While unclogging FD5 back mixer, operator was trying to move the stuck turbulizer to get it to rotate and continue cleaning. The turbulizer broke free and the paddle struck the operator's right palm resulting in a laceration that required medical attention. The operator needed 12 stitches.

Employee was using a grinder to prepare surface when a fragment of the wire wheel struck EE in EE's eye. Employee was wearing EE's PPE for the task which included Hard Hat, Safety Glasses, Face Shield and Gloves. Employee used the Emergency Eye Wash station on site and after the rinse EE still felt that there was more in the eye. Sent for medical evaluation

Drilling steel with a reamer. Reamer caught and spun the drill around.

Sparks from grinding activities above EE got into EE's eye. EE was wearing safety glasses.

While tightening lug nuts on a 992 loader the employee was using a 1 inch impact and EE noticed that the ring holding the safety pin on the extension was coming out and by the time EE could react the pin had come out and caught EE's wrist, causing the cut. A fellow employee helped administer first aid to stop the bleeding and bandaged it up.

Employee was driving a ground rod into the dirt using an electric hammer. The hammer slipped off the tip of the rod causing employee to fall forward. Employee fell into the ditch and landed wrong on foot.

An employee went to clear a bag that was stuck in-between the discharge conveyor belt and idle roller on Roto Packer C, while the equipment was energized and still moving, in clear violation of procedures . When EE tugged on the bag it dislodged and pulled EE's hand and arm into the pinch point. The employee received bruising to EE's hand and arm.

Employee welding pipe in an awkward position, causing the employee to get a strained neck.

While an employee was using a parts washer, the hose for the brush disconnected, spraying the cleaning solution onto the employees torso and face. Some of the solution went behind his safety glasses and into his right eye. The employee received eye irritation with no damage.

A miner trammed his excavator over the berm and edge of the 60 feet high bench, falling to the quarry floor below.

While using a right angle grinder on a flange that had been cut off. individual lost control of the grinder. Individual received a cut to his right forearm.

Employee was operating excavator equipped with ripper (rock breaker) breaking boulders when he felt upper back/neck pain.

Off loading a drill hammer from a pick up truck by hand to ground. End of right ring finger was crushed/amputated when driver sub slid down to bit while lowering end of hammer to ground.

Employee was drilling a hole above his head when the drill bit jammed causing the drill to rotate. The drill handle struck the employee on his lower lip causing an open wound requiring suture.

Employee was operating D9 bulldozer in quarry when it came to an abrupt stop when it hit a rock causing pain in upper back.

Welding on crusher, repairing broken tooth. Auto darkening lense failed on welding helmet, Caused flash burn to the eyes. Supervisor carried employee to the emergency room, doctor prescribed drops for eyes. Plant will be moving to non auto darkening lenses for major welding jobs.

Employee was using a hand-held drill to insert a screw into an electrical box he was holding. The box flipped causing the drill to slip resulting in the screw bit puncturing the soft tissue fingertip, ring finger, left hand.

Employee cut back of left index finger with hand-held grinder.

Driver was getting loaded by excavator, operator dropped a rock and driver said seatbelt grabbed him and injured his back. Driver had previous injuries to his back, not work related. Had went to doctor two weeks prior.

Employee was working in the fab shop grinding. He slipped and the grinder hit his hand.

Employee was putting a filter back into service after filter sock change. The filter lid gasket leaked upon the introduction of slurry product. Hot slurry released onto the back of the employee's head/neck/behind ear area. 1st degree burn to approximately 6" x 6" area and 2nd degree burn to approximately 1" x 1" area.

Employee operating overhead hoist. The control pendant slipped from his grasp and struck his knee causing a contusion.

EE WAS HAND-SPOUTING EVERY OTHER SPOUT ON ROTO PACKER WITH PLASTIC BAGS. EE FOUND HIS ARM/SHOULDER TO BE SORE WHEN SHIFT WAS OVER.

WHILE USING AN OVERHEAD STACKER CRANE, EE WAS MOVING A SMALL BLOWER FROM A PALLET TO THE BOTTOM OF THE STORAGE SHELF. WHILE POSITIONING THE BLOWER, THE WEIGHT SHIFTED & EE'S RIGHT THUMB WAS SM ASHED BETWEEN 2 BLOWERS.

THE MAINTENANCE TEAM WAS LOWERING A HEAVY STEEL TABLE AT THE ROLLER MILL #2 MILL BASE. THE SENIOR MECHANIC INSTRUCTED THE OTHERS TO MOVE OUT OF THE WAY. AS THE TABLE WAS BEING LOWERED, IT FELL TO ONE SIDE AND PINCHED THE EMPLOYEE'S LEFT ARM BETWEEN THE TABLE AND THE MILL CONCRETE BASE. THE TEAM WAS USING A CHAIN-FALL BUT SHOULD HAVE USED ADDITIONAL LIFTING DEVICES TO CONTROL TABLE.

APPLYING TENSION VIA HAND TO REEL OF PAPER BAGS, HAND GOT CAUGHT BRUISED, BUT NO BROKEN BONES.

GRINDING A NUT ON A BENCH GRINDER HE GOT METAL ABRASION IN RIGHT EYE.

HOLDING PUMP FOR TEST WHAN PUMP WAS TURNED ON. EVERYONG KNEW THE PUMP WAS TO BE TURNED ON. HOSE SLIPPED AND HIT EMPLOYEE IN THE HARD HAT. HE WAS KNOCKED TO THE FLOOR. HAD A LEFT SHOULDER ABRAS ION FROM THE HOSE.

EMPLOYEE WAS CUTTING STEEL AROUND THE AREA INJURED WAS WORKING IN. A PIECE OF STEEL FLEW IN THE AIR AND HIT INJURED IN THE BACK.

HE WAS REMOVING A SCREW FROM METAL STUDS AND APPLING PRESSURE TO THE RIGHT AND THE SCREW GUN SLID OVER ALLOWING HIS HAND TO HT A SHARP EDGE.
