were involved in "horseplay" inside of a designated break area. 1 miner was shoved, fell back and injured a knee.

Employee was closing valve when EE leaned over steam line. As EE stepped back and placed EE's right hand on the steam line for balance receiving a thermal burn.

At 5:10am EE was walking to observe A- Train Product to check to see if it was in service when EE tripped and fell on curbing hurting right shoulder.

Employee slipped while descending a ladder hitting ground with left foot, toes down. While walking away employee realized that the left great toe was swollen.

While grinding a banana wedge on a bench grinder, the wedge was pulled into the grinder causing right pinky finger and ring finger to be caught in between the grinder wheel and tool rest.

Employee was using screw driver it slipped off the screw head. Employee was no wearing gloves, hands were sweating and the top of left hand caught a corner of the sheet metal panel they were working on causing a laceration.

Employee while working on a dozer pushing material slid into a cavity causing employee's head to move side to side rapidly causing a strain to neck

At 11:30 am Employee reported that while moving the forks on the forklift they felt a pain in right shoulder.

Operators were beginning to work on installing steam and air rig on TK61 for caustic cleaning. While in process of installing steam hose operator was burned by steam.

Removing a driveshaft and drive tip from a capsten fitting to a hopper. The employee straddled the driveshaft with left hand holding the drive tip and right hand was holding the driveshaft. The employee out stretched arms could not control the driveshaft and drive tip, causing it to fall 18" to the concrete. Employee left hand middle finger was pinched causing a laceration.

While installing a steam rig on the bottom of Tk 48 employee stepped in residual ore/liquor which seeped into the bottom of EE's boot through a hole and burned a small area on EE's left lateral side foot.

Employee finger caught between wrench and top of freight elevator door. Causing fracture to the finger tip and laceration to finger requiring stitches.

Crew was swapping out a 12" pump at #6 pond. While moving pump to new location, with an extended boom forklift, pump was suspended from the forklift by rigging. A miner was walking along side of the pump. The pump was about 6" above the ground when it hit a high spot which pushed the pump against the forklift outrigger, smashing the end digit small finger left hand.

Operator was draining line through the steam rig. Operator checked pressure on airline to take off and nothing came out. Operator proceeded to take airline off and caustic bypassed check valve and sprayed on EE.

IW was connecting rigging to an excavator bucket when EE's left foot was pinched under the bucket resulting in partial amputation of left big toe. Bucket moved due to listing of derrick barge when crane on the barge swung in direction of dock. Excavator was on derrick barge with arm extended over the dock, IW was on dock rigging the load when incident occurred.

Employee was using forklift to transport 55 gal oil drum to powerhouse area. In the process of moving drum from warehouse pallet to transport pallet, drum rolled and pinched/smashed right middle and ring finger causing fractures to top of both digits.

Employee was walking into Press Floor area at NE entrance to assess scale buildup on wall to determine work that needed to be done in area. Was in process of communicating to mobile equipment operator supervisor when liquor dumped on back and neck from floor above. Caustic burns to upper back, posterior neck, upper anterior and posterior of left arm, small spot on left wrist

Personnel had rolled spool out of #1 Decanter U/F Pump and was washing rocks out of pump, this loosened plugged material in pump causing hot caustic mud to spill out of discharge line. Personnel heard material break free in line and turned to get away from it when it came out of pump across majority of back. This caused burns to back.

Employee was closing V1 steam valve to secure vessel when right thumb was pinched between the valve wrench and the railing.

Employee was opening a valve and EE's right index finger was pinched resulting in a laceration and fracture.

Employee stepped on grating while untwisting a hose and sprained (L) ankle.

Employee was a welding helper cutting and welding stainless steel. Employee worked a double shift and noted a problem on the drive home. Employee reported the eye incident the following day. Employee was taken off site for initial visit on 10/17/2019 and the following day was seen by an eye specialist on 10/18/2019.

Left forearm foreign body.

Employee was pushing forward to open a valve using force and EE's forearm made contact with a bolt underneath the valve. Resulting in a laceration to EE's (R) forearm that required 4 stitches.

Laceration without foreign body right little finger.

Employee was on a stand by HI-2 swinging a maul to open a drain valve when EE felt a pop in the right shoulder. EE was seen at Gulf Coast OCC Med and referred out to Ortho. MRI was done on 05-03-2019 follow up was on 5/7/2019. Results were right rotator cuff tear and bicep instability.

Operator was opening draw off valve at TK -34 where their left thumb was caught between the beater and maul. X-Ray was negative on day of incident. On 1/31/2019 after swelling went down they visited a hand specialist and was placed in a splint for 3 weeks.

Employee was removing scales with a chipping hammer when a piece of debris was knocked off and punctured the glove on employee's left hand. When the glove was removed there was a fine point on left thumb.

Employee was driving a cart, became distracted and drove into a hole striking a pipe. Employee was taken for medical care at Gulf Coast and (R) elbow x-ray was negative. First visit to Dr. on 2-6-2019 and EE was returned to full duty. 2nd visit was on 2-20-2019 and MRI was ordered and EE was placed on 0-10 lbs. upper extremity restrictions.

Powerhouse boiler operator was climbing steps (2) at a time going to restrooms. When the operator felt a pain in right shoulder.

Employee was trying to open a circulating line on T-6, in the process of swinging the maul they missed the valve and struck their right middle finger between the handle of the maul and a safety rail.

Employee was cutting tie wraps with a pocket knife. When the tie wrap broke the knife slipped and cut (L) middle finger.

Employee was leaving an office and walked around a barricaded area and slipped in the mud. EE braced self on the dumpster to keep from falling and right foot turned outward resulting in a fracture.

Employee was opening a three way valve and caustic made contact with their left eye and both nostrils.

Employee was removing the wrapping from around a new light stand, and when ee loosen the stabilizer bar finger was pinched between the two bars.

Employee was in precipitation standing on a platform at Tank -52. Removing a 90 degree steam injecting piping. When they removed the last (2) bolts they felt caustic liquid enter their left glove resulting in a burns to the fingers.

Employee was blasting a metal plate when the air hose was entangled on the plate resulting in a left shoulder strain.

While opening a screen box, a leaking nearby pipe sprayed spend liquor to both ears and eyes to left side of face.

Employee was nailing down a cloth on the #5 pan filter when EE hit left index finger with a hammer resulting in a fracture and a laceration.

The employee was changing a control valve on # 4 boot pump. EE has the controlroom operator stroke the valve and while stroking EE felt the glove on right hand being pulled into the shaft.

Employee was relocating sample bottles while walking underneath a crossbeam that struck employee's hardhat resulting in pain to the left cervical.

Operator was trying to put #8 heater on the bad condensate line using a wheel wrench on the wheel valve. The wrench slipped smashing ( R) middle finger between the wrench and piping.

Supervisor was walking to EE's office when EE slipped and fell backward landing on EE's right upper back. Seen at Gulf Coast Occ Med and returned to work after negative x-ray. Reported change in symptoms on 1/29/2018 and was seen by Ortho Dr. on 1/30/2018 and given restrictions and medications.

Employee was turning the belt pulley on the east mixer to see if it would turn freely when ee crushed 4th finger on left hand.

Maintenance employee was swinging a 10lb maul when they felt a pain in the (R) shoulder. Didn't report until day after injury. Negative x-ray was completed on 12-15-2017 at Gulf Coast remained problematic. Ortho MRI on 01/30/2018 revealed bicep tear and employee was restricted on 2/06/18.

Walking in Jamaica building. While walking fell in hole. Injury left knee.

EE taking motorized pipe beveller out of storage locker. Felt a pull in left arm. Started to swell went to medical, evaluated ice applied to area and RTW. Follow up visit to medical, EE sent to Orthopedic doctor 10-17-17 for evaluation. Orthopedic diagnosed rupture left bicep.

Employee was communicating to an operator and banged right hand on desk. At that point ee felt something snap in wrist / hand.

Employee was putting water on the suction of tank Y-5 to fill. After turning water on EE approached suction ball valve to check if water was flowing and at that time the hose came off from the Chicago coupling. Employee had caustic spray EE in the face EE was wear chem goggles spray entered goggles and contacted eyes.

Employee standing on elevated platform pulling 1B filter cloth to straighten out on unit. While pulling on filter cloth lost grip of cloth fell backwards from platform approximately 5' in height to lower level. Landing on back.

5-8-17 EE was walking across top of #2 washer tank, surface deflected. EE stated felt a pain in left knee. Sent to medical, ice applied to knee and OTC anti-inflammatory taken. EE working no restrictions. EE sent for follow ups including x-rays, CT and MRI on 6-16-17 found potential left knee ligament tear. Restricted duty Follow up with Orthopedic.

Motor was being lowered onto vertical pump shaft, choker cable broke motor dropped down vertical pump shaft approximately 3 feet. Employee was hit on brim of hard hat caused laceration over left eyebrow and abrasions on left hand and lower left leg.

Employee walking up stairway to work level to take down scaffold stumbled on sling ee was carrying. Lost balance causing ee to fall forward. As ee was going forward caught self with right arm to stop fall and felt pain in right shoulder. 3-24-17 Evaluated in medical first aid treatment. 4-10-17 F/U evaluation scan shows rotator cuff injury. Restricted duty.

GSI ee sitting in break room trailer speaking with coworkers. Began to cough, shake and gasping for air became unresponsive fell to floor remained unresponsive. Noranda ERT dispatched to access employee CPR started, outside emergency services notified Acadian Ambulance responded took over CPR. Employee remained unresponsive to CPR. Contract ee deceased.

Employee opening #11 filter press, press had not competed draining and caustic (liquor) came out when the press was being backed up. Caustic (liquor) sprayed employee lower legs. Employee sustained caustic chemical burns to lower leg area.

Employees were descaling spool using a hand maul. The injured employee was bending forward looking at the pipe spool as the 2nd employee was swing the maul the maul struck the pipe spool then bounced off the pipe spool striking the injured employee on the left side of the face.

Employee was moving a piece of metal plate on a shop table. While doing so, the employee felt pain in abdomen.

Employee was striking a chipping hammer with a ball peen hammer. While doing so, a piece of metal broke off of the chipping hammer striking the employee's left thumb. The metal punctured the glove and embedded in the employee's thumb.

Employee was walking on top of a dry bulk barge and missed a step, causing employee to fall. As employee was falling hit head and back on the dock. The result of this contact with the dock caused the employee to have Transverse Process fractures in lower spine and bruising to head and back.

Employee was flex lancing a vertical 6 inch pipe from the bottom. The opening was approximately 12 inches from the ground. As EE was flex lancing a piece of the product came lose falling and pulling the flex lance out. The tip of the lance struck EE's left leg below the knee causing a laceration. several times for follow up all ending in EE being released to light duty.

Employee was using an air chisel to descale a box in Precipitation. While doing so, the air chisel shifted and dropped, catching the employee's finger between the chisel and metal structure resulting in a fracture and laceration.

Employee was installing a sprocket. While doing so, the taper lock slid off of the shaft, striking the employee's finger, resulting in a laceration that required 4 sutures.

While removing a cylinder from the #2 kiln dust valve the employee caught the tip of right ring finger between the cylinder and the mounting bracket resulting in fracture.

Employee was removing a 4 inch ball valve to unplug a line. While doing so, the employee was splashed with caustic resulting in burns to the front of body.

With the assistance of another employee, an employee was carrying a countertop through a doorway. While doing so, the employee's hand was caught between the countertop and the door way resulting in a laceration that required sutures.

Employee was using an air chisel to descale B3 PAP discharge line in Precipitation and felt pain in right shoulder. Restricted duty started 8/25/16.

Employee in Precipitation stepped down into caustic material. Material entered the top of the employee's boots resulting in burns to both feet.

Employee was descaling #2 Washer. While setting down the air-powered scaling tool, the employee lost footing resulting in a back strain.

Employee was stepping off of a skid steer. While doing so, the employee slipped and caught left foot between the bucket and frame resulting in a strain / sprain of the left knee. Restricted duty started 8/4/2016.

Employee was opening a valve at the top of Precipitation. While doing so, caustic splashed on the employee resulting in burns.

Employee was walking down a stairway carrying a trash bag. While doing so, the employee missed a step and grabbed the handrail while off balance resulting in a dislocated left shoulder.

While taking the shifting cable off the barge, the cable pinched right hand. 1. employee failed to shift the barge into the correct position for cable release 2. employee placed hand in the pinch point between the cable and deck bit

Operator pulling on filter media sustained a back strain. Placed on restricted duty on May 9, 2016.

Two employees were pulling a slack rope to secure a ship at the dock. While pulling, one employee tripped resulting in a hernia.

Employee was swinging a 4 lbs. hammer to loosen a valve handle. While doing so, the employee felt a sharp pain in right arm.

While pulling on a wrench, employee felt pain in right wrist. Employee was placed on restricted duty on 4/18/16.

Employee was lifting an air tool stand with the assistance of another employee. While the employee felt slight back pain. The employee sought medical treatment 3 days later.

Employee failed to maintain proper footing when handling the rail switch

Employee was scaling hopper with pneumatic tool. When placing the tool on the ground, the employee felt a sharp pain in lower back. Employee was later referred to physical therapy.

Employee was replacing a section of 6-inch pipe and caught right thumb between pipe and an object resulting in a tuft fracture.

Employee complained of bilateral carpal tunnel syndrome. Surgery on right wrist on 12/28/2015 resulted in days away from work.

AT operator was standing in engine that was connected to empty rail cars. A KCS engine pushing empties coupled to the AT train causing a bump that knocked AT operator off balance.

Employee was standing upright from a stooped position and felt pain in his back.

Two employees were in an elevator when it stopped functioning. They were on the non-functional elevator for more than 30 minutes. No injuries or illnesses occurred. MSHA was notified within 15 minutes.

The employee allegedly tripped on a cable as he was attempting to walk over it. The cable is used to secure one dock barge to another dock barge. The employee stated that he felt pain in his back following the alleged incident.

While working during the day, the employee stated that he felt cramping and nausea.

Precip operator stepped into 4" x 14" floor opening resulting in abrasions and contusions.
