EE was cleaning out a pilot hole with a PVC blowpipe so EE could wire saw when the pipe separated and struck EE on the right forearm causing a contusion.

The Employee was changing a piece of Drill Steel in EE's Novamac Quarry drill. The employee stated that EE had EE's hand on the drill steel and hit the wrong lever which caused the extraction clamp to close and raised the steel bringing EE's hand through the clamp causing a fracture to EE's left hand. The employee returned to work the same day with light duty restrictions.

Employee was using a Band Saw to saw 6"x 6" wedges to be used as wheel chocks. When the blade went through the cut, the employees hand made contact with the blade causing a laceration.

A piece of 11' drill steel fell out of a holder on a drill unit, the employee reached to catch the piece of steel and caught his finger between the drill steel and the retractor wheels.

While changing a drill bit on a drilling machine, the employee accidentally activated the boom movement which caught his hand between the drill bit and stone surface. The result was a fractured 5th Metacarpal of the left hand.

The employee was using a band saw to cut a steel "H" beam. When the employee approached to move the completed piece it fell on his right foot, resulting in a contusion to the top of the right foot.

The employee was standing on a platform attached to a track drill. As he moved the drill he backed it into the side of an attached air compressor which pinned his foot between the two units.

The employee was operating a metal lathe. A piece of metal shaving came toward the employee, which the employee deflected with his left hand resulting in a laceration to the middle finger of the left hand.

Employee was attempting to free a rock hang in a jaw crusher using a hook attached to an electric winch. The hook jumped back striking the employee on the arm and hard hat. That night he went to the local emergency room where a precautionary CAT scan was preformed, there were no injuries, the employee received an injection of pain medication.

Employee was using a power saw to cut the top off a plastic barrel. The blade stuck causing the saw to jump up and catch the little finger of the left hand which cut half the nail off.

THE EE WAS OPERATING A HYDRAULIC DRILLING MACHINE. HE WAS CHANGING THE STEEL DRILL ROD. DURING THE PROCESS HE ACTIVATED THE WRONG LEVER CAUSING THE DRILL TO ROTATE. THE ROTATION CUT AND BROKE THE SECOND FINGER OF THE LEFT HAND.

WELDING PROJECT LASTING ALL DAY - FLASH BURN TO EYES.

A FORKLIFT PULLING OUT A BLOCK FROM A LEDGE DRUG A SPRAWL WITH THE BLOCK WHICH TURNED OVER ON HIS FOOT.

HE STUMBLED AND TIRED TO CATCH HIS BALANCE WITH HIS LEFT HAND, THE TORCH IN THE RIGHT HAND WENT ACROSS THE BACK OF HIS LEFT HAND BURNING THE BACK OF THE LAST 3 FINGERS.

A SMALL BLOCK OF GRANITE BROKE INTO TWO PIECES AND ONE PIECE FELL BACK ON HIS FINGER.

A CABLE FROM THE CRANE PULLED HIS FEET OUT FROM UNDER HIM CAUSING HIM TO FALL Y

EE IS BANKER MAN FOR SAW SHEET CRANE. THEY WERE PICKING UP BLOCK AND EE GOT FINGERS BETWEEN CABLE AND BLOCK.

JACKHAMMER GOT STUCK IN DRILL HOLE. EMP HURT HIMSELF LIFTING IT FREE. NOTE: AT THIS TIME EMP HAS NOT MISSED ANY TIME .

FOUR 3EMPLOYEES WERE MOVING QUARRY DRILL TO A NEW LOCATION THE DRILL WAS LOWERED INTO POSITION WITH CRANE EMPLOYEE WAS INJURED BY PULLING ON DRILL TO GET IT TO REST ON AN EXACT SPOT HE PULLED STOMACH MUSCLE

EMPLOYEE WAS BANKERING FOR OVERHEAD CRANE. THE INJURY OCCURRED WHEN THE BLOCK HE WAS GUIDING CRUSHED AGAINT A BLOCK NEXT TO IT.

EMPLOYEE WAS USING A HAND HELD AIR DRILL THE DUST SUCTION CUP & HOSE WERE NOT PUSHED DOWN ALL THE WAY HIS PANT LEG CAUGHT UP IN THE DRILL STEEL & SQUEEZED HIS KNEE AS IT WOUND AROUND DRILL STE EL

EE CLIMBED INTO GANG SAW WHILE RUNNING TO UNSTOP HOSE. HE WAS NOT SUPOSED TO DO THAT. PROPER PROCEDURE IS LOCK OUT MACH. HE PLACED HIS HAND OR THE TRACK FOR THE MOVING SHOWER IN THE TOP OF THE SAW THE WHEELS OF THE SHOWER RAN OVER HIS HAND.

EMPLOYEE WAS CATCHING WOOD ETC OFF BELT TO FIRST CONE CRUSHER HE WAS RUNNING SCRAP SLAB MATERIAL FROM THE LSU FIMNISHING PLANT THIS MATERIAL HAD WAY TOO MUCH WOODINGS ETC IN WITH IT CAUSING TH E CONE CRUSHER OPERATOR A LOT MORE TROUBLE THAN NORMAL HE GOT CARELESS & PUT HIS HAND IN STHE WRONG PLACE

EMPL BROKE 2 FINGERS WHEN HE RAN HAND-HELD AIR-POWERED STONE TRACER OFF END OF ROCK.MASHED 2 FINGERS BETWEEN TRACER AND ROCK WITH HIS OWN BODY WEIGHT.

EMPLOYEE WAS CA6LKING A SEARRING WITH A GLUE GUN. (HAND POWERED GLUE GUN) THE HOSE ON THE GUN BURST AND SOME OF THE GLUE WENT INTO EMPLOYEE'S EYES. (HE HAD HIS GLASSES ON).

EMP WAS USING A3 ELECTRIC DRILL. THE DRILL BIT CAUGHT AND THE DRILL TWISTED HIS HAND INTO METAL BEAM

CRUSHER MAINT. 9MPLOYEE WAS HELPING REPLACE JAWS OF CRUSHER HE GAVE UP SIGNAL TO CRANE OPERATOR WITH HIS HAND ON CABLE HIS LITTLE FINGER WENT THROUGH PULLEY

EMPL MAKING RUBLE USING AIRPOWERED HANDTOOL THIS SLIPPED OFF EDGE OF STONE HAND WAS MASHED BETWEEN TOOL AND ROCK.

DRILLMAN WAS DRILLING CLAY OUT OF A 30HR OLD LIFT HOLE. HE BENT OVER TO PICK UP A LOOSE BOLT HE FOUND ON THE GROUND. AT THAT TIME THE DRILL BROKE THREW THE CLAY & RELEASED THE FUMES & SLATE AIR. DAZED BY THE FUMES HE FELL FACE DOWN.

HIS RIGHT HAND WAS PINCHED BY A SPLITTING MACH.
