Employee was hydro-blasting some piping using a flex lance with no anti-withdrawal device on the flex lance. The flex lance came out of the pipe and struck the employee in the right upper thigh causing a water laceration to the upper thigh approximately half an inch long.

Operator was tossing small pices of scale into a wheelbarrow where an air lance was lying, energized with compressed air. One of the pieces of scale hit the valve causing it to open and the air lance jumped up toward the operator. The operator lifted his arm to protect his face. The lance struck his left pinky finger and helmet causing a fracture to his pinky.
