Bolter was stuck. Employee started to climb up out of bolter compartment. Miner operator saw employee moving and lowered the continuous miner tail. Employee's hand was between bolter compartment and tail and was caught and lacerated. Employee received sutures.

Mostrando los 11Employee was using a cutting torch to repair railcar when a hot piece of metal popped and a small piece went in behind face shield and safety glasses and hit left eye. Employee went to the OCI Safety Department and had his eye flushed out with sterile water. Employee went to Eye Doctor after work and the doctor gave him some eye drops.
Employee was helping direct a motor which was being lifted by a 2/1 directional pulley rigging. Employee had left hand on stationery cable & right hand reaching for motor, when left pinky finger went into traveling pulley, removing tip of finger just below nail.
While drilling a separation station, employee was adding the 3rd steel. While pushing the steels in the hole, the top of the 3rd steel slipped off of the bottom of the second steel, causing both first and second steels to fall out and pinch employee's thumb between second steel and base of drill feed motor. Employee received laceration and fracture to thumb.
Employee was drilling gas holes with the bolter while standing on the continuous miner. While employee attempted to insert another auger, his foot slipped at the same time the bolter operator was sliding the bolter boom. Employee's foot was pinched on the bottom of mast of the bolter. Employee received medical attention and was unable to return to work.
Employee was closing valves on the evaporator body, when hot crystals overflowed to the floor, surrounding employee. Employee received burns on bottom of foot.
GLOVE WAS CAUGHT ON A BOLT AS THE MACHINE WAS ROTATING THE BOLT. EE'S RIGHT MIDDLE FINGER WAS CAUGHT AND FRACTURED.
Employee was kneeling down on decking drilling holes in side bearing plate. as he stood up, he felt pain in his right knee.
EMPLOYEE WAS ADJUSTING STRIKER PLATES IN UNIT 6 CRUSHER. AS HAMMERS CONTACTED STRIKER PLATES, EE ATTEMPTED TO TIGHTEN EYEBOLT. THE EYEBOLT WAS BROKEN OFF OF ADJUSTING ROD & VIBRATION CAUSED EE'S RIGHT INDEX FINGER LACERATION, SKIN GRAPH WAS PERFORMED ON FINGER.
EE WAS CHECKING FOR AN AIR LEAK ON THE BAGGING MACHINE WHEN HE WAS STRUCK BY A BAGGING ARM.
WHILE OPERATING THE BOLTER ON THE MINER, EE WAS REACHING OVER WITH HIS RIGHT HAND TO TURN THE WATER OFF WHEN HE FELT A PAIN IN HIS RIGHT SHOULDER. THE FOLLOWING DAY HE COMPLAINED OF A PAIN IN HIS SHOULDER. HE WAS THEN TAKEN TO THE HOSPITAL EVAULATION.
