Employee was welding on a auger flight wen a small piece of metal got in EE's eye. Employee was wearing safety glasses at the time of the accident.

Employee had installed a 3" discharge line on water pump. When pump was started, the discharge line came off striking employee in the face.

Employee was operating a shuttle car. While being loaded, a rock fowled in the discharge end of the continuous miner conveyor chain. When the rock came loose, it was thrown toward employee, striking them on head behind right ear.

Employee was operating a shuttle car when a piece of rock was thrown from the continuous miner boom and struck employee on face and forehead.

Employee was trying to remove a piece of steel that was stuck in the drill chuck. EE's hand slid up the drill steel and across a steel burr on the drill steel lacerating EE's left hand.

EE accidentally swung head of roof bolter to close to himself, pinning his right arm between head of machine and rib.

EE was helping move a 245LN miner, when he reached for the cable, he put his left hand on the cable standoff when the back of the moner came up and caught his arm between the roof and the frame of the miner fracturing his left arm near his wrist.

The bolter operator was running the machine and when EE reached up to twist the roof bolt plate, his pinky finger on the right hand got caught under the plate, smashing the end of his finger from the back of the nail to the end of the finger.

Two employees were hanging miner cable when the cable was pulled down by the machine operator pulling up extra cable.

Injured EE and co-worker were hanging miner cable, when the cable that the two men were hanging was pulled down by machine operator, pulling extra cable.

WHILE TRAMMING MINER TO THE FACE OF 00 HDG., TRAMMED OVER A HUMP IN THE ROADWAY & MINER TEETERED OVER HUMP CAUSING OPER. TO HIT HIS HEAD ON TOP.

Piece of rock fell from roof between the bolts, hit person on right shoulder.

EE WAS INSTALLING BOLTS ON HELPER SIDE OF A DD015 FLETCHER BOLTER IN THE #2 FACE. AS HE WAS PUSHING UP THE BOLT, A LOOSE PIECE OF DRAW BROKE AROUND THE BOLT (APPROX SIZE 1' X 6" X 3"), FELL AND STRUCK EE ON HIS HAND. BROKE EE'S RING FINGER.

Rock falling hit him on top of right foot. Witness stated he was on the open side when this happened and when he got to him he had foot out under rock.

ROOF BOLTER OPERATING HELPER SIDE OF THE DOUBLE HEAD FLETCHER. WAS INSTALLING HIS SECOND BOLT IN THE FIRST ROW IN THE #7 ENTRY WHEN A LOOSE ROCK MEASURING 5' X 5' X 0 TO 5" THICK BROKE OFF AT THE EDGE OF THE ATRS AND FELL STRIKING OPERATOR ON THE RIGHT LOWER LEG.

BOLTER OPERATOR WAS INSTALLING HIS 2ND BOLT WHEN A LOOSE PIECE OF DRAW ROCK APP 0-3" THICK 3' LONG & 2' WIDE FELL STRIKING HIM IN THE BACK & BRUISING HIS RIBS. HE WAS SENT TO THE HOSPITAL, TRE ATED & RELEASED. RETURNED TO LITE DUTY NEXT DAY.

EMPLOYEE WAS BOLTING TOP WHEN THE DRILL BIT BROKE. HE CONTINUED TO PUT UP PRESSURE ON THE DRILL STEEL WHICH BROKE AND HIT HIM ABOVE THE RIGHT EYE.

MAN LEAN BETWEEN THE BOOM AND THE SYSTEM OF THE MACHINE TO REMOVE DUST HOSE, ACCIDENTLY HIT THE UP CONTROL OF THE BOOM WITH HIS LEG AND PIN HIMSELF BETWEEN THE ATRS AND BOOM OF THE MACHINE.

EE WAS ROOF BOLTING WITH A 300 GALIS ROOF BOLTERWHEN HIS GLOVES GOT CAUGHT WITH THE DRILL STEEL WRAPED THE GLOVES, BROWN JERSEY GLOVES AROND THE DRILL STEEL. [2 PAIR]

PIECE OF ROCK FROM RIB TO FIRST PIN.

WAS ADJUSTING STRAP (METAL) AND CAUGHT LEFT INDEX FINGER IN BETWEEN STRAP AND ROOF, MASHING FINGER.

MAN WAS TRAMMING MINER AND CAUGHT FOOT AGAINST RIB.

I NAME WAS PINNING TOP & GOT HIS GLAOVE CAUGHT IN DRILL STEEL & IT TWISTED HIS WRIST & ARM

CLOSING MAN DOOR RT HAND INDEX FINGER GOT CAUGHT IN DOOR TORE SOME SURFACE SKIN

HE WAS OPEARTING THE MINER IN THE #2 BREAK THOUGH WHEN A SLAB OFROCK FELL OUT BETWEEN THE BOLTS 4 FOOT LONG, 2 FOOT WIDE AND ABOUT 4 FEET THICH.
