Quarry worker, was cutting rock with the grinder, wiped EE's safety glasses with EE's glove and took them off, and wiped EE's eye while getting the silt off EE's face with EE's glove, which EE thought was clean and pushed debris into EE's eye.

Mostrando los 27Injury occured on 7/18/24, employee had let the company know it was work related injury on 7/25/24. Warm sunny day, cutting rocks on cutter machine, rock split, and broke into smaller pieces, and one bigger piece struck worker on the right forearm causing pain in right elbow.
Employee was off bearing on a cutter, machine crushed right wrist.
Employee was operating the Saw shop Corner machine. While readjusting the rock, the table slid causing a right-hand laceration to the top of the right hand getting caught between the rock and the saw.
On loading a stone onto the stone cutter rollers, stone slab caught the corner of the table and tilted causing reaction to tilt the rock back onto the rollers, and in the midst of trying to maneuver the rock back onto the rollers, the slab caught the worker's middle finger resulting in an injury.
EE was using the grinder when EE felt the grinder started to feel like it was becoming loose. The kill switch was located directly near the grinding wheel and guard. EE was slowly moving the grinder down towards the ground so the wheel wouldn't hit EE. As EE did the wheel came off and so did the guard.
EE was splitting rock when a metal piece shot up and hit EE in EE's chest.
EE was trying to help coworker adjust the cutter when ee finger was caught, causing ee left pinky to get stuck and possibly break. The nail broke and ee was wearing gloves.
EE was grinding on a bolt cutting, the cutting wheel disintegrated and a small piece of metal got in EE's eye. EE was wearing safety glasses at the time of the accident.
Worker was holding a rock on cutter #46, when the stone was snapped his foot slipped and he felt a pain in his ribcage. His supervisor saw him asked him if he was hurt he said that he was, but he thought it would get better over the weekend so he didn't wanted to go to the doctor, but on Monday he told his supervisor that his ribs still hurt and he wanted to go and see the doctor.
The employee was polishing the side of a stone when he felt a piece of stone hit his eye. He did not report it immediately hoping it would get better. It did not. He was diagnosed with a corneal abrasion and given a prescription.
Employee was off bearing stone from a stone cutter. There are two forks that come up from the table to "catch" the stone. The stone jumped and the employee caught his little finger between the stone and one of the forks. He received 7 stitches and was prescribed hydrocodone.
Employee was drilling holes for blasting when his foot slipped causing him to fall. He was prescribed ibuprofen and put on temporary lifting restrictions.
Employee was off bearing strips of stone when a stone fell off the cutter and landed on top of his right foot, just above the steel toe. Employee was placed on temporary restrictions.
Employee was polishing stone when the polisher jerked causing his wrist to twist. An hour later a bump appeared on his wrist accompanied by pain. Employee complains of pain in his wrist and arm. Amendment: Put back on restricted duty on 9/25/08.
Employee came in the morning of 8/27/08 reporting of pain in his left shoulder. He stated that he had injured it the day beofre while polishing, He was given percocet and was placed on temporary restrictions.
The employee was drilling holes with a hammer drill when the drill bit caught and caused him to twist his back. He was placed on temporary lifting restrictions and vicodin.
Employee was polishing stone and reported pain in his right shoulder.
EE was positioning a rock to be sawn. His left hand was holding the rock & template & his right hand was on the controls of the 24 inch prodigy saw. He then inadvertently pushed the wrong buttons & dropped the saw which was not in rotation on his left middle finger. He received stitches & was placed on temporary lifting restrictions.
Employee noticed pain at the base of his left index finger. A lump has also formed which has become increasingly larger over time. Prescribed 500 mg of Naproxen.
Employee was polishing rock with a rock polisher. His left side started to hurt. Prescribed Naproxyn 500 mg.

EE WAS BEARING STONE WHEN HIS FINGERS WERE CAUGHT IN BETWEEN THE STONE "STOPPERS" & ITS OPENING ON THE OFF BEARING SIDE OF THE TABLE.
EMPLOYEE INSERTED FINGER UNDER A 12" X 12" STONE IN ORDER TO MOVE THE STONE INTO POSITION ALONG THE ROLLERS OF A STONE CUTTER. WHEN HE MOVED THE CONTROL STICK TO LOWER THE HYDRAULIC CUTTER ONT O THE STONE TO SHEAR IT, THE ROLLERS LIFTED CAUSING HIS RIGHT MIDDLE FINGER TO BE SMASHED BETWEEN THE ROLLERS AND STONE.
WHILE OFF BEARING STONE FROM HYDRAULIC STONE CUTTER, EE FAILED TO REMOVED FINGERS FROM UNDER CUTTING TEETH BEFORE SECOND EE LOWERED THE CUTTER TO CUT ANOTHER STONE. FINGER WAS SMASHED UNDER TO OTH OF CUTTER.
