Employee was working on cutting and grinding metal for the fines chute on the LT330GP cone plant, EE at noon time felt something in EE's left eye. Went to urgent care where EE's left eye was flushed and drops added. They said EE's eye had a scratch/abrasion.

Laceration to left thumb while operating a power tool

Employee was using cutting torch and grinder to repair a rock crusher main frame liner, when a piece of metal went up under the face shield and safety glasses into EE's eye.

The miner was grinding a steel plate when the grinding wheel broke apart. The piece that came off cut their right leg requiring going to the emergency room for stitches.

Mechanic had just completed brake job on company 3/4 ton GM van. He had removed the jack stands from the rear axle. He had the hydraulic jack under the center of the axle. He was lying on a creeper and turned the handle to release the jack. The jack came down fast and the handle smashed his wrist onto the shop floor.

EE was performing repair work on an excavator bucket. The grinder he was operating kicked back and cut his wrist.

While working with an air tool to tighten a bolt the employee cut his hand.

While performing maintenance work on a jaw crusher, the employee activated a 1/2" drive air gun which blew dust into his face and eye. The employee was wearing safety glasses at the time.

Welding on a feeder in tight location, did not pull welding shield down fast enough. Received a welding flash.

Removing bolt using impact gun. Finger crushed between truck and gun.

A PIECE OF STEEL GOT STUCK IN THE CONE CRUSHER EE AND OTHER WERE USING A LONG BURNING ROD TO BURN THE STEEL OUT OF THE CRUSHER. ONCE THE STEEL WAS REMOVED HE PLACED THE BURNING ROD INTO A BUCK ET OF WATER TO COAL. THE WATER SPLASHED INTO EE'S FACE AND HE THINKS THAT S WHEN THE DEBRIS ENTERED HIS EYE.

CHANGING HOIST CABLE, CRANE SWUNG AND PUSHED EMPLOYEE TO QUARRY FLOOR INJURING LEG.

EE WAS STRADDLING OVER A PULL CABLE, WHEN THE SLACK WAS TAKEN UP ON CABLE. THE EE WAS LIFTED AND FELL TO GROUND.

WORKING IN JAW FEEDER WHEN HE WAS SITTING DOWN - THE GRINDER KICKED DOWN AT HIS LEG.

WORKING ON PORTABLE PALNT. HIT HEAD UNDER PLANT. CUT HIS EAR REQUIRING 10 STITCHES. EE WAS WEARING A HARDHAT.

EMPLOYEE BACKED TRUCK ONTO STOCK PILE. TRUCK WENT OVER BERM AND GOT STUCK. HE THOUGHT TRUCK WOULD GO OVER STOCK PILE SO HE JUMPED OUT AND LANDED ON HIS FACE AND FRONT PART OF BODY.

EE WAS LOOKING TO FIND A PIECE OF MATERIAL IN THE ASSEMBLY OF THE TEAMWATER. AS HE WAS LOOKING HE LOST HIS MIRROR, AND ASKED THE OPERATOR OF TH EMACHINE TO TILT THE ASSEMBLY IN ORDER TO RETRI EVE HIS MIRROR. AS HE REACHED INTO GET HIS MIRROR, THE HAMMER CAME DOWN UPON HIS HAND.

EE WAS GRINDING A STEEL PLATE WHEN HE BUMPED HIS KNEE WITH THE HAND GRINDER. HE HAD TO GET 2 STITCHES.

OBSERVING WELDER WORKING ON IRON BAR-DID NOT HAVE SAFETY GLASSES ON FLASH BURN OF EYES.

WHILE GRINDING STEEL, WRIST BRUSHED AGAINST SHARP EDGE. LACERATION REQUIRED A COUPLE OF STITCHES.

AFTER WELDING SCREEN, LEANED OVER TO GET NEW ELECTRODES, HAND SLIPPED ONTO HOT WELD.
