MMGs were prepping belt for splicing PP-P3. MMG picked up 1/4" die grinder w/buffing wheel, pressed trigger to test prior starting work. The quick connect holding 3" buffing wheel sheared at the base of the shaft nearly instantly upon rotation, sent buffing wheel/quick connect spinning to the right side of the tool, striking a nearby MMG ~3 ft to top of left-hand knuckle area.

Mostrando los 11EE was removing rock from the #1 apron feeder area. EE used hoist with a 4 ft. wire sling and rock hook to remove plug. When the EE began lifting the hook to remove the plugged rock, the crane moved causing the sling to rotate. EE's thumb was pinched in the loop of the sling as it rotated.
EE was cutting a bolt head off a plate that was secured in a vice. EE was using a portable circular saw. Saw blade contact the EE's left middle finger resulting in a laceration.
EE operating D-9 CAT was pushing loads off the dump. While operating in reverse (2nd gear), man basket came loose. This caused the equipment to stop abruptly resulting in pain in the EE's neck area.
EE was operating #327 D-11 on the 1001 dump. EE exited cab and used power lift to get off equipment. The locking pin for the basket had difficulty releasing. As the employee stepped out of the basket, the basket jumped over the catch/stop mechanism and struck the employee in the left lower leg area.
EE was lowering power cable standards at the mine view. The winch was hung up not allowing the cables to come down to the ground. EE tried to manually move/rotate the winch by hand when the winch released causing the EE's gloves to get caught in outside gear. Resulted in right middle and ring finger getting pinched in the spool/gear.
MMG was changing a 8" pallet car arm pin. The pin did not fall into place upon install. EE grabbed hex head of stuck pin with left hand, partner was asked to jog controls while EE pulled upward on pin which caused the MMG's left middle finger to be pinched resulting in a laceration.
EE working on ladder was cutting corner of guard on finisher in Concentrator to fit around newly installed pipe. While utilizing grinder w/cut-off wheel, grinder kicked back contacting left knee causing laceration.
As production truck driver was being loaded, shovel swung over box of the truck & experienced a "swing control loss fault" causing it to lose swing control striking the front of the box. This caused jarring to production truck driver.
EE was arcing holes in Line 3 fans with safety glasses and welding hood on. EE noticed later during the day irritation in right eye but cannot recall specifically when something entered their eye. EE flushed eye several times that day. The following day, EE sought medical evaluation resulting in material being removed from eye and issued antibiotic eye drops.
EE was using excavator to remove ice frozen in pipe by lifting & dropping pipe with bucket. EE reported pain/soreness in middle of back. Sought medical evaluation at later date, received restrictions.
