Employee was loosening a bolt using an air gun and applied pressure with the injured hand (left) with his palm but the bolt was not fully inserted into the segment, consequently when air gun pressure is applied to the nut it spun the bolt and injured the left hand (palm).

EE WAS WORKING ON A JAW CRUSHER ALONG WITH OTHER WORKERS. THE TASK WAS TO REMOVE TIRES TO GET TO THE FROZEN BRAKES. WHILE EE WAS PICKING UP THE TIRE NUTS FROM THE GROUND, THE TIRE WAS DISLODGE D FROM THE HUB AND HIT HIM ON THE HEAD THEN THE LEFT SHOULDER.
