The employee was welding a pump skid to a steel dredge pontoon when a pontoon hatch became displaced, pinching the top of the employee's foot just above the protective toe of the required work boot. The incident resulted in a metatarsal fracture in the employee's right foot.

Employee was using an impact wrench and a combination wrench to unscrew rusted bolts/nuts from a chute on the plant. While engaging the impact wrench on the bolt head, the combination wrench, which was positioned to hold the nut in place, slipped from the employees grip and struck them in the area of the left eye causing a contusion and laceration to the upper eye/eyelid.

Employee was using a cutting torch to cut a bolt on the bottom of a pipeline joint when the hot end of the bolt fell into employee's glove and burned hand.

Employee was repairing a piece of plant equipment and, while using a cutting torch to remove an overhead bolt, the wind blew a piece of hot metal into left ear causing an inner ear burn.

Employee was injured while guiding a 20" diameter steel pipe elbow, which was being lowered by a crane, onto some floor mounted pipe stands on the dredge. As the pipe was lowered, it was not seated properly on the stands. The crane was signaled to stop, but the pipe shifted and pinched the employee's lower left leg between the elbow and another piece of equipment.

A foreign object made its way into the ee's right eye when grinding on a metal drive sprocket. Safety goggles were worn by the ee but continued to fog up. The foreign object may have entered the eye when removing the goggles to clear the lens. The ee's eye was irritated & required medical attention of removing a rust spot & prescribing an antibiotic to clear up the irritation.

Employee was grinding steel on the underside of the dredge ladder. The employee was wearing safety glasses and a face shield. A foreign object made its way into the employee's right eye. The injury was not reported until two days after the incident. The foreign object was found to be imbedded and then removed by an eye doctor.

Two employees were disassembling a planetary drive gear box and shaft which was partially suspended by an overhead hoist and partially supported on a stand. The load shifted on the stand and caught the middle right finger of the employee and smashed the end of the finger. The employee received six stitches at the local hospital and was released.

WHILE WELDING HE PULLED TRIGGER ON MIG WELDER BEFORE HELMET WAS COMPLETELY DOWN. HOT SPARK FLEW INTO LEFT EYE BURNING CORNEA & CONJUNCTINAL SAC.

EMPLOYEE WAS GRINDING A FLANGE. THE GRINDER SLIPPED FROM HIS HAND AND CUT THROUGH A LONG SLEEVEDLEATHER WELDING GLOVE AND INTO EMPLOYEE'S LEFT FOREARM.

EE WAS GRINDING ON METAL PLATE WHEN A FOREIGN OBJECT FLEW UNDER HIS FACE SHIELD AND INTO HIS LEFT EYE.

EMPLOYEE WAS REMOVING A 5/8 NUT STUCK IN AN IMPACT SOCKET WHILE REMOVING THE NUT HE HIT THE TRIGGER ON THE IMPACT WRENCH & THREADED THE NUT ONTO HIS LEFT INDEX FINGER

EMPLOYEE WAS CUTTING A PIECE OF PIPE WITH A CUTTING TORCH, HE SHUT OFF THE TORCH, PUSHED THE AIR RELEASE TO COOL OFF TIP,THE TORCH SELF-IGNITED SETTING HIS GLOVE ON FIRE AND BURNING HIS LEFT HAND ON THE PALM BETWEEN THE THUMB AND FOREFINGER.

USING HAND HELD GRINDER IT KICKED BACK HIT EMP FACE CAUSING MOUTH INJ

EMPLOYEE WAS IN OUT SHOP USING AN ACETYLENE TORCH,CUTTING A SCREEN FOR A GUARD FOR THE SAND DRYER-THE TORCH FLARED,SHOOTING FIRE ONTO HIS LEFT PNAT LEG AND BURNING THROUGH TO HIS UPPER LEFT TH IGH.

EMPL OPERATING ROAD PATROL BLADING ROAD WHEN HE HIT CULVERT PIPE MACHINE STOPPED ABRUPTLY AND THROWN FORWARD HITTING FOREHEAD AND NOSE ON FRAME OF CAB.

EMPLOYEE WAS CUTTING, GRINDING AND WELDING ON A WIPER FOR THE STACKING CONVEYOR AT BIG SANDY #1 (#1210).

EMP WA> USING CUTTING TORCH TO CUT BOLTS OUT OF LOG WASSHER TRIED TO DODGE A SPARK WHICH FELL TOWARDS HIM A(D HIT BACK IOF NECK ON END OF CHUTE.

HE WAS GRINDING3A WELDED AREA WHEN PIEC EOF SLAG FROM WELD BECAME EMBEDDED IN LEFT EYE. EMPLOYEE DID NOT KNOW OF ACCIDENT UNTIL DO.

HOLDING DOWN CORNER OF SCREEN USING PLIARS WHILE IT WAS BEING BOLTED IN PLACE HE PUSHED THE START BUTTON TO ROTATE THE ROTARY A SHORT DISTANCE BUTTON STUCK AND DRUM CONTINUED TO TURN HE FAILED TO LET GO OF PLIARS AND WAS PULLED ONTO THE ROTATING DRUM

HE HAD PLACED A 4X4 WOOD TIMBER IN GEAR TO HOLD DIGGER CHAIN IN POLACE UNTIL IT COULD BE CLAMPED SO LINK COULD BE REPLACED WHEN OLD LINK CUT OUT GEAR STARTED TO MOVE BACK AND 4X4 FELL OUT HE T HOUGHT IT WAS RUNNING AWAY AND TRIED TO PUT 4X4 BACK IN IT SWUNG AROUND STRIKING HIS LEFT ARM
