EE was getting ready to replace a fan belt. He didn't close the fan louver's and put a board in the fan to keep it from turning as a safety, the fan started turning backwards he tried to stop it with his right foot. The belt pulled his foot between the belt and sheave. It broke both bones in his ankle.

Employee was operating crusher when silo got full and material started to plug up. Employee then put item in part way to stop material from being fed into machine. Employee took guard off and reached into machine to move material away from crusher rolls. When material was moved away the roll started to operate again pulling his left hand into it.
Injured was drilling a bolt out of a shaft on top shaker. He remembers rubbing his eye. Later that evening his right eye started paining him. He went to ER where they removed a piece of metal and advised him to follow up @ eye clinic. They cleaned out a rust ring that had developed and excused him from work the following day. He returned to full duty July 18th.
Employee was jackhammering, he slightly turned on his right leg and felt a pull in his right hip. An MRI was scheduled for 3/16/07.
Employee was jack hammering midway inside a kiln. He looked up above him to check the bruicks, doing so the jack hammer bounced down on top of his left foot.
EE WAS GRINDING ON A KILN TIRE & A PIECE OF SCRAP METAL FLEW INTO HIS LEFT EYE. EE WAS WEARING EYE PROTECTION.
EE WAS DRILLING. HE WAS HOLDING 5THE DRILL WITH HIS LEFT HAND & A BOARD WITH HIS RIGHT. THE BIT BROKE & JUMPED OVER CUTTING HIS RIGHT FOREARM. NO STITCHES--RECEIVED TETANUS SHOT.
EE WAS GRINDING METAL AND A PIECE FLEW UP INTO HIS LEFT EYE EMPLOYEE STATED HE WAS WEARING SAFETY GLASSES.
EMPLOYEE WAS OPERATING A DOZER PUSHING UP A STOCKPILE. IT LUNGED FORWARD JERKING HIM.
EE WAS GRINDING WHEN A FOEIGN BODY FLEW INTO HIS EYE.
