While cutting legs free on a refeed hopper at the jaw, a piece of rust popped and went behind employee's PPE. Employee was seen by a physician and diagnosed with a right eye abrasion.
EE started having eye irritation on 10/11 at 0900 hours. It is believed that a shard of metal from grinding the previous day caused the irritation to the eye. The flake of metal was removed and EE given antibiotics for this injury. EE was returned to work with no restrictions. During the grinding process EE was in full and appropriate PPE.
Employee was grinding on a six inch piece of steel pipe. Employee got a piece of metal in right eye. Employee was wearing proper safety protection. The conditions were very windy.
The miner was welding hammers on the primary crusher. He took welding helmet off and using his hands brushed the dust off of himself. When he brushed the dust off the fan he was using to blow welding fumes out of crusher, blew a small piece of metal in his eye. The miner had on all of his safety equipment, including his safety glasses.
Employee was hard surface welding. When he lifted up his welding helmet, the surface he was welding began to pop and a piece of slag got in the corner of his right eye.
Injured was removing a cutting edge on a 988 loader. He was using a 1 impact to remove the bolt. The bolt dropped and spuning hand. Resulting in a cut in his left hand.
Employee was between the two big screens on the stone plant, welding an overhead nut. A piece of slag flew into his right ear and immediately began to burn and sizzle.
