Changing bearing on screw/auger style washer. Auger was supported with crane. Bearing needed wedged free from auger. When bearing came loose, auger shifted pressing on left foot between auger and side wall of wash tub.

A shard of flew off of drill steel while roof bolting at the face, and stuck in ee right pointer finger. Reported it to his fellow bolter and removed it and continued to work every day till Friday the 14th., where he reported it as infected and sought medical attention.

While installing a roof bolt plate, EE roof bolter rested his left hand on the drill head of the bolter. While raising the drill head he pinched his thumb between the bolter head and the roof bolt plate that he was installing, causing a laceration to his thumb.

While attempting to work loose a stuck drill steel in the roof, the drill steel became bound in the drill chuck with some side pressure. The drill steel snapped loose from the chuck striking the middle finger, lacerating the knuckle of his left hand.

While tramming crawler bolter, operator's left thumb became pinched between the bolter and the mine roof, when the machine teetered over a high spot in the mine floor causing the back of the machine to pivot up against roof. A laceration occurred requiring 4 stiches.

Right side bolter operator was attempting set an extra bolt along the right rib and the bolter drillhead would not reach to that location, the left side bolter operator attempted to move the bolting machine so it would reach and it jumped pinning the right leg (kneeling position) against the floor, severly bruising the top of his right leg muscle

EE WAS DRILLING A HOLE WITH THE FLETCHER BOLTER TO SET A ROOF BOLT WHEN A PIECE OF ROOF COAL (BONE) BROKE LOOSE FROM THE TOP, HIT THE SPINNING DRILL STEEL AND A 3" SQUARE PIECE OF ROOF COAL FLEW AND HIT HIM IN THE NOSE, BREAKING HIS NOSE.

BOLTER OPERATOR PULLED INTO THE CUT, SET HIS TRS AND PREPARED TO DRILL HIS FIRST BOLT HOLE WHEN A PIECE OF ROOF ROCK ABOUT 100 LBS. FELL FROM THE ROOF, CATCHING HIM IN THE BACK OF THE NECK AND SLIDING DOWN HIS ARM HITTING HIS HAND AGAINST THE BOLTER.

I WAS CUTTING SCRAP WHEN MY CLOTHES CAUGHT ON FIRE. FIRST AND SECOND DEGREE BURNS OF LEFT SIDE OF BACK.

EE WAS IN PROCESS OF DRILLING A 13/8 ROOF BOLT HOLE WHEN HE LOWERED THE DRILL HEAD TO ATTACH ANOTHER PIECE OF DRILL STEEL WHEN THE DRILL STEEL FELL FROM THE HOLE HITTING THE EE'S HAND BETWE EN THE DRILL STEEL AND THE DRILLHEAD

ROOF BOLTER OPERATOR WAS EXTENDING THE ATRS ARMS ONT EH BOLTER AND GOT HIS HAND BETWEEN THUMB AND FINGER, PINCHED BY SUPPORT PAD WHICH PIVOTS ON THE ATRS ARM MUSCLE AND LIGIMENTS BETWEEN THUMB AND FINGER BRUISED OF LEFT HAND.

HE WAS MOVING THE BOLTER FROM A CROSSCUT TO POSITION THE BOLTER IN #3 ENTRY TO BOLT. HE WAS ON THE OPPOSITE SIDE OF THE BOLTER. OTHER EMPLOYEE TURNED THE BOLTER SLIGHTLY TO CENTER IT IN THE EI NTRY. HIS ANKLE WAS CAUGHT UNDER THE PANIC BAR BEHIND THE BOLTER HEAD.

WELDING ON BOWL OF PAN WHEN CLUMP OF DIRT FELL DOWN & HIT EMPL ON LEFT ARM & BACK OF NECK.

WHILE OPERATING DUMP HOPPER FEEDER IN TIPPLE EMPL TRIED TO PUSH HANDLE TO FREE FROZEN PIECE WEDGED IN DOOR FELT PRESSURE ON RIGHT SIDE OF BACK.

EMPLOYEE WAS HOLDING A CRANE HOOK FOR CABLE TENSION. HE WAS LOOKING DOWN AT THE CONTROL WHEN THE HOOK WENT UNDERNEATH A STEEL FLANGE WHILE GOING UP. HIS FINGER GOT SMASHED BETWEEN THEM AND RIP PED OFF HIS FINGER NAIL.

MAN WAS BENT OV9R CUTTING A PIECE OF STEEL WITH HIS WELDING TORCH, WHEN STEEL STARTED TO FALL, HE MOVED AND HIS BACK JUST SEEMED TO POP OUT.

WHILE MOVING ELEVATOR STONE GLANCED OFF ROOF SUPPORT ABOVE HIM RESULTING IN STONE HITTING HIS LEFT ARM RIGHT FOOT & HIS HARD HAT.
