Employee was removing a motor and reducer. Had motor rigged up with motor puller and chain and was helping lower the motor to the ground when the shaft slid out of the broken section of the auger jerking the employee forward. Declined initial treatment. Went to the clinic on April 10, 2026 due to shoulder pain and was diagnosed with a right shoulder strain and deltoid tendinitis.

Mostrando los 8While enlarging a hole for an E-Stop using a step bit, the bit snagged an aluminum label plate. The plate spun unexpectedly, resulting in cuts to the operator's thumb and index finger.
Employee was power washing the right port of the RK 1 Ram W, struck a pocket, causing hot material to blow back on EE. Employee jumped back to avoid material and hit EE's head on pin gate. Employee received sutures.

Employee was running the grader on 114 and had the blade in the float position. Turned off 114 into a heading and pulled the blade up. The blade caught a toe and it jolted the piece of equipment. Employee noticed discomfort in their back.
Unchoking PQL dust collector. Inserted air lance into ball valve to introduce air to unchoke the line and lime blew back in EE's face.
Employee was bagging at the hydrate packer and hydrate blew up into employee's face and went into the side of their goggles.
