Miner was breaking block with splitter. When miner went to pull splitter from block, block fell and pulled miner down, injuring the miner's arm and knee. Miner was sent home by manager the following day due to soreness and then miner sought medical treatment. Miner has not returned to work since.

Mostrando los 4On 7/29, miner was operating an excavator with door closed, window open; he got a piece of mud/dirt/rock in his left eye. A witness noticed irritated eye and offered miner eyewash solution but miner declined. On 7/30, miner reported to work but later left to see eye doctor. Miner returned to work next day on 7/31.
Miner was using hand drill when a piece of metal shaving became lodged in his eye. Foreman states that he turned accident report into Quarry Director at that time. No record of original report can be found, however. This came to my attention yesterday, and so I am now filing. Miner was wearing proper PPE.
The injured employee was cutting a piece of 1/8" steel with a hand held grinder when his finger came in contact with the cutting surface and cut his left index finger.
